In the West, mainstream bioethicists tend to appreciate intimate relationships as a hindrance to individual autonomy. Scholars have even argued against approaching a mother to donate a kidney to save the life of her child; the request, they claim, is too manipulative and, thereby, violates her autonomy. For Chinese bioethicists, such a moral analysis is absurd. The intimate relationship between mother and child establishes strong mutual obligations. It creates mutual moral responsibilities that often require sacrifices for each other. This paper (...) argues that while aiding others is a moral duty, helping one’s family is a much stronger duty and poses no threat to one’s autonomy. For Confucianism, empathetic intimate feelings, the heart and mind of ren, rest at the root of morality. It requires that we, as moral beings, assume duties to relieve the suffering of others. The more intimate the relationship the stronger the obligation to assist. The family is a closely knitted moral community. Family members often share living resources, mutual experiences, and a sense of identity. Family members act as a social unit, and, ordinarily, mutual obligations among members have priority over duties to those outside of the family. For Confucian bioethics, family-based consent to medical treatment is regarded as natural and reasonable. Family-based decision making is a taken-for-granted norm of social life. While close family members have priority, Confucianism extends such obligations outward toward members of the extended family and the society at large. There is a general principle of gradation of love, which reflects different degrees of personal intimacy and, therefore, of moral obligation. In this fashion, Confucianism seeks to treat the whole of society as one extended family. Hence, in bioethics, mutual responsibility and family-based consent are regarded as basic principles. Through a series of case discussions, this paper illustrates that atomistic individual-based autonomy offers but a poor response to bioethical issues. (shrink)

The controversy concerning the identity of thinking and being has been under way for several months. The truth is bound to become more clear when it is debated further. Our understanding of this problem has also been enhanced through discussion, and this discussion has all the more demonstrated its important significance. It will become very evident that this is by no means a problem of the individual term, which Engels used in Ludwig Feuerbach and the End of Classical German Philosophy, (...) but also a problem concerning the basic principles of Marxist philosophy; nor is it merely an abstract theoretical problem, but rather a problem of great realistic significance. Our conversation stems from a collaborative, fieldwork-based project on Chinese Islam, and on Chinese Muslim women’s rights, within a secular political ideology and legislative rights framework. Dialogue was a defining feature throughout the years of field investigation and research, fusing autobiographical and cultural trajectories in what the anthropologist Kirsten Hastrup calls the ‘betweenness’ of intersubjectively created experience and knowledge. The premise for our conversation lies in the claim that insufficient attention has been paid to developments, which suggest alternatives to the (...) near-axiomatic ethnographic relationship of outside researcher and native informant. In foregrounding the cross-cultural relationship between two researchers, we explore how in the course of fieldwork interaction, boundaries of self and familiar ‘standpoints’ are redrawn. We ask how the history of this collaborative relationship across cultures might question and redefine the classic ethnographic binary of inside and outside, the centre and the margin, the universal and the particular, the ‘western feminist’ and ‘other women’. Studies of style can be approached from two directions: the end and the means. From the end point of view, a style is a cluster of features present in artifacts; scholars usually classify the features in products to differentiate styles. Similar approaches used to examine features for further exploring the nature of style, the degree between styles, and the systematic measurement within style were extensively covered in Chap. 3. From the means point of view, a style is a mode by (...) which designers’ personal and professional preferences are expressed, and studies attempt to deliberate the mode of expression to mark styles. Although most style researchers have studied both directions, their efforts cannot provide clear explanations of how a style is generated. That is because not enough research has been devoted to the study of the means used that create a style. This chapter begins to explore, through a case study, the aspects of style creation and the forces that generate a style. Studies of style approached from the means point of view conducted in various fields and the factors determining the generation of style are reviewed first. (shrink)

The ethical governance of the global Internet is an accelerating global phenomenon. A key paradox of the global Internet is that it allows individual and collective decision making to co-exist with each other. Open source software (OSS) communities are a globally accelerating phenomenon. OSS refers to groups of programs that allow the free use of the software and further the code sharing to the general and corporate users of the software. The combination of private provision and public knowledge and software, (...) and the seeming paradox of economic versus social motivations have stimulated a wide debate between researchers and policymakers. In this article, we analyze OSS communities from the viewpoint of "intrinsic motivation," knowledge creation, and collective Internet governance. We believe that the growth of global OSS has fundamental implications for business ethics and the governance of the global Internet in the twenty-first century. (shrink)

BackgroundThe Public Health Service Increased Risk designation identified organ donors at increased risk of transmitting hepatitis B, hepatitis C, and human immunodeficiency virus. Despite clear data demonstrating a low absolute risk of disease transmission from these donors, patients are hesitant to consent to receiving organs from these donors. We hypothesize that patients who consent to receiving offers from these donors have decreased time to transplant and decreased waitlist mortality.MethodsWe performed a single-center retrospective review of all-comers waitlisted for liver transplant from (...) 2013 to 2019. The three competing risk events (transplant, death, and removal from transplant list) were analyzed. 1603 patients were included, of which 1244 (77.6%) consented to offers from increased risk donors.ResultsCompared to those who did not consent, those who did had 2.3 times the rate of transplant (SHR 2.29, 95% CI 1.88–2.79, p < 0.0001), with a median time to transplant of 11 months versus 14 months (p < 0.0001), as well as a 44% decrease in the rate of death on the waitlist (SHR 0.56, 95% CI 0.42–0.74, p < 0.0001). All findings remained significant after controlling for the recipient age, race, gender, blood type, and MELD. Of those who did not consent, 63/359 (17.5%) received a transplant, all of which were from standard criteria donors, and of those who did consent, 615/1244 (49.4%) received a transplant, of which 183/615 (29.8%) were from increased risk donors.ConclusionsThe findings of decreased rates of transplantation and increased risk of death on the waiting list by patients who were unwilling to accept risks of viral transmission of 1/300–1/1000 in the worst case scenarios suggests that this consent process may be harmful especially when involving “trigger” words such as HIV. The rigor of the consent process for the use of these organs was recently changed but a broader discussion about informed consent in similar situations is important. (shrink)

AimResting state functional magnetic resonance imaging was used to analyze changes in functional connectivity within various brain networks and functional network connectivity among various brain regions in patients with high myopia.Methodsrs-fMRI was used to scan 82 patients with HM and 59 healthy control volunteers matched for age, sex, and education level. Fourteen resting state networks were extracted, of which 11 were positive. Then, the FCs and FNCs of RSNs in HM patients were examined by independent component analysis.ResultsCompared with the HC (...) group, FC in visual network 1, dorsal attention network, auditory network 2, visual network 3, and sensorimotor network significantly increased in the HM group. FC in default mode network 1 significantly decreased. Furthermore, some brain regions in default mode network 2, default mode network 3, auditory network 1, executive control network, and significance network increased while others decreased. FNC analysis also showed that the network connection between the default mode network and cerebellar network was enhanced in the HM group.ConclusionCompared with HCs, HM patients showed neural activity dysfunction within and between specific brain networks, particularly in the DMN and CER. Thus, HM patients may have deficits in visual, cognitive, and motor balance functions. (shrink)

AimPatients with high myopia reportedly exhibit changes in functional brain activity, but the mechanism underlying such changes is unclear. This study was conducted to observe differences in dynamic spontaneous brain activity between patients with HM and healthy controls via dynamic regional homogeneity analysis.MethodsResting-state functional magnetic resonance imaging scans were performed on 82 patients with HM and 59 HCs who were closely matched for age, sex, and weight. The dReHo approach was used to assess local dynamic activity in the human brain. (...) The association between mean dReHo signal values and clinical symptoms in distinct brain areas in patients with HM was determined via correlation analysis.ResultsIn the left fusiform gyrus, right inferior temporal gyrus, right Rolandic operculum, right postcentral gyrus, and right precentral gyrus, dReHo values were significantly greater in patients with HM than in HCs.ConclusionPatients with HM have distinct functional changes in various brain regions that mainly include the L-FG, R-ITG, R-ROL, R-PoCG, and R-PreCG. These findings constitute important evidence for the roles of brain networks in the pathophysiological mechanisms of HM and may aid in the diagnosis of HM. (shrink)
