Background: Family members are often required to act as substitute decision-makers when health care or research participation decisions must be made for an incapacitated relative. Yet most families are unable to accurately predict older adult preferences regarding future health care and willingness to engage in research studies. Discussion and documentation of preferences could improve proxies' abilities to decide for their loved ones. This trial assesses the efficacy of an advance planning intervention in improving the accuracy of substitute decision-making and increasing (...) the frequency of documented preferences for health care and research. It also investigates the financial impact on the healthcare system of improving substitute decision-making.Methods/DesignDyads (n = 240) comprising an older adult and his/her self-selected proxy are randomly allocated to the experimental or control group, after stratification for type of designated proxy and self-report of prior documentation of healthcare preferences. At baseline, clinical and research vignettes are used to elicit older adult preferences and assess the ability of their proxy to predict those preferences. Responses are elicited under four health states, ranging from the subject's current health state to severe dementia. For each state, we estimated the public costs of the healthcare services that would typically be provided to a patient under these scenarios. Experimental dyads are visited at home, twice, by a specially trained facilitator who communicates the dyad-specific results of the concordance assessment, helps older adults convey their wishes to their proxies, and offers assistance in completing a guide entitled My Preferences that we designed specifically for that purpose. In between these meetings, experimental dyads attend a group information session about My Preferences. Control dyads attend three monthly workshops aimed at promoting healthy behaviors. Concordance assessments are repeated at the end of the intervention and 6 months later to assess improvement in predictive accuracy and cost savings, if any. Copies of completed guides are made at the time of these assessments.DiscussionThis study will determine whether the tested intervention guides proxies in making decisions that concur with those of older adults, motivates the latter to record their wishes in writing, and yields savings for the healthcare system.Trial RegistrationISRCTN89993391. Science has always been tightly associated with environmental ethics in a way traditional ethics has not. However, despite this proximity, science has had a merely informational role, where it must inform ethics but not intervene in ethical judgment. Science is seen as an amoral enterprise, requiring an ethics rather than recommending one. In this paper I try to go against this common view. First, I give a critique of the naturalistic fallacy following the lines of Frankena. Then I go on (...) to describe the two possible roles science can have in ethical though, and in environmental ethics in particular. As it turns out, science does not only inform ethics, but can actually have moral import and intervene in moral judgment. Finally, from an ecocentric point of view, I try to illustrate this last point by construing the ecological notion of resilience as a moral boundary—a scientifically determined boundary between right and wrong. In the present work, I will review how Aristotle understood the connection between reason and emotion - particularly, angry actions - in order to demonstrate that it is due to the presence of intellectual factors that emotions become ethically relevant and not merely an uncontrolled reaction. Then, I will summarize Aristotle's repeated analogies between reason as the master and anger as the slave to explain their connection. My specific contribution to the topic will be to reverse this analogy and, instead (...) of using it as an example of the _lógos-thymós_ connection, make the latter an example of the _despótes-doûlos_ connection as described in Politics I. If my hypothesis is plausible, establishing the actions and virtues of a slave will help shed light on the type of morality inherent in angry actions. (shrink)

The meanings of utterances and thoughts are commonly regarded in philosophical semantics as abstract objects, called «propositions», which account for how different utterances and thoughts can be synonymous and which constitute the primary truth-bearers. I argue that meanings are instead natural properties that play causal roles in the world, that the kind of «One over Many» thinking underlying the characterization of shared meanings as abstract objects is misguided and that utterances and thoughts having truth-values in virtue of their meanings does (...) not entail that meanings themselves are truth-bearers. In recent years, the integration of philosophical with scientific theorizing has started to yield new insights. This chapter surveys some recent philosophical and empirical work on the nature and structure of action, on conscious agency, and on our knowledge of actions.

Mobsters and others engaged in risky forms of social coordination and coercion often communicate by saying something that is overtly innocuous but transmits another message ‘off record’. In both ordinary conversation and political discourse, insinuation and other forms of indirection, like joking, offer significant protection from liability. However, they do not confer blanket immunity: speakers can be held to account for an ‘off record’ message, if the only reasonable interpreta- tions of their utterance involve a commitment to it. Legal liability (...) for speech in the service of criminal behavior displays a similar profile of significant protection from indirection along with potential liability for reasonable interpretations. Specifically, in both ordinary and legal contexts, liability depends on how a reasonable speaker would expect a reasonable hearer to interpret their utterance in the context of utter- ance, rather than on the actual speaker’s claimed communicative intentions. Hacia la autogestión feminista, María Medina-Vicent aborda desde una perspectiva crítica feminista los discursos gerenciales dirigidos a las mujeres, desvelando el androcentrismo y la presencia —y perpetuación— de tradicionales roles y estereotipos de género en los modelos de gestión. Mi propuesta pretende incidir en dos aspectos del análisis realizado por Medina-Vicent, por un lado, remarcar los peligros de la despolitización de los discursos gerenciales dirigidos a mujeres —sobre todo en tanto que la (...) literatura gerencial acaba por informarnos a todas—. Por otro, profundizar en la crítica que la autora realiza de la cooptación del feminismo por parte de estos discursos.: In her book Mujeres y discursos gerenciales. Hacia la autogestión feminista, Medina-Vicent approaches management discourses aimed at women from a critical feminist perspective, revealing the androcentrism and the presence and perpetuation of traditional gender roles and stereotypes in management models. My proposal aims to highlight two aspects of Medina-Vicent’s analysis: on the one hand, to highlight the dangers of depoliticizing management discourses aimed at women - especially insofar as management literature ends up informing us all. On the other hand, the author’s critique of the co-optation of feminism by these discourses will be explored in greater depth. Readings in Health Care Ethics provides a wide-ranging selection of important and engaging contributions to the field of health care ethics. The second edition adds a chapter on health care in Canada, and the introduction has been expanded to include discussion of a new direction in feminist naturalized ethics. The book presupposes no prior knowledge, only an interest in the bioethical issues that are shaping our world.

We sley Salmon, in his influential and detailed book, Four Decades of Scientific Explanation, argues that the pragmatic approach to scientific explanation, “construed as the claim that scientific explanation can be explicated entirely in pragmatic terms” (1989, 185) is inadequate. The specific inadequacy ascribed to a pragmatic account is that objective relevance relations cannot be incorporated into such an account. Salmon relies on the arguments given in Kitcher and Salmon (1987) to ground this objection. He also suggests that Peter Railton’s (...) concepts of the ideal explanatory text and explanatory information (Railton 1981) can provide what the pragmatic approach lacks. This suggestion is not a conclusion of course; we read it as the promotion of part of a research program aimed at forging a greater consensus on scientific explanation-an admirable goal. However, we do not see the pragmatic approach as inadequate. We will show that a synthetic account inspired by Salmon’s adaptation of Railton would be equivalent to van Fraassen’s pragmatic account in three respects: accepting or rejecting requests for explanation; the practice of giving scientific explanations; and the evaluation of the goodness of explanations. We include all three under the general rubric of explanatory “practice.” Admittedly these are not the only three features by which an account of explanation might be evaluated. Roughly, we mean to show that a synthetic account cannot do a better job of accounting for the scientific practices which are of importance to the constructive empiricist, and therefore no argument can be presented to the constructive empiricist to convince her that by her own standards the synthetic account is superior. A large part of technological advances, especially in the field of industry, have been focused on the optimization of productive processes. However, the detection of anomalies has turned out to be a great challenge in fields like industry, medicine or stock markets. The present work addresses anomaly detection on a control level plant. We propose the application of different intelligent techniques, which allow to obtain one-class classifiers using real data taken from the correct plant operation. Ethnomethodology criticises sociological objectivism in a double sense: a) concerning the idea of “objectively” given social facts; b) concerning the idea of objectivity as a realistic claim of common sense and scientific knowledge. The theoretical alternative presented by Garfinkel and his followers consists a) in an analysis of the interpretative procedures, by which common sense beliefs in the objectivity of reality are constituted; b) in the intention, to take practical reasoning not as a source, but as a topic of empirical (...) study. The paper argues that while the proposed analysis of practical activities conveys a useful approach, its epistemological implications lead to inconsistencies and problematical consequences. (shrink)

Nowadays, the quality standards of higher education institutions pay special attention to the performance and evaluation of the students. Then, having a complete academic record of each student, such as number of attempts, average grade and so on, plays a key role. In this context, the existence of missing data, which can happen for different reasons, leads to affect adversely interesting future analysis. Therefore, the use of imputation techniques is presented as a helpful tool to estimate the value of missing (...) data. This work deals with the academic records of engineering students, in which imputation techniques are applied. More specifically, it is assessed and compared to the performance of the multivariate imputation by chained equations methodology, the adaptive assignation algorithm based on multivariate adaptive regression splines and a hybridization based on self-organisation maps with Mahalanobis distances and AAA algorithm. The results show that proposed methods obtain successfully results regardless the number of missing values, in general terms. (shrink)

Objective: To assess the knowledge of four groups of individuals regarding who is legally authorised to consent to health care or research involving older patients.Design: A provincewide postal survey.Setting: Province of Quebec, Canada.Participants: Three hundred older adults, 434 informal caregivers of cognitively impaired individuals, 98 researchers in aging and 136 members of research ethics boards .Measurements: Knowledge was assessed through a pretested postal questionnaire comprising five vignettes that describe hypothetical situations involving an older adult who requires medical care or is (...) solicited for research. The respondent had to identify the person who is legally authorised to provide consent.Results: Nearly 80% of all respondents provided the correct answer when the hypothetical scenario depicted a person who was competent to consent or incompetent but legally represented. Knowledge was worse for the scenario describing a research situation that involved an incompetent adult without a legal guardian.Conclusion: The observed lack of knowledge raises doubts about the ability of current legislation to truly protect the rights of older adults with diminished decision making capacity. It points to the need for educational programmes aimed at increasing public awareness of the legislation put in place for those requiring special protection. (shrink)
