Variability in the symptom experience in patients diagnosed with chronic conditions may be related to social determinants of health (SDoH). The purpose of this critical review was to (1) summarize the existing literature on SDoH and symptom clusters (i.e., multiple, co‐occurring symptoms) in patients diagnosed with common chronic conditions, (2) evaluate current variables and measures used to represent SDoH, (3) identify gaps in the evidence base, and (4) provide recommendations for the incorporation of SDoH into future symptom cluster research. We (...) identified 118 articles including information on SDoH in chronic condition symptom cluster research. Articles primarily focused on cancer populations. Few articles had the explicit purpose of investigating relationships between SDoH and symptom clusters, and the inclusion of SDoH was often limited to variables used to describe samples. Future studies should be designed to “move beyond Table 1” in their utilization of SDoH as variables and examine relationships between SDoH and symptom clusters. Attention should be paid to the appropriateness of measures being used to collect information on SDoH, and analysis methods that estimate causal connections between variables should be considered. Research regarding the relationship of SDoH with symptom clusters in patients with chronic conditions has the potential to reveal mechanisms of symptom disparities and guide changes to alleviate these disparities. Widespread concerns about the long-term fiscal gap in Social Security have prompted various proposals for structural reform, with individual accounts as the centerpiece. Carving out individual accounts from the existing system would shift significant risks and responsibilities to individual workers. A parallel development has already occurred in the area of private pensions. Experience with 401 plans indicates that many workers will have difficulty making prudent decisions concerning investment and withdrawal of funds. Moreover, in implementing any system of voluntary individual accounts, (...) it will be important to design default settings that provide appropriate guidance for workers with heterogeneous levels of financial sophistication and risk tolerance. The central goal of Social Security reform should be to close the fiscal gap in a way that preserves rather than undermines the existing system of mandatory defined benefits for all workers. This article provides the first comprehensive reconstruction and analysis of Hintikka’s attempt to obtain a measure of the information yield of deductive inferences. The reconstruction is detailed by necessity due to the originality of Hintikka’s contribution. The analysis will turn out to be destructive. It dismisses Hintikka’s distinction between surface information and depth information as being of any utility towards obtaining a measure of the information yield of deductive inferences. Hintikka is right to identify the failure of canonical information theory (...) to give an account of the information yield of deductions as a scandal, however this article demonstrates that his attempt to provide such an account fails. It fails primarily because it applies to only a restricted set of deductions in the polyadic predicate calculus, and fails to apply at all to the deductions in the monadic predicate calculus and the propositional calculus. Some corollaries of these facts are a number of undesirable and counterintuitive results concerning the proposed relation of linguistic meaning (and hence synonymy) with surface information. Some of these results will be seen to contradict Hintikka’s stated aims, whilst others are seen to be false. The consequence is that the problem of obtaining a measure of the information yield of deductive inferences remains an open one. The failure of Hintikka’s proposal will suggest that a purely syntactic approach to the problem be abandoned in favour of an intrinsically semantic one. (shrink)

This article mounts a defence of Floridi’s theory of strongly semantic information against recent independent objections from Fetzer and Dodig-Crnkovic. It is argued that Fetzer and Dodig-Crnkovic’s objections result from an adherence to a redundant practice of analysis. This leads them to fail to accept an informational pluralism, as stipulated by what will be referred to as Shannon’s Principle, and the non-reductionist stance. It is demonstrated that Fetzer and Dodig-Crnkovic fail to acknowledge that Floridi’s theory of strongly semantic information captures (...) one of our deepest and most compelling intuitions regarding informativeness as a basic notion. This modal intuition will be referred to as the contingency requirement on informativeness. It will be demonstrated that its clarification validates the theory of strongly semantic information as a novel, and non ad hoc solution to the Bar-Hillel-Carnap semantic paradox. (shrink)

K-axiom-based epistemic closure for explicit knowledge is rejected for even the most trivial cases of deductive inferential reasoning on account of the fact that the closure axiom does not extend beyond a raw consequence relation. The recognition that deductive inference concerns interaction as much as it concerns consequence allows for perspectives from logics of multi-agent information flow to be refocused onto mono-agent deductive reasoning. Instead of modeling the information flow between different agents in a communicative or announcement setting, we model (...) the information flow between different states of a single agent as that agent reasons deductively. Performing an inference involves irreducibly dynamic cognitive procedures. The article proposes that a non-associative information frame, corresponding to a residuated pogroupoid, underpins the information structure involved. The argument proceeds by expounding the informational turn in logic, before outlining the cognitive actions at work in deductive inference. The structural rules of Weakening, Contraction, Commutation, and Association are rejected on the grounds that they cause us to lose track of the information flow in inferential procedures. By taking the operation of information application (...) as the primary operation, the fusion connective is retained, with commutative failure generating a double implication. This essay proposes a procedural interpretation of negative information in terms of split negation as procedural prohibition. Information frames and models are introduced, with negation defined as the implication of bottom, 0. A method for extracting the procedures prohibited by complex formulas is outlined, and the relationship between types of prohibited procedures is identified. Definitions of negation types in terms of the implication of 0 on an informational interpretation have been criticized. This criticism turns on the definitions creating a purportedly (...) unnatural asymmetry between positive and negative information. It is demonstrated below that a strong asymmetry between positive and negative information is in fact the case. As such, an asymmetry between positive and negative information is natural, and something that we should want an informational interpretation of negation to preserve. (shrink)

Background: Compassion fatigue is recognized as impacting the health and effectiveness of healthcare providers, and consequently, patient care. Compassion fatigue is distinct from “burnout.” Reliable measurement tools, such as the Professional Quality of Life scale, have been developed to measure the prevalence, and predict risk of compassion fatigue. This study reviews the prevalence of compassion fatigue among healthcare practitioners, and relationships to demographic variables. Methods: A systematic review was conducted using key words in MEDLINE, PubMed, and Ovid databases. Data were (...) extracted from a total of 71 articles meeting inclusion criteria, from studies measuring compassion fatigue in healthcare providers using a validated instrument. Quantitative and qualitative data were extracted and compiled by three independent reviewers into an evidence table that included basic study characteristics, study strength and quality determination, measurements of compassion fatigue, and general findings. Meta-analysis, where data allowed, was stratified by Professional Quality of Life version, heterogeneity was quantified, and pooled means were reported with 95% confidence interval. A table of major study characteristics and results was created. Ethical consideration: This paper contains no primary data obtained directly from research participants. Data obtained from previously published resources have been acknowledged within references. Psychological distress, particularly compassion fatigue, can be insidious, no health profession is immune, and may significantly impact the ability to provide care. Results: A total of 71 studies were included. Compassion fatigue was reported across all practitioner groups studied. Relationships to most demographic variables such as years of experience and specialty were either not statistically significant or unclear. Variability in reporting of Professional Quality of Life results was found. Interpretation: Compassion fatigue exists across diverse practitioner groups. Prevalence is highly variable, and its relationship with demographic, personal, and/or professional variables is inconsistent. Questions are raised about how to mitigate compassion fatigue. (shrink)

This article adds to the growing literature distinguishing second and third wave feminist theory. It opens by outlining theoretical differences between second wave and third wave definitions of femininity and the role and impact of femininity on gender relations of domination, whilst briefly acknowledging the complications of the relationship between third wave feminism and post-feminism. It then suggests that these diverging perspectives on embodied femininity result from fundamentally different second wave and third wave theories of power. In doing so, it (...) develops a third wave conceptual definition of femininity and a theoretical framework for power relations that are distinct from a second wave approach. According to most accounts of trust, you can only trust other people (or groups of people). To trust is to think that another has goodwill, or something to that effect. I sketch a different form of trust: the unquestioning attitude. What it is to trust, in this sense, is to settle one’s mind about something, to stop questioning it. To trust is to rely on a resource while suspending deliberation over its reliability. Trust lowers the barrier of monitoring, challenging, checking, (...) and questioning. Trust sets up open pipelines between yourself and parts of the external world. Trust permits external resources to have a similar relationship to one as one’s internal cognitive faculties. This creates efficiency, but at the price of exquisite vulnerability. We must trust in this way because we are cognitively limited beings in a cognitively overwhelming world. Crucially, we can hold the unquestioning attitude towards objects and technologies. When I trust my climbing rope, I stop worrying about its reliability. When I trust my online calendaring system, I simply go to the events indicated, without question. But, one might worry, how could one ever hold such a normatively loaded attitude as trust towards mere objects? How could it ever make sense to feel betrayed by an object? Trust is our engine for expanding and outsourcing our agency — for binding external processes into our practical selves. Thus, we can be betrayed by our smartphones in the same way that we can be betrayed by our memory. When we trust, we try to make something a part of our agency, and we are betrayed when our part lets us down. To unquestioningly trust something is to let it in—to attempt to bring it inside one’s practical functioning. This suggests a new form of gullibility: agential gullibility, which occurs when agents too hastily and carelessly integrate external resources into their own agency. (shrink)

Value capture occurs when an agent’s values are rich and subtle; they enter a social environment that presents simplified — typically quantified — versions of those values; and those simplified articulations come to dominate their practical reasoning. Examples include becoming motivated by FitBit’s step counts, Twitter Likes and Re-tweets, citation rates, ranked lists of best schools, and Grade Point Averages. We are vulnerable to value capture because of the competitive advantage that such crisp and clear expressions of value have in (...) our private reasoning and our public justification. There is, however, a price. In value capture, we take a central component of our autonomy — our ongoing deliberation over the exact articulation of our values — and we outsource it. And the metrics to which we outsource usually engineered for the interests of some external force, like a large-scale institution’s interest in cross-contextual comprehensibility and quick aggregability. That outsourcing cuts off one of the key benefits to personal deliberation. In value capture, we no longer adjust our values and their articulations in light of own rich experience of the world. Our values should often be carefully tailored to our particular selves or our small-scale communities, but in value capture, we buy our values off the rack. In some cases – like decreasing CO2 emissions – the costs of non-tailored values are outweighed by the benefit of precise collective coordination. In other cases, like in our aesthetic lives, they are not. This suggests that we should want different values suited to different scales. We should want value federalism. Some values are perhaps best pursued at the largest-scale level, others at smaller scales. The problem occurs when we exhibit an excess preference for the largest-scale values – when we consistently let the universal metrics swamp our quieter interests. (shrink)

In Canada, all research conducted by individuals associated with universities must be subjected to review by research ethics boards (REB). Unfortunately, decisions reached by REBs may seriously compromise the integrity of university-based research. In this paper attention will focus on how requirements of REBs and a legal department in four Canadian universities affected response rates to a survey of domestic and international students. It will be shown that in universities in which students were sent a legalistic cover letter to a (...) mail survey, or were required to sign a consent form, lower response rates were achieved than in universities in which students were sent a relatively friendly letter. In turn, lower response rates resulted in: sample characteristics that deviated from population characteristics; a reduction in the possibility of testing research hypotheses; and increased survey costs. As a consequence, it is argued that the unreasonable demands of REBs are seriously compromising the quality of research that can be carried out on Canadian university students. (shrink)

Recent conversation has blurred two very different social epistemic phenomena: echo chambers and epistemic bubbles. Members of epistemic bubbles merely lack exposure to relevant information and arguments. Members of echo chambers, on the other hand, have been brought to systematically distrust all outside sources. In epistemic bubbles, other voices are not heard; in echo chambers, other voices are actively undermined. It is crucial to keep these phenomena distinct. First, echo chambers can explain the post-truth phenomena in a way that epistemic (...) bubbles cannot. Second, each type of structure requires a distinct intervention. Mere exposure to evidence can shatter an epistemic bubble, but may actually reinforce an echo chamber. Finally, echo chambers are much harder to escape. Once in their grip, an agent may act with epistemic virtue, but social context will pervert those actions. Escape from an echo chamber may require a radical rebooting of one's belief system.
