Background: To evaluate the effectiveness of a multifaceted intervention in improving emergency department (ED) patient privacy and satisfaction in the crowded ED setting. Methods: A pre- and post-intervention study was conducted. A multifaceted intervention was implemented in a university-affiliated hospital ED. The intervention developed strategies to improve ED patient privacy and satisfaction, including redesigning the ED environment, process management, access control, and staff education and training, and encouraging ethics consultation. The effectiveness of the intervention was evaluated using patient surveys. Eligibility (...) data were collected after the intervention and compared to data collected before the intervention. Differences in patient satisfaction and patient perception of privacy were adjusted for predefined covariates using multivariable ordinal logistic regression. Results: Structured questionnaires were collected with 313 ED patients before the intervention and 341 ED patients after the intervention. There were no important covariate differences, except for treatment area, between the two groups. Significant improvements were observed in patient perception of "personal information overheard by others", being "seen by irrelevant persons", having "unintentionally heard inappropriate conversations from healthcare providers", and experiencing "providers' respect for my privacy". There was significant improvement in patient overall perception of privacy and satisfaction. There were statistically significant correlations between the intervention and patient overall perception of privacy and satisfaction on multivariable analysis. Conclusions: Significant improvements were achieved with an intervention. Patients perceived significantly more privacy and satisfaction in ED care after the intervention. We believe that these improvements were the result of major philosophical, administrative, and operational changes aimed at respecting both patient privacy and satisfaction. (shrink)

It is known that, on average, people adapt their choice of memory strategy to the subjective utility of interaction. What is not known is whether an individual's choices are boundedly optimal. Two experiments are reported that test the hypothesis that an individual's decisions about the distribution of remembering between internal and external resources are boundedly optimal where optimality is defined relative to experience, cognitive constraints, and reward. The theory makes predictions that are tested against data, not fitted to it. The (...) experiments use a no-choice/choice utility learning paradigm where the no-choice phase is used to elicit a profile of each participant's performance across the strategy space and the choice phase is used to test predicted choices within this space. They show that the majority of individuals select strategies that are boundedly optimal. Further, individual differences in what people choose to do are successfully predicted by the analysis. Two issues are discussed: the performance of the minority of participants who did not find boundedly optimal adaptations, and the possibility that individuals anticipate what, with practice, will become a bounded optimal strategy, rather than what is boundedly optimal during training. Body ownership concerns what it is like to feel a body part or a full body as mine, and has become a prominent area of study. We propose that there is a closely related type of bodily self-consciousness largely neglected by researchers—experiential ownership. It refers to the sense that I am the one who is having a conscious experience. Are body ownership and experiential ownership actually the same phenomenon or are they genuinely different? In our experiments, the participant watched a (...) rubber hand or someone else’s body from the first-person perspective and was touched either synchronously or asynchronously. The main findings: (1) The sense of body ownership was hindered in the asynchronous conditions of both the body-part and the full-body experiments. However, a strong sense of experiential ownership was observed in those conditions. (2) We found the opposite when the participants’ responses were measured after tactile stimulations had ceased for 5 s. In the synchronous conditions of another set of body-part and full-body experiments, only experiential ownership was blocked but not body ownership. These results demonstrate for the first time the double dissociation between body ownership and experiential ownership. Experiential ownership is indeed a distinct type of bodily self-consciousness. The experts reached a consensus on each item after the three-round process. The final knowledge measure comprising 10 questions was developed and validated. Thirty eligible trauma patients presenting to the Emergency Department were approached and completed the questionnaires in this pilot study. The participants exhibited significantly higher mean knowledge and satisfaction scores after watching the educational video than before watching the video. Our process is promising for developing procedure-specific informed consent and audiovisual aids in medical and surgical specialties. The educational video was developed using a scientific method that integrated the opinions of different stakeholders, particularly patients. This video is a useful tool for improving the knowledge and satisfaction of trauma patients in the ED. We investigated whether, in the emergency department, educational video-assisted informed consent is superior to the conventional consent process, to inform trauma patients undergoing surgery about the procedure, benefits, risks, alternatives, and postoperative care. We conducted a prospective randomized controlled trial, with superiority study design. All trauma patients scheduled to receive trauma-related debridement surgery in the ED of Kaohsiung Medical University Hospital were included. Patients were assigned to one of two education protocols. Participants in the intervention group watched an educational video (...) illustrating informed consent information, whereas those in the control group read an informed consent document. The primary outcome was knowledge scores and the secondary outcome was assessment of patient satisfaction. A multivariable regression model, with predefined covariates, was used to analyze differences in knowledge scores and patient satisfaction levels between the groups. A total of 142 patients were enrolled, with 70 and 72 assigned to the intervention and control groups, respectively. Mean knowledge scores were higher in the intervention ) than in the control group. By multivariate analysis, the intervention group had significantly greater differences in knowledge scores. Age, injury severity score, and baseline knowledge score significantly affected the differences in knowledge scores. Significant improvements were observed in patients’ perception of statements addressing comprehension of the information provided, helpfulness of the supplied information for decision making, and satisfaction with the informed consent process. Multivariate analysis showed significant correlations between video education and patient satisfaction. Both the educational approach and severity of injury may have an impact on patient understanding during the informed consent process in an emergency environment. Video-assisted informed consent may improve the understanding of surgery and satisfaction with the informed consent process for trauma patients in the ED. Institutions should develop structured methods and other strategies to better inform trauma patients, facilitate treatment decisions, and improve patient satisfaction. This paper takes an initial but significant step toward penetrating the intricate historiography of the renowned siyin half seal, which appears on 199 surviving or now-lost canonical Chinese paintings and calligraphies. Through a forensic tracking of the siyin art pieces, Ming dynasty court diaries, legal statutes, and other official seals ending in the words si and yin, I refute the dominant twentieth-century theory by arguing that this seal could not have originated from the eunuch-run Dianli jicha si in 1373–84, nor (...) could it have been used by the Ming general Xu Da on the Yuan palace collection as previously hypothesized. The alleged Dianli jicha si characters do not match the siyin seal strokes. New evidence suggests that the siyin seal came into existence after 1385, by which time the Directorate of Ceremonial had superseded the shuttered Dianli jicha si office. Instead of simply dividing the various siyin seal impressions into binary genuine/forged categories as past studies have done, I demonstrate that three types of siyin seals can be considered authentic, all of which were used by different branches of the jingli si. The fact that a half seal was employed suggests that the jingli si registry office followed the kanhe inventory system, which imposed an official seal on the seam of both an inventorying object and its complementary ledger. By deconstructing the siyin historiography, this paper raises new socio-political questions about ownership, censorship, and imperial competition over the siyin-marked art pieces. It also builds a foundation for the further investigation of the role inventory systems played in mapping, defining, and legitimizing the royal power of the Ming dynasty. (shrink)

BackgroundIn China, undergraduate students face both academic and career selection pressures, sleep is an important physiological process for them. Investigate the physical exercise, sleep quality of undergraduate students in the education stage in Xuzhou City, and analyze the factors affecting their sleep quality, to promote the health education and psychological health of undergraduate students.Materials and methodsThe Physical Activity Rating Scale-3, the Pittsburgh Sleep Quality Index, and the demographic information questionnaire were used to survey a whole-group sample of four undergraduate colleges (...) and universities in Xuzhou by cluster sampling, the general characteristics including gender, grade, height, weight, domicile, race, economic income, etc., were collected, and the data were analyzed and processed using chi-square tests and multi-factor logistic regression.Results3,366 valid questionnaires were collected from four undergraduate colleges and universities, including 1,355 males and 2,011 females. The detection rate of exercise in Jiangsu Normal University was lower than that in other universities, and the detection rate of sleep disorders was higher than that in other universities. Xuzhou Medical University, the highest detection rate of large exercise, Xuzhou Institute of Technology, the lowest detection rate of sleep disorders. There were significant differences in the detection rate of large amount of exercise among college students of different genders, grades, body types, and majors. There were significant differences in the detection rate of sleep disorders among college students with different gender, grade, family economic status, daily cell phone use time, cell phone purposeless usage, and exercise level. Multivariate logistic regression analysis showed that grade, family economic status, cell phone use time, cell phone purposeless usage is the main influencing factors of sleep disorders in college students.ConclusionThe sleep problems of undergraduates are serious, especially in Jiangsu Normal University. Scientific and appropriate exercise is an important measure to solve the sleep problems of undergraduates. Colleges and universities should actively carry out health education, college students living habits such as cell phone use should be guided training. (shrink)

Publications by Chinese researchers in scientific journals have dramatically increased over the past decade; however, academic misconduct also becomes more prevalent in the country. The aim of this prospective study was to understand the perceptions of Chinese biomedical researchers towards academic misconduct and the trend from 2010 to 2015. A questionnaire comprising 10 questions was designed and then validated by ten biomedical researchers in China. In the years 2010 and 2015, respectively, the questionnaire was sent as a survey to biomedical (...) researchers at teaching hospitals, universities, and medical institutes in mainland China. Data were analyzed by the Chi squared test, one-way analysis of variance with the Tukey post hoc test, or Spearman’s rank correlation method, where appropriate. The overall response rates in 2010 and 2015 were 4.5% and 5.5%, respectively. Data from 15 participants in 2010 were invalid, and analysis was thus performed for 1263 participants. Among the participants, 54.7% thought that academic misconduct was serious-to-extremely serious, and 71.2% believed that the Chinese authorities paid no or little attention to the academic misconduct. Moreover, 70.2 and 65.2% of participants considered that the punishment for academic misconduct at the authority and institution levels, respectively, was not appropriate or severe enough. Inappropriate authorship and plagiarism were the most common forms of academic misconduct. The most important factor underlying academic misconduct was the academic assessment system, as judged by 50.7% of the participants. Participants estimated that 40.1% of published scientific articles were associated with some form of academic misconduct. Their perceptions towards academic misconduct had not significantly changed over the 5 years. Reform of the academic assessment system should be the fundamental approach to tackling this problem in China. (shrink)

BackgroundThe acceptance of informal payments by doctors is usually viewed as unethical behavior. However, in China, such behavior is a common practice. In this study, we focus on the gender differences in accepting red packets by young doctors in China.MethodsA total of 413 young doctors were selected for the study, all of whom were grouped by gender. The questionnaire was designed to include general demographic characteristics, whether they had ever been offered red packets, whether they had ever accepted red packets, (...) the reasons for accepting red packets and so on. Wilcoxon rank-sum test, Pearson’s chi-squared test, univariable and multi-variable logistic regressions were used for all analyses by Stata 17.0 SE and p-value < 0.05 was considered statistically significant.ResultsCompared to women, men were more likely to be offered red packets, and the odds ratio was statistically significant after adjusting for age, education, position and geographical areas. In terms of the question of whether or not they had accepted red packets, more male doctors answered “yes” compared to female doctors. However, among those who had accepted red packets, we found that only 42.0% [25/60] of male doctors considered that it was normal to accept such red packets, compared to 85.0% [11/13] of women.ConclusionThe study revealed that Chinese patients and their families were more likely to offer red packets to male doctors. Secondly, among doctors who had been offered red packets, male doctors were more likely to accept red packets than female doctors. In addition, among doctors who had accepted red packets, female doctors were more likely to believe that it was not morally wrong to accept such red packets. (shrink)

Background Based on the principle of informed consent, doctors are required to fully inform patients and respect their medical decisions. In China, however, family members usually play a special role in the patient’s informed consent, which creates a unique “doctor-family-patient” model of the physician-patient relationship. Our study targets young doctors to investigate the ethical dilemmas they may encounter in such a model, as well as their attitudes to the family roles in informed consent. Methods A questionnaire was developed including general (...) demographic characteristics, the fulfillment of the obligation to fully inform, who will be informed, and the ethical dilemmas in decision-making. We recruited a total of 421 doctors to complete this questionnaire, of which 368 met the age requirements for this study. Cross tabulation and Pearson’s chi-squared test were used to analyze the differences between types of patients for categorical variables, and a p-value < 0.05 was considered statistically significant. Results Our data shows that only 20 doctors (5.40%) stated “informing the patient alone is sufficient” when it comes to informing patients of their serious conditions. The rest of the participants would ensure that the family was informed. When facing elderly patients with decision-making capacity, the data was statistically different (3.8%; P < 0.001) The primary reason for ensuring that family members be informed differs among the participants. In addition, when family members asked doctors to conceal the patient’s medical condition for the best interests of patients, 270 doctors (73.4%) would agree and cooperate with the family. A similar proportion (79.6%) would do so when it comes to elderly patients. Conclusions (1) Chinese doctors pay extra attention to informing the patient’s family, which may not be in the patient’s best interests. (2) Chinese doctors treat adult (but not elderly) patients and elderly patients differently when it comes to informing family members. (3) When family members request that doctors withhold information from patients “in the best interest of the patient,” the majority choose to comply with the request, although this may cause them distress. This article examines the rise and decline of the enthusiasm for intelligence testing in early twentieth-century China, focusing on the appeal, the challenges, and the critiques revolving around this psychological instrument. The introduction of intelligence testing reflected not only China’s urgent needs in modernizing its merit system, but also Chinese psychologists’ aspirations for pursuing exactitude and redefining the racial characteristics of their compatriots against foreign interpretations. But despite psychologists’ endeavors, the political and geographical fragmentation of Republican China troubled the epistemic (...) imperative of uniformity demanded by Euro-American psychometrics and therefore undermined the validity of measurement. Subsequently, the legitimacy of intelligence testing began to be questioned by several influential Chinese psychologists in the late 1920s and 30s. The difficulties in standardization and the hostility within the psychology community formed a vicious cycle, impeding the progress of nationwide testing. Through this history, the article demonstrates not only the elevation of measurement to epistemic authority in modern China, but also how its promise was challenged by a diverse and rapidly changing society. This article analyzes a popular meme that has spread rapidly among Chinese internet users in the last few years, ‘garlic chives’ ( jiucai), as a self-mockery of the bio-economic subject in contemporary China. This metaphor refers to those ordinary Chinese people who are constantly lured to participate in all kinds of economic activities, but whose investments are destined to be consumed by the establishment. Through a close study of this popular meme and the social conditions from which it arises, this (...) article demonstrates two main features of the Chinese economic subject that supports the state’s economic sovereignty: the thriving of self-sufficient and rule-abiding individuals, and the quality of these individuals as hard working and capable of producing wealth on their own. This article offers a critique of PRC’s post-socialist governmentality, and it provides insights about the relation between the biological and the political in our complicated world order. (shrink)

Augustine of Hippo is often regarded as the champion of the doctrine of weakness of the will. John M. Rist in his 1994 'Augustine: Ancient Thought Baptized' draws an interesting analogy between Aristotle's 'akrasia' and Augustine's 'concupiscentia'. However, such an analogy without further qualification is defective and misleading because it implies that Augustine commits himself to the notion that since everyone is perpetually akratic and, thus, always morally blameworthy. I argue that, for Augustine, weakness of the will has equivocal meanings (...) and is manifested in four kinds of case--their scope goes far beyond Aristotle's discussion of 'akrasia' in Book Seven of the 'Nicomachean Ethics'. There are, therefore, considerable differences between Aristotle's and Augustine's account of weakness of the will. Consequently, for Augustine, moral responsibility for the moral agent also varies in each of the four cases. (shrink)

Pang Laikwan Identity Politics and Democracy in Hong Kong’s Social Unrest Hong Kong’s anti-Extradition Law Amendment Bill (anti-ELAB) movement began with legislation proposed in February 2019 to allow the transfer of fugitives to jurisdictions with which the city lacks formal extradition treaties. The law quickly attracted a tremendous amount of criticism and generated enormous anxiety because (...) mainland China was one of those jurisdictions.1 In fact, many believe that this bill was proposed to allow the government of the People’s Republic of China (PRC) to transfer wanted persons of any nationality from Hong Kong to China, subject to a legal process that many view as highly dubious. A large-scale anti-government movement exploded in June 2019 when pro-establishment legislators in Hong Kong insisted on passing it, and millions of people took to the streets. The government waited three months to withdraw the bill, only doing so after some of the most violent confrontations between civilians and the police in Hong Kong’s history. With Beijing’s 1. The law was originally proposed after a Hong Kong man, who killed his pregnant girlfriend in Taiwan, could not be extradited back to Taiwan to stand trial for the murder due to the lack of formal extradition laws. The proposed law, which would be applied to Hong Kong citizens, foreign residents, and even people passing through on business or as tourists, is being opposed due to the fear that it would be used to extradite Hong Kong political/civil rights activists and dissidents to China, where they would be harshly punished. Pang Laikwan 207 support, the Hong Kong government has doggedly resisted negotiating with the protestors over other demands, such as setting up an independent inquiry into police misconduct. As people have become completely disillusioned with the government and their own futures, tensions have increased. Collapsing many of the city’s socioeconomic problems into one cause, the anti-ELAB movement is primarily an anti-authoritarian impulse motivated by the unifying power of the term “Hongkongers.” As the movement unfolds, one of the most popular slogans, “Add Oil, Hongkongers!” (meaning “Keep going, Hongkongers!”) gradually transformed into “Rebel, Hongkongers!” and “Revenge, Hongkongers!” showing the protestors ’ anger about unrestrained police violence and the Beijing regime behind it. The militarization associated with these protests is both worrisome and symptomatic. This short essay analyzes this movement and uses it as a vehicle to disentangle social and political identity. My concern is about how a large-scale political movement can realign identities to generate power from the bottom. I use an Arendtian approach to ground my theoretical orientation, asking: How can political actions be understood as the pursuit of freedom to transcend one’s biological and social identity? From an intersectional feminist angle, if we are aware that social discrimination operates within a complex network of simultaneous discriminations, real social change can only be envisioned using a broad political canvas. Hong Kong’s anti-Extradition Law Amendment Bill movement provides such a setting. Division and Inclusion “The people” might be the emptiest, but also the most powerful, phrase in the modern political lexicon. It can be used by politicians to justify groundless proposals or criticisms, but it is also the foundation and source of legitimacy for all modern political institutions. Indeed, the most basic meaning of democracy is “rule of the people,” which is impossible to avoid in any democratic imagination. As has been widely seen throughout the world, however, “the people” is often attached to various degrees of xenophobia—hatred of outsiders in the name of in-group solidarity. This article analyzes the current political predicament of Hong Kong by examining Nightmare Wallpaper, an art project composed of a series of automatic drawings made by local artist Pak Sheung Cheun. He made them while attending the court cases of political activists on trial, and the article further explores his subsequent efforts to transform this work into wallpaper prints, a series of installations, and a book. This political work, which is also very private, vividly and honestly demonstrates the artist’s intense (...) struggles, along with the despair felt by many in the city. The earnest self-reflection shown in the art does not give his audience a way out of the blind alley of the present but invites us to express ourselves and to connect with others. It is both a work of abjection and intersubjectivity, with no naïve expectation to reconcile the tensions between them. It shows, rather, a determination to participate in an uncertain future, combining the artist’s and the city’s capacity of meaning production and imagination.
