Background In 2001 a report on the provision of clinical ethics support in UK healthcare institutions identified 20 clinical ethics committees. Since then there has been no systematic evaluation or documentation of their work at a national level. Recent national surveys of clinical ethics services in other countries have identified wide variation in practice and scope of activities. Objective To describe the current provision of ethics support in the UK and its development since 2001. Method A postal/electronic questionnaire survey administered (...) to the chairs of all 82 clinical ethics services registered with the UK Clinical Ethics Network in July 2010. Results Response rate was 62% with the majority of responding services situated in acute trusts. All services included a clinical ethics committee with one service also having a clinical ethicist. Lay members were present in 72% of responding committees. Individual case consultation has increased since 2001 with 29% of chairs spending more than 50% of their time on this. Access to and involvement in the process of case consultation is less for patients and families than for clinical staff. There is wide variation in committee processes and levels of institutional support. Over half of the responding committees undertook some form of evaluation. Conclusion Clinical ethics services in the UK are increasing as is their involvement in case consultation. However, the significant variation in committee processes suggests that further qualitative research is needed to understand how these committees function and the role they play in their institution. Should moods be regarded as intentional states, and, if so, what kind of intentional content do they have? I focus on irritability and apprehension, which I examine from the perspective of a teleosemantic theory of content. Eric Lormand has argued that moods are non-intentional states, distinct from emotions; Robert Solomon and Peter Goldie argue that moods are generalised emotions and that they have intentional content of a correspondingly general kind. I present a third model, on which moods are regarded, not (...) as generalised emotions, but as states of vigilance; and I argue that, on this model, moods should be regarded as intentional states of a kind quite distinct from emotions. An advantage of this account is that it allows us to distinguish between a mood of apprehension and an episode of objectless fear. (shrink)

Donald Gustafson has argued that grief centres on a combination of belief and desire:The belief that the subject has suffered an irreparable loss.The desire that this should not be the case.And yet, as Gustafson points out, if the belief is true, the desire cannot be satisfied. Gustafson takes this to show that grief inevitably implies an irrational conflict between belief and desire.I offer a partial defence of grief against Gustafson's charge of irrationality. My defence rests on two elements. First, I (...) offer an alternative model of emotion, which presents emotions as complex episodes, initiated by emotional appraisals. Secondly, I appeal to John Bowlby's account of grief to argue that grief involves two forms of sadness (anguish and desolation) which Gustafson's analysis runs together. I concede that anguish does characteristically involve an element of irrationality. But the irrationality of anguish does not arise from an apparently gratuitous clash between belief and desire, but from a conflict between emotion and belief—a form of irrationality that is both familiar and easily explained. Emotion is at the centre of our personal and social lives. To love or to hate, to be frightened or grateful is not just a matter of how we feel on the inside: our emotional responses direct our thoughts and actions, unleash our imaginations, and structure our relationships with others. Enhancing body awareness has been described as a key element or a mechanism of action for therapeutic approaches often categorized as mind-body approaches, such as yoga, TaiChi, Body-Oriented Psychotherapy, Body Awareness Therapy, mindfulness based therapies/meditation, Feldenkrais, Alexander Method, Breath Therapy and others with reported benefits for a variety of health conditions. To better understand the conceptualization of body awareness in mind-body therapies, leading practitioners and teaching faculty of these approaches were invited as well as their patients to participate in focus (...) groups. The qualitative analysis of these focus groups with representative practitioners of body awareness practices, and the perspectives of their patients, elucidated the common ground of their understanding of body awareness. For them body awareness is an inseparable aspect of embodied self awareness realized in action and interaction with the environment and world. It is the awareness of embodiment as an innate tendency of our organism for emergent self-organization and wholeness. The process that patients undergo in these therapies was seen as a progression towards greater unity between body and self, very similar to the conceptualization of embodiment as dialectic of body and self described by some philosophers as being experienced in distinct developmental levels. Prevalent narratives of agricultural innovation predict that we are once again on the cusp of a global agricultural revolution. According to these narratives, this so-called fourth agricultural revolution, or agriculture 4.0, is set to transform current agricultural practices around the world at a quick pace, making use of new sophisticated precision technologies. Often used as a rhetorical device, this narrative has a material effect on the trajectories of an inherently political and normative agricultural transition; with funding, other policy instruments, and (...) research attention focusing on the design and development of new precision technologies. A growing critical social science literature interrogates the promises of revolution. Engagement with new technology is likely to be uneven, with benefits potentially favouring the already powerful and the costs falling hardest on the least powerful. If grand narratives of change remain unchallenged, we risk pursuing innovation trajectories that are exclusionary, failing to achieve responsible innovation. This study utilises a range of methodologies to explore everyday encounters between farmers and technology, with the aim of inspiring further work to compile the microhistories that can help to challenge robust grand narratives of change. We explore how farmers are engaging with technology in practice and show how these interactions problematise a simple, linear notion of innovation adoption and use. In doing so, we reflect upon the contribution that the study of everyday encounters can make in setting more inclusionary, responsible pathways towards sustainable agriculture. (shrink)

In this paper, I try to develop an account of functions which might be of use to a biologist engaged in classifying and explaining natural phenomena. The most pressing difficulty facing such an account is the need to reconcile the normativity of function statements with their explanatory force. I consider two familiar accounts of function statements, offered by Andrew Woodfield and Larry Wright . I examine both accounts in search of the strongest possible formulation of each type of theory. I (...) fail to find a formulation of the ACT which is completely immune from counterexamples, but I do find a satisfactory formulation of the HT. In particular, I argue that the HT should incorporate a point central to the ACT that functions involve a means/end relationship between two devices. I then consider functional explanations: I argue that an ACT which holds that function statements are intrinsically explanatory cannot offer a satisfactory account. The account offered by Woodfield, who rejects this assumption, is adequate, though less attractive than the account suggested by the HT. I conclude by considering how the HT can account for functional norms. (shrink)

Regret is a slippery phenomenon. Fundamental questions about its fittingness conditions and functions have yet to be settled. Here, I offer a diagnosis of regret’s slippery character. Extending a suggestion made by Daniel Kahneman, I argue that regret comes in a range of emotional flavours, distinguished in the first instance by their phenomenology. While regret has received some attention from philosophers, its varied phenomenology has not been investigated. Yet the varied phenomenology of regret is significant: it reflects further variations in (...) its cognitive and motivational effects. As a result, I argue, different flavours of regret have different fittingness conditions and contribute to our lives in different ways. In Section I, I offer a preliminary sketch of regret and introduce the questions that drive my discussion. In Section II, I introduce regret’s different flavours. In Section III, I investigate their contrasting cognitive and motivational effects, with a view to determining their functions. In Section IV, I investigate what this implies concerning regret’s fittingness conditions—and, in particular, what constitutes a regrettable action. (shrink)

What is the difference between an emotional appraisal and a dispassionate judgement? It has been suggested that emotional appraisals are states of a special kind that play a distinctive role in our psychology; it has also been suggested that emotional appraisals have a distinctive kind of content. In this paper, I explore the links between the function and content of an emotional appraisal, making use of a teleosemantic account of intentional content that I have developed elsewhere.

Abstract Why should we love the people we do and why does love motivate us to act as it does? In this paper, I explore the idea that these questions can be answered by appealing to the idea that love has to do with close personal relationships (the relationship claim). Niko Kolodny (2003) has already developed a relationship theory of love: according to Kolodny, love centres on the belief that the subject shares a valuable personal relationship with the beloved. However, (...) this account has some implausible consequences. I shall develop an alternative account, discarding the assumption that love centres on a belief, and beginning instead from a conception of love as an emotional attitude, which, I suggest, involves a form of evaluation that is not belief. As I explain, adopting this view allows us to interpret the relationship claim, not as a claim about the subject?s beliefs, but as a claim about the function of love. This approach allows us to answer the questions above, while avoiding the difficulties that confront Kolodny's account. I end by exploring a case that might be thought to raise some difficulties for my account. (shrink)

This essay reviews a collection of thirteen critical essays on the work of Ruth Millikan. The collection covers a broad range of her work, focusing in particular on her account of simple intentionality, her theory of concepts and her metaphysical views. I highlight and briefly discuss three issues that crop up repeatedly though the collection: (1) Millikan’s externalism (and in particular, her emphasis on how intentional states are used, rather than how they are produced); (2) the nature of intentional explanation; (...) and (3) the normativity of meaning. What does it mean to say that an emotional response fits the situation? This question cannot be answered simply by specifying the core relational theme (loss or risk, say) associated with each emotion: we must also explain what constitutes an emotionally significant loss or risk. It is sometimes suggested that emotionally significant situations are those that bear on the subject’s interests or concerns. I accept that this claim is plausible for some emotional responses, and I propose a particular way of (...) interpreting it. I suggest that, for many emotions, emotional significance is determined by the subject’s likes and dislikes – that is, settled dispositions to find a certain situation pleasant or distressing. I contrast this account with other preference-based accounts and with an account that appeals to the subject’s interests. I argue that we should prefer the likes-based account to these rival views. Compassion is widely regarded as an important moral emotion – a fitting response to various cases of suffering and misfortune. Yet contemporary theorists have rarely given it sustained attention. This volume aims to fill this gap by offering answers to a number of questions surrounding this emotion.

This article considers a central question in the philosophy of emotion: what is an emotion? This is a highly controversial question, which has attracted numerous answers. I argue that a good answer to this question may prove very hard to find. The difficulty, I suggest, can be traced back to three features of emotional phenomena: their diversity, their complexity and their coherence. I end by suggesting that we should not be disturbed by this result, as we do not need to (...) know what an instance of emotion is in order to investigate the topic of emotion. (shrink)

Combating the identity problem is crucial and urgent as false identity has become a common denominator of many serious crimes, including mafia trafficking and terrorism. Without correct identification, it is very difficult for law enforcement authority to intervene, or even trace terrorists’ activities. Amongst several identity attributes, personal names are commonly, and effortlessly, falsified or aliased by most criminals. Typical approaches to detecting the use of false identity rely on the similarity measure of textual and other content-based characteristics, which are (...) usually not applicable in the case of highly deceptive, erroneous and unknown descriptions. This barrier can be overcome through analysis of link information displayed by the individual in communication behaviours, financial interactions and social networks. In particular, this paper presents a novel link-based approach that improves existing techniques by integrating multiple link properties in the process of similarity evaluation. It is utilised in a hybrid model that proficiently combines both text-based and link-based measures of examined names to refine the justification of their similarity. This approach is experimentally evaluated against other link-based and text-based techniques, over a terrorist-related dataset, with further generalization to a similar problem occurring in publication databases. The empirical study demonstrates the great potential of this work towards developing an effective identity verification system. (shrink)

Functional imaging studies of neurologically intact adults have demonstrated that the right posterior cerebellum is activated during verb generation, semantic processing, sentence processing, and verbal fluency. Studies of patients with cerebellar damage converge to show that the cerebellum supports sentence processing and verbal fluency. However, to date there are no patient studies that investigated the specific importance of the right posterior cerebellum in language processing, because: case studies presented patients with lesions affecting the anterior cerebellum, and group studies combined patients (...) with lesions to different cerebellar regions, without specifically reporting the effects of right posterior cerebellar damage. Here we investigated whether damage to the right posterior cerebellum is critical for sentence processing and verbal fluency in four patients with focal stroke damage to different parts of the right posterior cerebellum. We examined detailed lesion location by going beyond common anatomical definitions of cerebellar anatomy, and employed a recently proposed functional parcellation of the cerebellum. All four patients experienced language difficulties that persisted for at least a month after stroke but three performed in the normal range within a year. In contrast, one patient with more damage to lobule IX than the other patients had profound long-lasting impairments in the comprehension and repetition of sentences, and the production of spoken sentences during picture description. Spoken and written word comprehension and visual recognition memory were also impaired, however, verbal fluency was within the normal range, together with object naming, visual perception and verbal short-term memory. This is the first study to show that focal damage to the right posterior cerebellum leads to language difficulties after stroke; and that processing impairments persisted in the case with most damage to lobule IX. We discuss these results in relation to current theories of cerebellar contribution to language processing. Overall, our study highlights the need for longitudinal studies of language function in patients with focal damage to different cerebellar regions, with functional imaging to understand the mechanisms that support recovery. In this paper, I consider the objection, raised by Radu Bogdan, that a teleological theory of content is unable to ascribe content to a general-purpose, doxastic system. I begin by giving some attention to the notion of general-purpose representation, and suggest that this notion can best be understood as what I term "interest-independent" representation. I then outline Bogdan's objection in what I take to be its simplest form. I attempt to counter the objection by explaining how a teleologist might ascribe (...) content in a particular case - the case of a perceptual judgement whose content is learned. I reject the idea that the teleologist can appeal to the way in which the subject has used the judgement, or its constituent concepts, in the past, on the grounds that it is possible for the subject to produce judgements and concepts that never help her to satisfy any of her interests. Instead, my account depends on the idea that the process of learning is regulated by a mechanism whose function is to produce a harmony between the information carried by perceptual judgements and the way in which they are used in inference. To examine the underlying processes of an innovative mind-body practice, Mindful Body Awareness, this exploratory study involved four case studies analyzed phenomenologically using the dialogal method. Mindful Body Awareness combines manual and verbal processing, and is focused on facilitation of client body awareness. Four individuals were recruited to receive weekly 1.25 hour sessions over four weeks. The Helpfulness Aspects of Therapy form was administered immediately after each session to access participants’ perceptions of the therapy experience. In addition, the Scale of (...) Body Connection was used to examine pre- and post-body awareness and bodily dissociation. Analysis involved phenomenology and descriptive statistics. The overall perceived helpfulness of the intervention was evident in the four themes that emerged from the analysis. These themes were gaining interoceptive awareness, personal agency, therapist trust and conceptual framing, and transformation. The participants’ responses were also used to investigate the therapy process across time. A pattern of increased interoceptive depth was apparent, as was a concomitant progression in embodied sense of self. Improvements in body awareness and bodily dissociation were evident for two of the four participants. These findings help to identify primary components of Mindful Body Awareness and suggest the role of these components in the embodiment process. (shrink)

A historical theory of rational norms claims that, if we are supposed to think rationally, this is because it is biologically normal for us to do so. The historical theorist is committed to the view that we are supposed to think rationally only if, in the past, adult humans sometimes thought rationally. I consider whether there is any plausible model of rational norms that can be adopted by the historical theorist that is compatible with the claim that adult human beings (...) are subject to rational norms, given certain plausible empirical assumptions about our history and capabilities. I suggest that there is one such model: this model centres on the idea that a procedure is rational if it has been endorsed (or at least not rejected) by mechanisms that have the function to ensure that the subject learns to reason in a way that approaches a certain kind of optimality. A review of Teleological Realism: mind, agency and explanation by Scott R. Sehon.

Intravenous drug abusers may incur bloodstream infections, in particular those involving the heart valves, that often require extended courses of antibiotics, commonly on the order of six weeks.Conventional wisdom has dictated that even when patients are sufficiently well to not need ongoing hospitalization, it is unsafe to complete their antibiotic course in any setting other than in a closely supervised facility, even if this is contrary to their wishes. The assumption has been that such patients would be at risk of (...) using their indwelling intravenous catheter for illicit purposes. Recent advances in the care of patients who suffer from addiction disorders suggest that when patients receive state-of-the-art addiction treatment, many may be able to continue their intravenous antibiotic course unsupervised, at home. This represents a departure from the parentalistic model of care of impaired patients who are prone to self-harm, moving towards a model that respects autonomy and trusts patients who are in recovery to continue their care in a manner that is self-beneficial. Using fMRI, we investigated how right temporal lobe gliomas affecting the posterior superior temporal sulcus alter neural processing observed during speech perception and production tasks. Behavioural language testing showed that three pre-operative neurosurgical patients with grade 2, grade 3 or grade 4 tumours had the same pattern of mild language impairment in the domains of object naming and written word comprehension. When matching heard words for semantic relatedness (a speech perception task), these patients showed under-activation in the tumour infiltrated right (...) superior temporal lobe compared to 61 neurotypical participants and 16 patients with tumours that preserved the right postero-superior temporal lobe, with enhanced activation within the (tumour-free) contralateral left superior temporal lobe. In contrast, when correctly naming objects (a speech production task), the patients with right postero-superior temporal lobe tumours showed higher activation than both control groups in the same right postero-superior temporal lobe region that was under-activated during auditory semantic matching. The task dependent pattern of under-activation during the auditory speech task and over-activation during object naming was also observed in eight stroke patients with right hemisphere infarcts that affected the right postero-superior temporal lobe compared to eight stroke patients with right hemisphere infarcts that spared it. These task-specific and site-specific cross-pathology effects highlight the importance of the right temporal lobe for language processing and motivate further study of how right temporal lobe tumours affect language performance and neural reorganisation. These findings may have important implications for surgical management of these patients, as knowledge of the regions showing functional reorganisation may help to avoid their inadvertent damage during neurosurgery.
