BackgroundRandomized controlled trials (RCTs) are often complex and expensive to perform. Less than one third achieve planned recruitment targets, follow-up can be labor-intensive, and many have limited real-world generalizability. Designs for RCTs conducted using cohorts and routinely collected health data, including registries, electronic health records, and administrative databases, have been proposed to address these challenges and are being rapidly adopted. These designs, however, are relatively recent innovations, and published RCT reports often do not describe important aspects of their methodology in (...) a standardized way. Our objective is to extend the Consolidated Standards of Reporting Trials (CONSORT) statement with a consensus-driven reporting guideline for RCTs using cohorts and routinely collected health data.MethodsThe development of this CONSORT extension will consist of five phases. Phase 1 (completed) consisted of the project launch, including fundraising, the establishment of a research team, and development of a conceptual framework. In phase 2, a systematic review will be performed to identify publications (1) that describe methods or reporting considerations for RCTs conducted using cohorts and routinely collected health data or (2) that are protocols or report results from such RCTs. An initial “long list” of possible modifications to CONSORT checklist items and possible new items for the reporting guideline will be generated based on the Strengthening the Reporting of Observational Studies in Epidemiology (STROBE) and The REporting of studies Conducted using Observational Routinely-collected health Data (RECORD) statements. Additional possible modifications and new items will be identified based on the results of the systematic review. Phase 3 will consist of a three-round Delphi exercise with methods and content experts to evaluate the “long list” and generate a “short list” of key items. In phase 4, these items will serve as the basis for an in-person consensus meeting to finalize a core set of items to be included in the reporting guideline and checklist. Phase 5 will involve drafting the checklist and elaboration-explanation documents, and dissemination and implementation of the guideline.DiscussionDevelopment of this CONSORT extension will contribute to more transparent reporting of RCTs conducted using cohorts and routinely collected health data. Cet article se fonde sur un retour d’expérience auprès d’un nouveau-né présentant une maladie invalidante, lors de ses séjours en service de néonatologie au CHU d’Amiens puis de son hospitalisation à domicile. L’équipe de soins psychiques périnataux accompagne la famille dans le cadre d’une application thérapeutique de l’observation du nourrisson selon la méthode Esther Bick. Initié à l’hôpital et se continuant au domicile, ce travail a porté sur la restauration de la fonction parentale, celle-ci permettant la construction d’une peau psychique (...) relationnelle et le déploiement d’une qualité relationnelle étonnante et particulièrement vivante. From the start, I followed the case of Jahi McMath with great interest. In December 2013, she clearly fulfilled the diagnostic criteria for brain death. As a neurologist with a special interest in chronic brain death, I was not surprised that, after she was flown to New Jersey, where she became statutorily resurrected and was treated as a comatose patient, Jahi's condition quickly improved. In 2014, her family reported that she sometimes responded to simple motor commands. I shared the general (...) skepticism regarding these reports, assuming that the family was in denial and was misinterpreting spinal myoclonus (a rapid, involuntary twitch generated by the spinal cord) as volitional. The family had noticed that when Jahi's heart rate was above eighty beats per minute, she was more likely to respond, as though the heart rate reflected some sort of inner level of arousal. So they began to make video recordings. I have been privileged to be entrusted with copies of these recordings, forty‐eight of which proved suitable for assessing alleged responsiveness. All have been certified by a forensic video expert as unaltered. The first thing that struck me was that the great majority of the alleged responses were not spinal myoclonus. In fact, they did not resemble any type of spontaneous, involuntary movement described in patients paralyzed from high spinal cord lesions. (shrink)

Endothelial cells, when cultured on gelled basement membrane matrix exert forces of tension through which they deform the matrix and at the same time they aggregate into clusters. The cells eventually form a network of cord-like structures connecting cell aggregates. In this network, almost all of the matrix has been pulled underneath the cell cords and cell clusters. This phenomenon has been proposed as a possible model for the growth and development of planar vascular systems in vitro. Our hypothesis (...) is that the matrix is reorganized and the cellular networks form as a result of traction forces exerted by the cells on the matrix and the latter's elasticity. We construct and analyze a mathematical model based on this hypothesis and examine conditions necessary for the formation of the pattern. We show cell migration is not necessary for pattern formation and that isotropic, strain-stimulated traction is sufficient to form the observed patterns. (shrink)

Black Panther draws from a long tradition of Afrofuturist literature that is influenced by a desire to remedy (...) the injuries of the past and reconnect people of African descent with the continent from which they've been severed. Black Panther 's Afrofuturism reimagines antebellum slavery society's flight fantasy as a powerful science fiction, T'Challa's Royal Talon Flyer. The narratives of T'Challa, Shuri, and Wakanda have been continuously remolded and expanded upon since the 1960s by artists who have been inspired by other Afrofuturist works. (shrink)

I was recently invited to talk about ethics with the staff of a level‐three neonatal intensive care unit. They presented a case featuring a full‐term baby born by emergency caesarean‐section after a cord prolapse that caused prolonged anoxia. Her initial pH was 6.7. She was intubated and resuscitated in the delivery room. Her Apgar score remained at 1 for ten minutes. Further evaluation over the next two days revealed severe brain damage. Her prognosis was dismal.The doctors recommended a do‐not‐resuscitate order. (...) The parents agreed. The doctors suggested a gastrostomy tube. The parents disagreed. Instead, they requested that fluid and nutrition be discontinued. The emotions in the room, as we started to discuss the case, were strong and discordant. Eventually, the conversation wound down. They turned, expectantly, to the visiting bioethicist. (shrink)

It has been 35 years since the publicationMelzack and Wall's Gate Control Theory whichhypothesized that nociceptive information wassubject to dynamic regulation by mechanismslocated in the spinal cord dorsal horn thatcould ultimately lead to hyperalgesic orhypoalgesic states. This paper examines GateControl Theory in light of our currentunderstanding of the neuroanatomical,neurophysiological and neurochemical substratesof nociception and antinociception. Despiteits initial controversies, no one has proposeda more comprehensive overall theory of painmodulation or has successfully refuted most ofthe basic tenets of this theory.

Adenosine transport in cultured human umbilical vein endothelial cells (HUVEC) was characterized and shown to be mediated by a single facilitated diffusion mechanism. Initial rates of adenosine influx at 22 degrees C were saturable [apparent Michaelis constant, 69 +/- 10 mu M; maximum velocity (V-max), 600 +/- 70 pmol.10(6) cells(-1).s(-1)] and inhibited by nitrobenzylthioinosine (NBMPR). Formycin B had an unusually high affinity [inhibitory constant K-i), 18 +/- 4.3 mu M], whereas inosine had a low affinity (K-i, 440 +/- 68 mu (...) M) and nucleobases were without effect on adenosine influx. The number of transporters (1.2 x 10(6) sites/cell) was estimated by NBMPR equilibrium binding (apparent dissociation constant, 0.11 +/- 0.01 nM; maximum binding, 2.0 +/- 0.15 pmol/10(6) cells). In addition, we compared these endothelial cells with those obtained from cords from pregnancies complicated by diabetes (HUVEC-D), since embriopathy may occur in these conditions. HUVEC-D exhibited a 2.3-fold reduction in both the V-max for adenosine influx and the maximum number of NBMPR binding sites (260 +/- 40 pmol.10(6) cells(-1).s(-1) and 0.86 +/- 0.08 pmol/10(6) cells, respectively). However, the turnover number for each nucleoside transporter in normal and diabetic HUVEC was similar (approximate to 300 adenosine molecules/s). Adenosine metabolism at 10 mu M in HUVEC-D was modified compared with normal cells. Intracellular phosphorylation (> 90%) was the predominant pathway in normal HUVEC, whereas in HUVEC-D, substantial levels of adenine and adenosine were detected. The present results demonstrate therefore the downregulation of the NBMPR-sensitive nucleoside transporter and changes in adenosine metabolism in HUVEC from diabetic pregnancies. (shrink)

Both bosons and fermions satisfy a strong version of Leibniz’s Principle of the Identity of Indiscernibles (PII), and so are ontologically on a par with respect to the PII. This holds for non-entangled, non-product states and for physically entangled states—as it has been established in previous work. In this paper, the Leibniz strategy is completed by including the (bosonic) symmetric product states. A new understanding of Pauli’s Exclusion Principle is provided, which distinguishes bosons from fermions in a peculiar ontological way. (...) BackgroundFunctional changes after spinal cord injury are related to changes in cortical plasticity. These changes can be measured with electroencephalography and has potential to be used as a clinical biomarker.MethodIn this longitudinal study participants underwent a total of 30 sessions of robotic-assisted gait training over a course of 6 weeks. The duration of each session was 30 min. Resting state EEG was recorded before and after 30-session rehabilitation therapy. To measure gait, we used the Walking Index for Spinal Cord Injury (...) Scale, 10-Meter- Walking Test, Timed-Up-and-Go, and 6-Min-Walking Test. Balance was measured using Berg Balance Scale.ResultsFifteen participants with incomplete SCI who had AIS C or D injuries based on American Spinal Cord Injury Association Impairment Scale classification were included in this study. Mean age was 35.7 years and the mean time since injury was 17.08 months. All participants showed clinical improvement with the rehabilitation program. EEG data revealed that high beta EEG activity in the central area had a negative correlation with gait and balance measured at baseline, in a way that greater high beta EEG power was related to worse clinical function at baseline. Moreover, improvement in gait and balance had negative correlations with the change in alpha/theta ratio in the parietal area.ConclusionIn SCI, functional impairment and subsequent improvement following rehabilitation therapy with RAGT correlated with the change in cortical activity measured by EEG. Our results suggest that EEG alpha/theta ratio may be a potential surrogate marker of functional improvement during rehabilitation. Future studies are necessary to improve and validate these findings as a neurophysiological biomarker for SCI rehabilitation. (shrink)

L'inscription latine qui l'accompagne suggère que la (...) défunte, louée pour son dévouement aux Muses, était vraisemblablement une musicienne de métier. Nous sommes en présence d'un cas très rare où l'inscription sert de légende pour expliquer le décor sculpté de la tombe. (shrink)

Causal processes can give rise to distinctive distributions in the linguistic variables that they affect. Consequently, a secure understanding of a variable's distribution can hold a key to understanding the forces that have causally shaped it. A storied distribution in linguistics has been Zipf's law, a kind of power law. In the wake of a major debate in the sciences around power-law hypotheses and the unreliability of earlier methods of evaluating them, here we re-evaluate the distributions claimed to characterize phoneme (...) frequencies. We infer the fit of power laws and three alternative distributions to 166 Australian languages, using a maximum likelihood framework. We find evidence supporting earlier results, but also nuancing them and increasing our understanding of them. Most notably, phonemic inventories appear to have a Zipfian-like frequency structure among their most-frequent members (though perhaps also a lognormal structure) but a geometric (or exponential) structure among the least-frequent. We compare these new insights the kinds of causal processes that affect the evolution of phonemic inventories over time, and identify a potential account for why, despite there being an important role for phonetic substance in phonemic change, we could still expect inventories with highly diverse phonetic content to share similar distributions of phoneme frequencies. We conclude with priorities for future work in this promising program of research. (shrink)

The method of explication has been somewhat of a hot topic in the last 10 years. Despite the multifaceted research that has been directed at the issue, one may perceive a lack of step-by-step procedural or structural accounts of explication. This paper aims at providing a structural account of the method of explication in continuation of the works of Geo Siegwart. It is enhanced with a detailed terminology for the assessment and comparison of explications. The aim is to provide means (...) to talk about explications including their criticisms and their interrelations. There is hope that this treatment will be able to serve as a foundation to a step-by-step guide to be established for explicators. At least it should help to frame and mediate explicative disputes. In closing the enterprise will be considered an explication of ‘explication’, though consecutive explications improving on this one are undoubtedly conceivable. (shrink)

The paper addresses the referring problem in quantum mechanics, by spelling out the alternatives with complete or individual concepts, with directly referential labels, and with intuition. The connection between the way of referring and some metaphysical theses about objects will be explained. Then, the paper intends to make plausible that the Kantian way is the best way for the quantum domain, including quantum field theories.

At first glance, every metaphysics of laws can be combined with every ontology of time. In contrast, the paper intends to show that Humeanism requires eternalism and that Power metaphysics must presuppose an existentially dynamical view of temporal existence, i.e. growing block or presentism. The presented arguments turn out to be completely independent of whether the laws of nature are deterministic or probabilistic: the world is non-productive and static or productively dynamical, the future be ‘open’ or not.

This encyclopedia article provides a procedural account of explication outlining each step that is part of the overall explicative effort (2). It is prefaced by a summary of the historical development of the method (1). The latter part of the article includes a rough structural theory of explication (3) and a detailed presentation of an examplary explication taken from the history of philosophy and the foundations of mathematics (4).

This paper contributes to the calculization of evocation and erotetic implication as defined by Inferential Erotetic Logic (IEL). There is a straightforward approach to calculizing (propositional) erotetic implication which cannot be applied to evocation. First-order evocation is proven to be uncalculizable, i.e. there is no proof system, say FOE, such that for all X, Q: X evokes Q iff there is an FOE-proof for the evocation of Q by X. These results suggest a critique of the represented approaches to calculizing (...) IEL. This critique is expanded into a programmatic reconsideration of the IEL-definitions of evocation and erotetic implication. From a different point of view these definitions should be seen as desiderata that may or may not play the role of a point of orientation when setting up "rules of asking". (shrink)

We argue that the need for commentary in commonly used linear calculi of natural deduction is connected to the “deletion” of illocutionary expressions that express the role of propositions as reasons, assumptions, or inferred propositions. We first analyze the formalization of an informal proof in some common calculi which do not formalize natural language illocutionary expressions, and show that in these calculi the formalizations of the example proof rely on commentary devices that have no counterpart in the original proof. We (...) then present a linear natural deduction calculus that makes use of formal illocutionary expressions in such a way that unique readability for derivations is guaranteed – thus showing that formalizing illocutionary expressions can eliminate the need for commentary. Kant’s ontology of appearances implies that the numerical distinctness of empirical objects is grounded in their appearance-aspect, more precisely in space as pure intuition, in which alone such objects can be given. With distinguishing concepts things can only be thought: in contrast to Leibniz’s complete concepts and to Kripke’s rigid designators, Kant’s general concepts do not entail their referents analytically. They must be applied to intuition, i. e. be completed synthetically. Consequently, Kant’s ontology of merely singular individuals is closely connected (...) with a genuine semantics of synthetic reference via intuition, expressed by irreducible demonstratives such as “this”, “here”, and “now”. Accordingly, the judgment “There can be indiscernibles” is synthetic-apriori, which distinguishes Kant’s view both from skeptical empiricism and from heavyweight ontological realism. (shrink)

The traditional use of the expression 'pseudoproblem' is analysed in order to clarify the talk of pseudoproblems and related phenomena. The goal is to produce a philosophically serviceable terminology that stays true to its historical roots. This explicative study is inspired by and makes use of the method of logical reconstruction. Since pseudoproblems are usually expressed by pseudoquestions a formal language of questions is presented as a possible reconstruction language for alleged pseudoproblems. The study yields an informal theory of pseudoproblems (...) whose presuppositions are critically investigated right away. At least one result remains: Claims of pseudoproblemship and their refutations must not be voiced casually - they are to be relativized and need substantial interpretive effort. (shrink)

Questions are everywhere and the ubiquitous activities of asking and answering, as most human activities, are susceptible to failure – at least from time to time. This volume offers several current approaches to the systematic study of questions and the surrounding activities and works toward supporting and improving these activities. The contributors formulate general problems for a formal treatment of questions, investigate specific kinds of questions, compare different frameworks with regard to how they regulate the activities of asking and answering (...) of questions, and situate these activities in a wider framework of cognitive/epistemic discourse. From the perspectives of logic, linguistics, epistemology, and philosophy of language emerges a report on the state of the art of the theory of questions. (shrink)

Obwohl sich die Quantentheorie durch spektakuläre empirische Erfolge auszeichnet, wird bis heute kontrovers diskutiert, wie sie zu verstehen ist. In diesem Werk geben die Autoren einen Überblick über die zahlreichen philosophischen Herausforderungen: Verletzen Quantenobjekte das Prinzip der Kausalität? Sind gleichartige Teilchen ununterscheidbar und daher keine Individuen? Behalten Quantenobjekte in der zeitlichen Entwicklung ihre Identität? Ein Ausblick in die Quantenfeldtheorie verschärft das Hauptproblem der Nichtlokalität. Philosophie der Quantenphysik richtet sich an Philosophen mit Interesse für Physik, macht Physiker mit den philosophischen Fragen ihres Faches vertraut und liefert Anregungen für den gymnasialen Physik-Unterricht. Das Buch schließt damit eine Lücke zwischen populären Einführungen und spezialisierten Monografien zur Philosophie der Quantenphysik. Pseudoproblems, pseudoquestions, pseudosentences (etc.) constitute an iridescent group of concepts which were prominently used by the Vienna Circle (including Wittgenstein). In the course of an explication this paper presents a compilation of the many different meanings that were given to these expressions. This includes the more prominent Viennese approaches as well as a more recent one by Roy Sorensen. A novel proposal concerning the use ofthe term is made, suggesting that nothing is just a pseudoproblem, but only relative to a (...) certain state of discourse. While the paper follows an explicative methodology, several uses of 'pseudoproblem', including the explicated one, relate pseudoproblemhood to other kinds of analysis. (shrink)

So far, most of the philosophical literature on occupations has tried to assess the legitimacy of military rule in the aftermath of armed conflicts by exclusively employing the theoretical resources of just war theory. In this paper, I argue that this approach is mistaken. Occupations occur during or in the aftermath of wars but they are fundamentally a specific type of rule over persons. Thus, theories of political legitimacy should be at least as relevant as just war theory for the (...) moral evaluation of occupations. This paper, therefore, draws on both traditions and argues that just war theory plays a limited role in identifying the purposes and appropriate agents of occupation authority, but that theories of legitimacy are necessary for explaining why and under which conditions foreign actors have the right to rule in the aftermath of armed conflicts. Kant rejects Leibniz’s Principle of the Identity of Indiscernibles. In quantum mechanics, Leibniz’s principle is also apparently violated. However, both ways of rejecting the PII differ significantly. In particular, Kant denies that spatiotemporal objects are unique individuals and establishes appearances as merely singular ones. The distinction between ‘unique’ and ‘singular’ individuals is crucial for the role that intuition plays in cognition: it will be shown that Kant’s way of rejecting the PII goes against the standard versions of conceptualism and non-conceptualism (...) which, in turn, points out the relevance of this issue for the understanding of transcendental idealism. Finally, the systematic relevance will be checked by defending a Kantian interpretation of quantum individuality. (shrink)

Frege's philosophy of language includes detailed views on judgments. His formal logic - the Begriffsschrift - documents some of these views in the introduction and treatment of the judgment stroke. In current logic such an expression is either entirely ignored or, appearing as turnstile, plays an fundamentally different role. In this paper I put forward four claims: (i) Considering Frege's Begriffsschrift, it is methodologically palpable why the judgment stroke was omitted in nearly all logical systems developed after Frege. (ii) The (...) Frege-style inclusion of the judgment stroke in a formal language represents a partial implementation of results from speech act theory. (iii) A more comprehensive implementation, not limited to the judgment stroke, helps to satisfy justified expectations about formal languages and logical calculi. (iv) Such an implementation has not yet been achieved in the current logical mainstream. (shrink)
