RationaleSurrogates of critically ill patients often have inaccurate expectations about prognosis. Yet there is little research on how intensive care unit clinicians should discuss prognosis, and existing expert opinion-based recommendations give only general guidance that has not been validated with surrogate decision makers.ObjectiveTo determine the perspectives of key stakeholders regarding how prognostic information should be conveyed in critical illness.MethodsThis was a multicenter study at three academic medical centers in California, Pennsylvania, and Washington. One hundred eighteen key stakeholders completed in-depth semistructured (...) interviews. Participants included 47 surrogates of adult patients with acute respiratory distress syndrome; 45 clinicians working in study ICUs, including physicians, nurses, social workers, and spiritual care providers; and 26 experts in health communication, decision science, ethics, family-centered care, geriatrics, healthcare disparities, palliative care, psychology, psychiatry, and critical care.Measurements and main resultsThere was broad support among surrogates for existing expert recommendations, including truthful prognostic disclosure, emotional support, tailoring the disclosure strategy to each family's needs, and checking for understanding. In addition, stakeholders offered suggestions that add specificity to existing recommendations, including: In addition to conveying prognostic estimates, clinicians should help families "see the prognosis for themselves" by showing families radiographic images and explaining the clinical significance of physical manifestations of severe disease at the bedside. Many physicians did not support using numeric estimates to convey prognosis to families, whereas many surrogates, clinicians from other disciplines, and experts believed numbers could be helpful. Clinicians should conceptualize prognostic communication as an iterative process that begins with a preliminary mention of the possibility of death early in the ICU stay and becomes more detailed as the clinical situation develops. Although prognostic information should be initially disclosed by physicians, other members of the multidisciplinary team-nurses, social workers, and spiritual care providers-should be given explicit role responsibilities to reinforce physicians' prognostications and help families process a poor prognosis emotionally.ConclusionsFamily members, clinicians, and experts identified specific communication behaviors that clinicians should use to discuss prognosis in the critical care setting. These findings extend existing opinion-based recommendations and should guide interventions to improve communication about prognosis in ICUs. (shrink)

In recent decades, interest in non-monogamous intimate relationships has grown rapidly. Polyamory, relationship anarchy, consensual or ethical non-monogamy, and more have become popular in academic and public discourse. These practices destabilize the privileging of heterosexual nuclear families and the assumption that romantic coupledom is the ultimate form of love. Non-monogamous approaches flout cultural norms of exclusivity by avowing that intimacy is compatible with multiple dyadic and/or multi-party relationships. The essay has been written to contribute to the debate on Dooyeweerd’s social philosophy. The goal is to argue that the central focus of his social philosophy is the “problem of order,” not communitarian issues. This article emphasizes that Dooyeweerd’s social theory was not oriented by communitarian concerns, but in comprehending the relation between creational order and social order.

The central assumption behind James’s project, which is noted in many of the essays, is that religious knowledge is not possible. This assumption shapes the approach James takes, and limits the possible conclusions he can reach. It was an assumption shared by William Clifford, who is the chief target of James’s The Will to Believe. However, James goes in a different direction than Clifford. James agrees that religious knowledge is not possible, and yet asserts that religious experiences are useful. His (...) hypothesis is that while religious experience does not provide knowledge, it can be the cause of beneficial effects in a person’s life. His focus on results or effects is an important part of his pragmatism, and seems to grow out of his assumption that religious knowledge is not possible. It is the combination of making religious experience an object of scientific inquiry and a pragmatic assessment of religious experience that makes James unique during his time. (shrink)

E a perspectiva de Arendt é chamada de liberdade na política devido a sua aceitação positiva do espaço político enquanto campo para o exercício da ação e da liberdade dos indivíduos. ABSTRACT Nietzsche’s perspectivism can fruitfully be understood as a claim that all our representations are perspectival and absolute representations are impossible. But that treatment leaves unclear another key aspect of Nietzschean perspectivism—the idea that our representations are perspectival because they are ultimately rooted in some way in our values. I motivate this latter aspect of Nietzsche’s account through an argument that relies on the contrast between Bernard Williams’s rejection of “external reasons statements” in the case of practical reasoning, and his (...) reliance on an “absolute conception of the world” in theoretical reasoning. The importance of _naturalization_—the establishment of species introduced into foreign places—to the early development of Darwin’s theory of evolution deserves historical attention. Introduced and invasive European species presented Darwin with interpretive challenges during his service as naturalist on the HMS _Beagle_. Species naturalization and invasive species strained the geologist Charles Lyell’s creationist view of the organic world, a view which Darwin adopted during the voyage of the _Beagle_ but came to question afterward. I suggest that these phenomena primed Darwin to (...) question the “stability of species.” I then examine the role of introduced and invasive species in Darwin’s early theorizing and negotiation with Lyell’s ideas, recorded in his post-voyage “transmutation notebooks.” Therein, the subject was an inflection point in his contention with Lyell’s views and moreover, his theorizing on invasive species occasioned some of his earliest inklings of natural selection. Finally, I examine how naturalization was crucial to Lyell’s own eventual conversion to evolutionism. I conclude with brief reflections on the implications of this narrative for our understanding of Darwin’s reasoning, his intellectual relationship to Lyell, and the historical context that shaped his theory. How should we conceive of conflicts that seem intractable? Is there any hope of a resolution? We observe impasses between various groups concerning the Israeli and Palestinian conflict, the Movement for Black Lives and racial conservatives, and Indigenous voices versus settler colonial states. Some aspects of these impasses can surely be explained by an unwillingness by one or more parties to the conflict to yield any ground. Might there also be room for misunderstanding generated by radically different ways of conceiving (...) the world? According to the different worlds thesis, people come to radically different understandings of the world because they inhabit incompatible conceptual realities. In this article, I endeavor to explore possible ways of understanding the thesis and its potential impact on certain normative practices we tend to take for granted. (shrink)

O objetivo do presente artigo é apresentar o conceito de “motivos básicos religiosos” no pensamento de Herman Dooyeweerd. Em primeiro lugar, será apresentado a concepção de religião em Dooyeweerd e, em seguida, serão apresentados o conceito e o desenvolvimento dos “motivos básicos religiosos” no pensamento ocidental. Concerns over students engaging in various forms of academic misconduct persist, especially with the post-COVID19 rise in online learning and assessment. Research has demonstrated a clear role of the personality trait psychopathy in cheating, yet little is known about why this relationship exists. Building on the research by Curtis et al. (Personality and Individual Differences, 185, 111277, 2022a), this study tested an extended Theory of Reasoned Action (TRA) model, including psychopathy as a precursor to attitudes and subjective norms, and (...) measures of anticipated moral emotions (shame and guilt), to predict cheating intentions and cheating behaviours. A cross-sectional survey was administered online to university students from around the globe (n = 257). Results from a serial mediation analysis revealed that psychopathy predicted academic misconduct behaviours indirectly through attitudes, subjective norms, anticipated guilt (but not anticipated shame), and intentions. These findings indicate that cheating may be reduced by modifying attitudes to cheating, subjective norms regarding cheating, and anticipated feelings of guilt related to engaging in academic misconduct. In addition, the results revealed high rates of several forms of cheating, particularly in unsupervised online tests, which have been used more widely since the COVID-19 pandemic. This finding raises concerns regarding the poor security of such assessments. (shrink)

Open‐ended tasks can be decomposed into the three levels of Newell's Cognitive Band: the Unit‐Task level, the Operation level, and the Deliberate‐Act level. We analyzed the video game Co‐op Space Fortress at these levels, reporting both the match of a cognitive model to subject behavior and the use of electroencephalogram (EEG) to track subject cognition. The Unit Task level in this game involves coordinating with a partner to kill a fortress. At this highest level of the Cognitive Band, there is (...) a good match between subject behavior and the model. The EEG signals were also strong enough to track when Unit Tasks succeeded or failed. The intermediate Operation level in this task involves legs of flight to achieve a kill. The EEG signals associated with these operations are much weaker than the signals associated with the Unit Tasks. Still, it was possible to reconstruct subject play with much better than chance success. There were significant differences in the leg behavior of subjects and models. Model behavior did not provide a good basis for interpreting a subject's behavior at this level. At the lowest Deliberate‐Act level, we observed overlapping key actions, which the model did not display. Such overlapping key actions also frustrated efforts to identify EEG signals of motor actions. We conclude that the Unit‐task level is the appropriate level both for understanding open‐ended tasks and for using EEG to track the performance of open‐ended tasks. (shrink)

Emrah Atasoy, a prolific author who reviewed Turkish speculative fiction in a well-received 2021 (...) Utopian Studies article, is modeling a search for hope in dystopian texts. In this book, derived from his dissertation, he examines three British novels written in three different eras of the twentieth century: Katherine Burdekin's Swastika Night (1937), Anthony Burgess's The Wanting Seed (1962), and P. D. James's The Children of Men (1992). In each novel, Atasoy argues, the protagonists move from an ignorance that served the interest of the dystopian forces to a resistant knowledge that should be read as offering hope, both as resistance and as knowledge.The book is organized in a straightforward way, with an introduction, conclusion, and then three analytical chapters, one for each of the novels. The introduction is expansive, offering both brief surveys of the nature of utopia and dystopia and a recounting of a number of themes taken up by dystopian texts: education, politics, gender roles, family life, and more. Layered onto this is a discussion of theorizing around knowledge and power and the transition from ignorance to understanding. The chapters focused on the individual novels are all similarly expansive. Atasoy offers biographical information about the authors along with considerations of the primary social and political events to which the authors might be responding. The analyses of the novels focus primarily on the motivations of the protagonists, but interspersed in the plot summaries are narrative cul-de-sacs explaining particular textual details (for example, the discussion of Theo's choice of the Greek statue of Diadoumenos as allocation to leave a note for the Warden [James, 37]). These [End Page 519] little detours are often interesting but can be distracting from the primary analytic focus.The analyses themselves also cover a lot of territory. The middle section of the chapter on The Children of Men (118–34) includes discussions of power, eugenics, religion, sexuality, punishment, assisted suicide, and immigration. Each of these is used to elucidate modes of social and political control wielded by the Warden, but the sheer number of issues included in the fast-moving analysis left me wanting more. Atasoy's analysis of Quietus nicely captures the ways in which James has created a world where the elderly are both encouraged and manipulated to end their lives for the good of the whole. This discussion fits especially well with the larger focus of his analysis on modes of social engineering. Fewer themes and more close readings like this one would have deepened the reading experience. But for anyone beginning a project on one of these novels, this book is a good place to start.This book begins and ends with the interesting contention that times of catastrophe diminish the literary production of utopia, and potentially the capacity for hope itself. While the point of this book is not to defend this claim empirically, it is one worth considering for any scholar of utopia, or any person interested in the potential of future change for the better. The book may underplay the fact of twentieth-century literary utopias (there is a brief discussion of the feminist utopias written in the 1970s and 1980s), but it is surely correct that there is an imbalance in the production of utopia in favor of dystopia. Utopias are more challenging to construct and potentially (especially in this day and time) less formulaic than dystopia, which is an increasingly popular (and marketable) genre.Kim Stanley Robinson noted in a recent interview that "[utopias] shape people's conception of what's possible that could be good in the future."1 Atasoy's book offers the possibility that a close reading of dystopian texts might well help us uncover other ways of finding out what future good is possible.
