During the coronavirus disease 2019 (COVID‐19) pandemic, bioethical analyses often emphasized population health and societal benefit. Hospital policies frequently focused on reducing risk of transmitting SARS‐CoV‐2 by restricting visitors; requiring protective equipment; and screening staff, patients and visitors. While restrictions can be burdensome, they are often justified as essential measures to protect the whole population against a virus with high rates of transmission, morbidity and mortality. Yet communities are not monolithic, and the impacts of these restrictions affect different groups differently. (...) An ophthalmological unit outreach program in Ethiopia serves to illustrate. Pre‐operative screening policies were designed to protect as many patients as possible but had adverse impacts on underserved communities. As this case study demonstrates, creating hospital policies that truly serve the good of the society may require a more holistic review of impacts on inequitably positioned communities. (shrink)

There are many gene families that are specific to multicellular animals. These have either diverged from ancestral genes that are shared with fungi and/or plants or evolved from an ancestral gene unique to animals. The evolution of gene families involved in cell–cell communication and developmental control has been studied to establish whether the number of member genes increased dramatically immediately prior to or in concert with the Cambrian explosion. A molecular phylogeny‐based analysis of several animal‐specific gene families has revealed that (...) gene diversification by duplication occurred during two active periods interrupted by a long intervening quiescent period. Intriguingly, the Cambrian explosion is situated in the silent period, indicating that there is no direct link between the first burst of gene diversification and the Cambrian explosion itself. The importance of gene recruitment as a possible molecular mechanism for morphological diversity, and its possible role for the Cambrian explosion, are discussed. The medical model of childbearing assumes that a pregnancy always has the potential to turn into a risky procedure. In order to advocate humanized birth in high risk pregnancy, an important step involves the enlightenment of the professional’s preconceptions on humanized birth in such a situation. The goal of this paper is to identify the professionals’ perception of the potential obstacles and facilitating factors for the implementation of humanized care in high risk pregnancies. Twenty-one midwives, obstetricians, and health administrator professionals (...) from the clinical and academic fields were interviewed in nine different sites in Japan from June through August 2008. The interviews were audio taped, and transcribed with the participants’ consent. Data was subsequently analyzed using content analysis qualitative methods. Professionals concurred with the concept that humanized birth is a changing and promising process, and can often bring normality to the midst of a high obstetric risk situation. No practice guidelines can be theoretically defined for humanized birth in a high risk pregnancy, as there is no conflict between humanized birth and medical intervention in such a situation. Barriers encountered in providing humanized birth in a high risk pregnancy include factors such as: the pressure of being responsible for the safety of the mother and the fetus, lack of the women’s active involvement in the decision making process and the heavy burden of responsibility on the physician’s shoulders, potential legal issues, and finally, the lack of midwifery authority in providing care at high risk pregnancy. The factors that facilitate humanized birth in a high risk include: the sharing of decision making and other various responsibilities between the physicians and the women; being caring; stress management, and the fact that the evolution of a better relationship and communication between the health professional and the patient will lead to a stress-free environment for both. Humanized birth in a high risk pregnancy is something that goes beyond just curing women of their illnesses. It can be considered as a token of caring, and continued support, which positively consolidates the doctor-patient relationship. The main topic of this article is Otto Frank's forgotten notion of the pressure-volume diagram of the cardiac ventricle as a means to assess the external mechanical work of the heart. Developed by Frank at the end of the 19th century, this idea was reenvisioned as pressure-volume area about 70 to 80 years later by Hiroyuki Suga. This notion now serves as a perspective for defining cardiac contractility and thus enabling the controlled clinical application of cardiac assist devices. According to the hierarchical model of sensory information processing, sensory inputs are transmitted to cortical areas, which are crucial for complex auditory and speech processing, only after being processed in subcortical areas (Hickok and Poeppel, 2007; Rauschecker and Scott, 2009). However, studies using electroencephalography (EEG) indicate that distinguishing simultaneous auditory inputs involves a widely distributed neural network, including the medial temporal lobe, which is essential for declarative memory, and posterior association cortices (Alain et al., 2001; Squire et al., 2004). More (...) recent studies have even demonstrated plasticity of auditory signals as low as the brainstem (Suga, 2008). Collectively, studies suggest that the functional architecture of perceptual processing involves primarily top-down modulation (Suga et al., 2002; Gilbert and Li, 2013; Chandrasekaran et al., 2014). Top-down influences exerted throughout the auditory systems (Lotto and Holt, 2011) include: memory (Goldinger, 1998)1, attention (Choi et al., 2014), which has been found to modulate auditory encoding in the cochlea, a subcortical area (Maison et al., 2001), (prior) knowledge of syntax or words (Ganong, 1980; Warren, 1984)2, and experience-based expectations pertaining to the speaker's accent (Deutsch, 1996; Deutsch et al., 2004; Irino and Patterson, 2006), gender (Johnson et al., 1999), and vocal folds or tract (Irino and Patterson, 2002; Patterson and Johnsrude, 2008) . As muitas verdades sobre a infância de nosso tempo, das mais catedráticas às mais “inovadoras”, encontram-se compiladas em livros e artigos dentro e fora de nossas bibliotecas. Grosso modo, elas constroem dois tipos de niilismo. Um niilismo de tipo universalista, que suga a criança para fora da imanência da vida e da histórica e a torna refém de estereótipos universais. Outro niilismo de tipo relativista, que esquece a criança e apenas acusa os estudos da infância como mera invenção. Evitar (...) esses extremos requer uma espécie de arte acrobática de equilibrar-se na possibilidade de um entre. Essa via busca retomar a imanência histórica da criança. Trata-se de uma pesquisa em busca da potencialização da ideia de invenção cuja atitude crítica implicasse não a pergunta pelo governo certo ou pela ausência de governo da infância, mas como não a governar assim, por essas pessoas, em nome destes princípios, em vista de tais objetivos e por meio de tais procedimentos. Der vorliegende Band umfasst Beitrage zu einem zentralen Thema bei Platon: 'Physis' kann bei Platon im naturwissenschaftlichen Sinne als physische, biologische, materielle Natur oder im ubertragenen Sinne als eigenes Wesen, etwa hinsichtlich Seele, Kosmos oder Gottlichem, verstanden werden. So werden in diesem Band medizinische, biologische und kosmologische Ansatze ebenso wie ontologische, epistemologische und padagogische Themen zu Platons 'Physis'-Konzept fokussiert. Die zeitgenossische Nomos-Physis-Diskussion Platons mit den Sophisten sowie seine sprach- und kulturphilosophischen Uberlegungen spielen hier eine wichtige Rolle. Die anspruchsvolle literarische Gestaltung (...) der Platonischen Dialoge ist fur die genannten Fragestellungen hochst relevant, ebenso die Auseinandersetzung spaterer platonischer Philosophen mit Platons Physis-Konzept.
