We use Homer and Sun Tzu as a background to better understand and reformulate confrontation, anger and violence in medicine, contrasting an unproductive ‘love of war’ with a productive ‘art of war’ or ‘art of strategy’. At first glance, it is a paradox that the healing art is not pacific, but riddled with militaristic language and practices. On closer inspection, we find good reasons for this cultural paradox yet regret its presence. Drawing on insights from Homer's The Iliad and The (...) Odyssey, we argue for better understanding of confrontation, anger, bullying, intimidation and violence in medicine in order to change the culture. For example, equating medicine with war is not a given condition of medicine but a convenient metaphor with historical origins and a historical trajectory. Other, non-martial metaphors, such as medicine as collaboration, may be more appropriate in an age of team-based care. Taking lessons from Homer, we suggest three key ways in which cold-hearted confrontation and anger in medicine can be transformed into productive, warm-hearted engagement: the transformation of angry impulse into reflection, moral courage and empathy. Thinking with Homer can offer an aesthetically and morally charged alternative to the current body of literature on topics, such as anger in doctors, and how this may be ‘managed’, without recourse to an instrumental economy where emotions are viewed as commodities, and emotional responses can be ‘trained’ through communication skills courses. We report the results of a randomized trial to assess the impact of an innovative ethics curriculum on the knowledge and confidence of 85 medical house officers in a university hospital programme, as well as their responses to a simulated clinical case. Twenty-five per cent of the house officers received a lecture series, 25 per cent received lectures and case conferences, with an ethicist in attendance, and 50 per cent served as controls. A post-intervention questionnaire was administered. Knowledge scores did (...) not differ among the groups. Confidence regarding ethical issues was significantly greater in the aggregate intervention group compared to the control group. Confidence regarding procedural issues related to ethics was significantly higher for the EI group than for the controls. Responses to a simulated case showed that significantly fewer house officers in the EI group would intubate a patient for whom such therapy would be futile. We conclude that ethics education can have an impact on house officers' confidence and their responses to a simulated case, and that the EI was more effective than the LI. Such results have implications regarding the implementation of ethics education during residency. (shrink)

Yet behavioral decision‐making data suggest a contrary biological drive to minimize cognitive effort by solving problems using simplifying heuristics. Individuals differ, and the same person varies over time, in the strength of the knowledge instinct. Neuroimaging studies suggest which brain regions might mediate the balance between knowledge expansion and heuristic simplification. One region (...) implicated in primary emotional experience is more activated in individuals who use primitive heuristics, whereas two areas of the cortex are more activated in individuals with a strong knowledge drive: one region implicated in detecting risk or conflict and another implicated in generating creative ideas. Knowledge maximization and effort minimization are both evolutionary adaptations, and both are valuable in different contexts. Effort minimization helps us make minor and routine decisions efficiently, whereas knowledge maximization connects us to the beautiful, to the sublime, and to our highest aspirations. We relate the opposition between the knowledge instinct and heuristics to the biblical story of the fall, and argue that the causal scientific worldview is mathematically equivalent to teleological arguments from final causes. A theoretical structure for multiattribute decision making is presented, based on a dynamical system for interactions in a neural network incorporating affective and rational variables. This enables modeling of problems that elude two prevailing economic decision theories: subjective expected utility theory and prospect theory. The network is unlike some that fit economic data by choosing optimal weights or coefficients within a predetermined mathematical framework. Rather, the framework itself is based on principles used elsewhere to model many other cognitive and behavioral (...) data, in a manner approximating how humans perform behavioral functions. Different, interconnected modules within the network encode (a) attributes of objects among which choices are made, (b) object categories, (c) and goals of the decision maker. An example is utilized to simulate the actual consumer choice between old and new versions of Coca-Cola. Potential applications are also discussed to market decisions involving negotiations between participants, such as international petroleum traders. (shrink)

Contrary to common perceptions of Simmel’s work as dividing into three stages of Darwinism, Kantianism, and Goethean/Bergsonian Life-Philosophy, consideration of the full scope of the Georg Simmel Gesamtausgabe demonstrates Simmel’s concern with both Kant and Goethe as life-long, just as was his engagement with core principles respectively associated with them: Form and Life. What changed in his mind over time was how those two principles were construed and related. In this view, Simmel’s Soziologie can be read as a treatise on (...) the properties of social forms that emerge out of vital processes – following a dynamic akin to Blumenbach’s Bildungstrieb, whereas the Lebensanschauung puts more emphasis on the ways in which forms channel, constrain, and inspire the streams of life. This opens the door for creative work pursuing lines of connection between the two disciplines – a sociology of authentic individuality and a kind of existentialized sociology – and, beyond that, to a unified theory of action and evolution. Although Parsons encountered the works of both Simmel and Weber during his stay at Heidelberg in the late 1920s, his appropriation of the two became increasingly asymmetrical, issuing in a lifelong devotion to Weber and a pronounced disavowal of Simmel around the time Parsons published The Structure of Social Action. This reaction deprived Parsons of the substantial support he could have found in Simmel's work for his effort to counteract `utilitarian' theories of action. Simmel not only went beyond Parsons in (...) revealing a number of perspectives that demonstrate the permeation of action by moral orientations, but he counteracted economistic thinking along a number of other dimensions. Protection of civilians has become enshrined as a core task for international peacekeeping missions. How to ensure that civilians are safe from violence and human rights abuses is central to developing military doctrine for peacekeeping; how safe civilians are from attack is central to how peacekeeping missions are assessed both by locals and international observers. However, protection of civilians is often seen as something that is done by active peacekeepers on behalf of passive civilians, potentially missing the ways in which (...) peacekeepers’ actions interact with strategies that civilians undertake on their own behalf. Integrating peacekeeper and civilian self-protection strategies is not trivial, either from a practical or a moral standpoint. Drawing on primary research among women in Liberia, as well as case studies of civilian protection elsewhere, this essay examines the ways in which working with civilians on protection—creating ‘hybrid’ systems of protection—inevitably entangles peacekeepers in civilians’ other social, political, and moral concerns, undermining at least a naïve impartiality. To retain their moral stance, peacekeepers ought to focus on using the safety they provide to allow different local actors to interact safely and, ideally, constructively. (shrink)

Respect for victims requires that we have social systems for punishing and condemning (reproving) serious crimes. But, the conditions of social marginalization and political subordination of the communities from which an overwhelming number of prisoners in the United States come place serious barriers in the face of effective reprobation. Mass incarceration makes this problem worse by disrupting and disrespecting entire communities. While humanities education in the prisons is far from a total solution, it is one way to make reprobation meaningful, (...) so long as the prison classroom is a place where the educators’ values are also put at risk. Famously deemed “the brightest man in Europe” by George Santayana, Simmel addressed a variety of topics across his essayistic writings, which have influenced scholars in aesthetics, ethics, epistemology, and sociology. Nevertheless, a set of core issues emerged over the course of his career, most centrally the genesis, structure, and transcendence of social and cultural forms and the nature and genesis of authentic individuality. In much of the developing world daughters receive lower education and other investments than do their brothers, and may even be so devalued as to suffer differential mortality. Daughter disadvantage may be due in part to social norms that prescribe that daughters move away from their natal family upon marriage, a practice known as virilocality. We evaluate the effects of virilocality on female disadvantage using data from the Indonesia Family Life Survey. We find little support for the hypothesis. There is (...) no evidence that the overall pattern of rough equality in the treatment of boys and girls in Indonesia masks differences according to post-marital residential practice. Virilocal groups do not have "missing daughters." Nor is there other evidence of son preference, such as in relatively low height-for-age or education for girls and women in virilocal areas. Explanations of daughter disadvantage as due to virilocality should be subject to further scrutiny and contextualization. (shrink)

The shape our personalities and life experiences take depends on a process. At the outset of this process, the self is, in a sense, a stranger; both to us and to others. The more this is the case, the greater the openness of the process of self-formation to a kind of freedom, which is the (...) freedom from predetermination of its outcome. In his final work, Donald N. Levine, one of the great late-twentieth-century sociological theorists, brings together diverse social thinkers. Simmel, Weber, Durkheim, Parsons, and Merton are set into a dialogue with philosophers such as Hobbes, Smith, Montesquieu, Comte, Kant, and Hegel and pragmatists such as Peirce, James, Dewey, and McKeon to describe and analyze dialogical social theory. This volume is one of Levine's most important contributions to social theory and a worthy summation of his life's work. Levine demonstrates that approaching (...) social theory with a cooperative, peaceful dialogue is a superior tactic in theorizing about society. He illustrates the advantages of the dialogical model with case studies drawn from the French Philosophes, the Russian Intelligentsia, Freudian psychology, Ushiba's aikido, and Levine's own ethnographic work in Ethiopia. Incorporating themes that run through his lifetime's work, such as conflict resolution, ambiguity, and varying forms of social knowledge, Levine suggests that while dialogue is an important basis for sociological theorizing, it still vies with more combative forms of discourse that lend themselves to controversy rather than cooperation, often giving theory a sense of standing still as the world moves forward. The book was nearly finished when Levine died in April 2015, but it has been brought to thoughtful and thought-provoking completion by his friend and colleague Howard G. Schneiderman. The authors regard opiates as the primary neural substrate for social attachment, and peptide hormones as subsidiary. One may instead conclude from their evidence that oxytocin, vasopressin, and opiates play complementary roles in attachment. Oxytocin and vasopressin relate to different aspects of emotional experience, and opiates to quiescence from long-term attachment. This is related to intimacy versus affiliation.

The authors wish that the psychology of human decision making should borrow methodological rigor from economics. However, unless economics also borrows from psychology this poses a danger of overly limiting the phenomena studied. In fact, an expanded economic theory should be sought that is based in psychology (and ultimately neuroscience) and encompasses both rational and irrational aspects of decision making.

No topic could be more relevant in these times than tyranny, “the greatest sickness of the soul.” The Charmides of Plato gives us an opportunity to look deeply into the soul or cognitive structure of one of Athens’s most notorious tyrants, Critias, and looks deeply into its dialectical opposite, the soul and cognitive structure of Socrates.
