The Office of Human Research Protections was not justified in issuing findings against the SUPPORT Institutions. Our community can learn from the evolving healthcare transformation into learning health systems by thinking about the novel ethical issues about standard of care research raised by the SUPPORT with the same spirit of quality improvement. The current regulatory framework and the concept of foreseeable research risks is insufficient to advance the debate about the ethics of randomization of standard clinical interventions. This article uses (...) the example of the Wisconsin cystic fibrosis randomized clinical trial for newborn screening trial to explore the distinctions between risks of research and clinical care and waivers of informed consent for randomization. Collaborative exploration of these complex policy issues is needed and further deliberation, community engagement, and social science research will be critical to advance novel approaches for informed consent. The English Neoplatonic philosopher Ralph Cudworth introduced the term ‘consciousness’ into the English philosophical lexicon. Cudworth uses the term to define the form and structure of cognitive acts, including acts of freewill. In this article I highlight the important role of theological disputes over the place and extent of human freewill within an overarching system of providence. Cudworth’s intellectual development can be understood in the main as an increasingly detailed and nuanced reaction to the strict voluntarist Calvinism that is typified (...) in the thought of his near contemporary William Perkins. At the heart of Cudworth’s rejection of Calvinism is the dilemma over whether God is understood primarily in terms of will or justice. In this fleshing-out of the power of consciousness Cudworth moves from an instrumental account of the working of the human mind towards an account of human consciousness that is intrinsic to his definition of human agency. People often face preferential decisions under risk. To further our understanding of the cognitive processes underlying these preferential choices, two prominent cognitive models, decision field theory (DFT; Busemeyer & Townsend, 1993) and the proportional difference model (PD; González‐Vallejo, 2002), were rigorously tested against each other. In two consecutive experiments, the participants repeatedly had to choose between monetary gambles. The first experiment provided the reference to estimate the models’ free parameters. From these estimations, new gamble pairs were generated for the second (...) experiment such that the two models made maximally divergent predictions. In the first experiment, both models explained the data equally well. However, in the second generalization experiment, the participants’ choices were much closer to the predictions of DFT. The results indicate that the stochastic process assumed by DFT, in which evidence in favor of or against each option accumulates over time, described people’s choice behavior better than the trade‐offs between proportional differences assumed by PD. In this paper the border is evaluated as a fold of power relations in which sovereign capacity and competence is marshaled in the furtherance of illiberal practices. Drawing from interview data of officials in various agencies engaged in the US-Canada and particularly the Windsor-Detroit corridor, the argument is made that the border is a site for both negative and positive power, for insertion and subtraction, and that surveillance and compliance regimes are ‘run’ not so much in the furtherance of a (...) precautionary or preemptive end-state, but as intermediate values that are sufficiently malleable by an invigorated sovereign, expressed in the residue of discretion in and between the many border agencies. From the structure of quantum dynamics for closed and open systems, we describe several general results about information flow between interacting systems, which can be expressed in diagrammatic form. Conditions on information flow (e.g., that no information is transferred from system A to system B) imply that the overall dynamical evolution has a particular structure. We also remark that one simple type of two-qubit interaction, the unitary CNOT gate, cannot be represented by local operations and a single simultaneous information exchange.

Ground Representationism is the position that for each phenomenal feature there is a representational feature that accounts for it. Against this thesis, Ned Block (The Puzzle of Phenomenal Precision, 2015) has provided an intricate argument that rests on the notion of “phenomenal precision”: the phenomenal precision of a percept may change at a different rate from its representational counterpart. If so, there is then no representational feature that accounts for a specific change of this phenomenal feature. Therefore, Ground Representationism cannot (...) be generally true. -/- Although the notion of phenomenal precision is intuitive, it is admittedly in need of clarification. Here I reconstruct Block’s argument by suggesting a way of estimating phenomenal precision that is based on the assumption that parts of perceptual wholes can share phenomenal features independently of their place in the whole. Understood like this, the overall argument shows what it is supposed to show. -/- A more thorough look at the notion of phenomenal precision suggests tension with Block’s other work: in order to be non-trivial, we have to accept that some of our phenomenality is not concrete, but only generic. Such “solely generic phenomenology”, however, is a position mainly held by opponents to Block’s Access- vs. Phenomenal Consciousness-distinction. Interpreting phenomenal imprecision as constituted by introspective imprecision does not suffice as a way out. It seems that phenomenal precision is either trivial, self-contradictory, or incompatible with Block’s position elsewhere. So some additional elucidation on this crucial notion is needed. (shrink)

This volume presents new perspectives on transparency-theoretic approaches to self-knowledge. It addresses many under-explored dimensions of transparency theories and considers their wider implications for epistemology, philosophy of mind, and psychology. It is natural to think that self-knowledge is gained through introspection, whereby we somehow peer inward and detect our mental states. However, so-called transparency theories emphasize our capacity to peer outward at the world, hence beyond our minds, in the pursuit of self-knowledge. For all their popularity in recent decades, transparency (...) theories have also met with myriad challenges. The chapters in this volume seek to forge new ground in debates about the role of transparency in self-knowledge. Some chapters deepen our understanding of key themes at the heart of transparency theories, such as the ways in which transparent self-knowledge is properly 'first-personal' or 'non-alienated'. Other chapters extend transparency theory to different kinds of mental states and phenomena such as memory, actions, social groups, credences, projection, second-order sincerity, and Moore's Paradox. A model of “ephemeral” population structure is presented that applies not only to biological systems in which discrete groups form but also to networks without group boundaries. The evolution of altruistic behaviors is discussed. Nonrandom interaction and nonlinear fitness structures are modeled; together, these factors can produce stable polymorphisms of altruistic and selfish types, as well as bistability. Empirical applications of the model may be found in microbes, marine invertebrates, annual plants, and other organisms.

This is the first comprehensive analysis of the royal and princely courts of Europe as important places of Enlightenment. The households of European rulers remained central to politics and culture throughout the eighteenth century, and few writers, artists, musicians, or scholars could succeed without establishing connections to ruling houses, noble families, or powerful courtiers. Covering case studies from Spain and France to Russia, and from Scandinavia and Britain to the Holy Roman Empire, the contributions of this volume examine how Enlightenment (...) figures were integrated into the princely courts of the Ancien Régime, and what kinds of relationships they had with courtiers. Dangers and opportunities presented by proximity to court are discussed as well as the question of what rulers and courtiers gained from their interactions with Enlightenment men and women of letters. The book focusses on four areas: firstly, the impact of courtly patronage on Enlightenment discourses and the work as well as careers of Enlightenment writers; secondly, the court as an audience to be catered for by Enlightenment writers; thirdly, the function of Enlightenment narratives and discourses for the image-making of rulers and courtiers; and fourthly, the role the interaction of courtiers and Enlightenment writers played for the formulation of reform policies. OBJECTIVE: Pilot study to characterize treatment differences between patients treated in clinical trials and those treated in a clinical setting. Previous studies have shown higher survival rates for participants in trials of cancer therapy. This difference is observed even after rates are adjusted for important covariates such as age and stage of disease. DESIGN: Retrospective chart review. SETTING: Oncology outpatient department in a tertiary care hospital. PATIENTS: Ninety women 18 to 70 years of age with early-stage breast cancer who were (...) diagnosed in 1990. Fifty-one of the women were treated through clinical trials and 39 were treated outside of clinical trials. OUTCOME MEASURES: Number of blood tests, telephone calls, clinic visits and imaging procedures as well as intensity of chemotherapy and use of radiation therapy. The age of the patient and the stage of disease were important covariates. RESULTS: After the analysis was controlled for patient age and stage of disease, patients treated through a clinical trial were more likely to receive standard-dose chemotherapy (p = 0.020, 95% confidence interval 1.20 to 200.73) and more frequent blood tests (p < 0.001, 95% confidence interval 1.02 to 1.13) than other patients treated in the clinic. CONCLUSIONS: Our results provide a plausible mechanism for the observed survival advantage for participants in clinical trials in oncology. Further study is called for. If these results are confirmed, they have important implications for informed consent to participate in clinical trials and for clinical practice. (shrink)
