Substituted judgment assumes adequate knowledge of patient’s mind-set. However, surrogates’ prediction of individual healthcare decisions is often inadequate and may be based on shared background rather than patient-specific knowledge. It is not known whether surrogate’s prediction of patient’s integrative life-story narrative is better. Respondents in 90 family pairs rank-ordered 47 end-of-life statements as life-story narrative measure and completed instruments on decision-control preference and healthcare-outcomes acceptability as control measures, from respondent’s view and predicted pair’s view. They also scored their confidence in (...) surrogate’s decision-making and familiarity with pair’s healthcare-preferences. Life-story narratives’ prediction was examined by calculating correlation of statements’ ranking scores between respondent-personal and respondent-surrogate Q-sorts and between respondent-surrogate and pair-personal Q-sorts before and after controlling for correlation with respondent-personal scores, and by comparing percentages of respondent-surrogate Q-sorts co-loading with pair-personal vs. respondent-personal Q-sorts. Accuracy in predicting decision-control preference and healthcare-outcomes acceptability was determined by percent concordance. Results were compared among subgroups defined by intra-pair relationship, surrogate’s decision-making confidence, and healthcare-preferences familiarity. Mean age was 35.4 years, 69% were females, and 73 and 80% reported ≥ very good health and life-quality, respectively. Mean surrogate’s decision-making confidence score was 3.35 and 75% were ≥ familiar with pair’s healthcare-preferences. Mean projection, simulation, and adjusted-simulation correlations were 0.68, 0.42, and 0.26, respectively. Out of 180 respondent-surrogate Q-sorts, 24, 9, and 32% co-loaded with respondent-personal, pair-personal, or both Q-sorts, respectively. Accuracy in predicting decision-control preference and healthcare-outcomes acceptability was 47 and 52%, respectively. Surrogate’s decision-making confidence score correlated with adjusted-simulation’s correlation score. There were significant differences among the husband-wife, parent-child, and sibling-sibling subgroups in percentage of respondent-surrogate Q-sorts co-loading with pair-personal Q-sorts and percent agreement on healthcare-outcomes acceptability. Despite high self-reported surrogate’s decision-making confidence and healthcare-preferences familiarity, family surrogates are variably inadequate in simulating life-story narratives. Simulation accuracy may not follow the next-of-kin concept and is 38% based on shared background. (shrink)

It turns out that the "Line City in Saudi Arabia" is the most famous and modern of these smart cities in terms of preserving 100 percent natural life, owing to the fact that they rely totally on natural energy and do not use any automobiles. One of the most generally implemented projects in the field of smart cities is the technological and integrative developments of the use of sensors and data collecting in conjunction with rising difficulties aimed at enhancing human well-being and improving human livelihood. This is especially true in Saudi Arabia, where visitors can arrive quickly by just walking through the checkpoints, resulting in a journey of little more than a few minutes to reach any regular destination. Because the pandemic has kept the world busy in the last two years, a revival of Derrida's writings on biology is an urgent prerequisite today. The tsunamic invasion of the Coronavirus or Covid-19 pandemic took more than two million lives and thousands are still being tested positive as I write. The speed of the virus and the seeming human inability to stop its spread necessitates a study on the relationship between Covid-19 and Derrida's deconstructive philosophy. Hence the portmanteau Coviderrida – (...) inspired by Hélène Cixous’ ‘ derridevilishly’ – attempts to demonstrate how the pathogen operates as an event or possibility to which Derrida's neologisms ‘différance’, ‘supplement’, ‘trace’, ‘archi-writing’, and ‘text’ testify. The main concern of this paper is to restore the deconstructive logic that Derrida advocates to restructure the relationship between life and death in his 1975-6 seminar La vie la mort, and reshape our notion of human survival in his 2001 interview with Giovanna Borradori, ‘Autoimmunity’. The biblical story on the miracle of the manna in the Sinai Desert aroused many discussions and interpretations over the generations. The current study focuses on Ibn Ezra’s controversy with Hiwi al-Balkhi on the question of whether the manna was a natural or miraculous phenomenon. The article explores the claims of the two sides in light of the historical evidence and the literature describing the phenomenon of ‘falling manna’ in various areas of the Sinai Desert and Eastern countries. According (...) to Hiwi, the manna that rained down on the Israelites is taranjebin, a semi-liquid resinous sweet secretion of insects (honeydew) that exudes onto plants. Ibn Ezra deals with Hiwi’s claims through a series of refutations and arguments. He argues that the characteristics of the taranjebin do not fit the description of the biblical manna. For example, it does not come down in the Sinai Desert, it appears during a limited season, does not melt in the sun and does not rot during the night, and serves as a medicine rather than as food. Contribution: This article contributes to the understanding of Hiwi al-Balkhi’s identification of the biblical manna as honeydew and Ibn Ezra’s claims against his thesis. It expounds the commentators’ interpretations from a multidisciplinary perspective, such as the reality of harvesting the taranjebin in Iran and North Africa and its uses as food and medicine in the medieval culture. The history of mental illnesses is as old as human history. Mental disorders are affected by changing social and cultural factors during the historical process, and have been conceptually restructured and their definitions and classifications have been changed. The evolution of obssessive-compulsive disorders with roots as old as human history into modern concepts took place in the 19th century. The first scientific views on the spiritual origin of OCD belong to S. Freud. Freud observed that mental causes in OCD are (...) different from other mental disorders, and he examined obsession as a separate and specific disorder. Over time, it has been understood that OCD is a very common disorder. The American Psychiatric Association (APA) has classified OCD under an independent title in the latest version of the Diagnostic and Statistical Manual of Mental Disorders (DSM-5) by removing it from the category of anxiety disorders. OCD is a mental disorder caused by mental actions such as obsessive thoughts (obsessions) and / or repetitive and compulsive behaviors (compulsion). The patient is often aware that such thoughts and behaviors are irrational and absurd. For this reason, he tries to get away obsessive thoughts that he finds contrary to his beliefs, opinions, moral values and logic from his mind. However, as he tries, obsessive thoughts become more and more frequent and create a great anxiety in the person. This time, the patient resorts to compulsions to alleviate the anxiety he feels or to get rid of it completely. As his compulsions repeat, his anxiety increases and he goes into a vicious circle. There are many symptom subgroups of OCD that seriously worsen the quality of life of the individual. One of these, and even the most overwhelming, is religious obsessions. It has been thought that there has been a relationship between religion and OCD for a long time in the psychology literature, and some remarkable studies have been conducted to clarify this relationship. Although there are many research findings showing that religion is an indispensable element in the protection of the mental health integrity of the individual, some forms of religious understanding and some experiences gained as a result of religious experiences may affect the concerns about OCD. This stage of religiosity is closely related to some sub-symptom groups of OCD. The explanations of the founder of the psychoanalytic method, Freud psychoanalytic method, and his followers made a great contribution to the understanding of OCD. However, since the psychoanalytic method exhibits a theoretically reductionist approach to religion, it is possible that the individual's devotion to religion is initially evaluated as a pathology or at least an important obstacle to insight. Today, cognitive behavioral psychotherapies are considered the most effective psychological treatment method for OCD, but their success rates in the treatment of religious OCD are low. Since religious OCD concerns often involve spiritual issues rather than concrete repeatable situations, behavioral methods can be quite difficult to apply in the treatment of religious OCD. Therefore, traditional psychotherapy approaches, even if they are not openly against religions, are insufficient to relieve the suffering in the lives of individuals with religious obsessions, as they generally neglect the religious dimension. Due to the vacuum that arises from restricting spiritual treatments to secular approaches, religious patients are threatened by the illegal practices of people who abuse religious feelings. Clinically, religious obsessions are more worrying and overwhelming. Therefore, their treatment is more difficult than others. Selective approaches that use different scientific disciplines together in the treatment of OCD and can stretch around religious beliefs and values can create more effective treatment models. Thus, the success rate in the treatment of religious OCD can be carried to higher levels thanks to the appropriate cooperation of the disciplines of medicine, psychology and theology. There is a rich religious cultural heritage that can support this collaboration in terms of theological disciplines. As an example, Abū Zayd al-Balkhī (d. 322/934), in his Maṣāliḥ al-abdān wa-l-anfus, probably made a clear distinction between neuroses and psychosis and classified neurotic disorders for the first time about ten centuries before Freud. In this study, the importance of approaching to religious obsessions as both a psychological disorder and a theological problem is emphasized. For this reason, cooperation of psychiatric and psychological approaches with spiritual counseling and guidance is recommended in the treatment of religious OCD. The difficulty in distinguishing whether the symptoms seen in a religious person are normal, natural religious thoughts and behaviors of religious life or abnormal appearances in the religious guise necessitates this interdisciplinary cooperation. Newly accessible source material calls for a revision of our picture of the more technical transmission of sceptical epistemologies in the intellectual landscape of early Islam. Abū al‐Qāsim al‐Balkhīʼs (ninth/tenth century) Book of Doctrines shows that naẓar as the basic argumentative method of kalām is defined by the encounter with a broad spectrum of sceptical strategies. By the beginning of the tenth century, Greek traditions were amalgamated in a complex way with other intellectual traditions, most notably dualist ontologies. This situation (...) has shaped the evolution of kalām methodology decisively. Based on extensive paraphrases in Ibn al‐Malāḥimī (twelfth century) going back to Nawbakhtī (tenth century), it can be seen that arguments of the Greek sceptical tradition were accessible in Arabic in the early tenth century. The scarcity of coherent information on sceptical approaches before that time rather has to do with the problem that in general few sources of this earlier formative period of Islamic theology are preserved. Early Muʿtazilī treatises from this period are superseded by Ashʿarī texts in the later transmission. Sceptical approaches, being systematically involved in the very formation of early Muʿtazilī theology, are particularly invisible due to this process. (shrink)

Traditional measurement of multisensory facilitation in tasks such as speeded motor reaction tasks (MRT) consistently show age-related improvement during early childhood. However, the extent to which motor function increases with age and hence contribute to multisensory motor reaction times in young children has seldom been examined. Thus, we aimed to investigate the contribution of motor development to measures of multisensory (auditory, visual, and audiovisual) and visuomotor processing tasks in three young school age groups of children (n = 69) aged (5−6, (...) n = 21; 7−8, n = 25.; 9−10 n = 18 years). We also aimed to determine whether age-related sensory threshold times for purely visual inspection time (IT) tasks improved significantly with age. Bayesian results showed decisive evidence for age-group differences in multisensory MRT and visuo-motor processing tasks, though the evidence showed that threshold time for visual identification IT performance was only slower in the youngest age group children (5−6) compared to older groups. Bayesian correlations between performance on the multisensory MRT and visuo-motor processing tasks indicated moderate to decisive evidence in favor of the alternative hypothesis (BF10 = 4.71 to 91.346), though not with the threshold IT (BF10 < 1.35). This suggests that visual sensory system development in children older than 6 years makes a less significant contribution to the measure of multisensory facilitation, compared to motor development. In addition to this main finding, multisensory facilitation of MRT within race-model predictions was only found in the oldest group of children (9−10), supporting previous suggestions that multisensory integration is likely to continue into late childhood/early adolescence at least. Al-Shāfiʿī’s works contain reports of numerous debates on the epistemological value of hadith reports, but unfortunately rarely name specific opponents or interlocutors or the groups to which they belong. In Jimāʿ al-ʿilm al-Shāfiʿī summarizes the views of scholars whom he considered opponents of hadith under two broad rubrics: those who reject hadith categorically and those who reject only part of the hadith. There have been speculations regarding their identity but only in 2013 was a concrete advance made, when Ahmed El (...) Shamsy identified one of al-Shāfiʿī’s opponents in debate as Ibn ʿUlayya. Knowledge is inextricably bound to power in the context of settler colonialism where apprehension of the Other is a tool of domination. Tracing the development of the “settler colonial” paradigm, this article deconstructs Zionist and Israeli dispossession of Palestinian land and sovereignty, applying the sociology of knowledge production to the study of the Israeli-Palestinian case. The settler colonial paradigm, linked to Israeli critical sociology, post-Zionism, and postcolonialism, reemerged following changes in the political landscape from the mid-1990s that reframed the history (...) of the Nakba as enduring, challenged the Jewish definition of the state, and legitimated Palestinians as agents of history. Palestinian scholars in Israel lead the paradigm’s reformulation. This article offers a phenomenology of Palestinian positionality, a critical potential for decolonizing the settler colonial structure and exclusive Jewish sovereignty, to consolidate a field of study that shapes not only research into the Israeli-Palestinian case but approaches to decolonization and liberation. This article articulates a sociological conception of settler colonial remembering as a tool of legitimation. Theories of memory in the context of settler colonialism generally center counter-memories by the subaltern or colonized, or official hegemonizing representations at the level of state institutions. Underexamined is the dialectical nature of memory and discursive representations that help reproduce settler colonial processes of accumulation and displacement at the micro-level. The article draws on archival data from avowedly socialist-leftist Zionist colonies to explicate patterned representations of (...) Palestinian villages that Zionist forces gradually displaced prior to and during the 1948 War/Nakba. I demonstrate how the colonial settlers attributed political meaning through five representational modes: contrasting the indigenous as backward and primitive and settlers as progressive and developed; denying an indigenous connection to the land; emphasizing amiable relations through the promotion of cultural progress and settler superiority; asymmetrically assessing settler and indigenous belongings to national collectives; and legitimizing land purchases that dispossessed indigenous cultivators, despite the settlers’ socialist ideology, while reducing conflict to the issue of economic compensation. I theorize a form of settler colonial memory based not merely on erasure, but on recognition and disavowal. Finally, I argue that local memory is a significant site of production in which the conceptual tools to both trace the historical processes of supremacy and subvert asymmetrical sociality lie. This article extends critical trends of citizenship studies and the theory of accumulation by dispossession to articulate how settler colonial citizenship is instantiated through the active accrual of land and resources and how the emerging settler colonial citizenship entrenches both structural subjugation and resistance. The article then examines the reformation of the boundaries of citizenship through indigenous agency. I do so through examining the Palestinian citizens in Israel, specifically centering the Internally Displaced Persons—Palestinians who received Israeli citizenship even as they (...) were displaced from their places of origin. I conclude by asserting citizenship’s double paradox in settler colonial contexts: It regulates certain rights and mobilities but simultaneously entraps the indigenous in a structure in which recursive accumulation is constitutive, thus entrenching dispossession and the further loss of collective rights and other claims. This paper asks how perception becomes racializing and seeks the means for its critical interruption. My aim is not only to understand the recalcitrant and limitative temporal structure of racializing habits of seeing, but also to uncover the possibilities within perception for a critical awareness and destabilization of this structure. Reading Henri Bergson and Maurice Merleau-Ponty in dialogue with Frantz Fanon, Iris Marion Young and race-critical feminism, I locate in hesitation the phenomenological moment where habits of seeing can be internally (...) fractured. Hesitation, I claim, makes visible the exclusionary logic of racializing and objectifying perception, countering its affective closure and opening it to critical transformation. (shrink)

Disclosure of health information is a sensitive matter, particularly in the context of serious illness. In conservative societies—those which predominate in the developing world—direct truth disclosure undoubtedly presents an ethical conundrum to the modern physician. The aim of this study is to explore the truth disclosure practices of physicians in Jordan, a developing country. In this descriptive, cross-sectional study, 240 physicians were initially selected by stratified random sampling. The sample was drawn from four major hospitals in Amman, Jordan. A closed-ended (...) questionnaire was distributed and completed by self-report. A total of 164 physicians completed the questionnaire. Thirty-seven physicians usually withheld the diagnosis of “serious illness” from patients, while 127 physicians usually divulged the information directly. Among the latter, 108 physicians made exceptions to their disclosure policy. Specialists were more likely to withhold health information. Non-disclosure was primarily motivated by request from the patient’s family. In twenty cases, non-disclosure was undertaken independently. In conclusion, most respondents opt to disclose the truth; however, the vast majority of these respondents make exceptions. Instances of non-disclosure are primarily motivated by sociocultural constructs. (shrink)

BackgroundInstitutional review boards (IRBs) are formally designated to review, approve, and monitor biomedical research. They are responsible for ensuring that researchers comply with the ethical guidelines concerning human research participants. Given that IRBs might face different obstacles that cause delays in their processes or conflicts with investigators, this study aims to report the functions, roles, resources, and review process of IRBs in Saudi Arabia.MethodThis was a cross-sectional self-reported survey conducted from March 2021 to March 2022. The survey was sent to (...) 53 IRB chairpersons and the administration directors (or secretary) across the country through email after receiving verbal consent. The validated survey consisted of eight aspects: (a) organizational aspects, (b) membership and educational training, (c) submission arrangements and materials, (d) minutes, (e) review procedures, (f) communicating a decision, (g) continuing review, and (h) research ethics committee (REC) resources. A total of 200 points indicated optimal IRB functions.ResultsTwenty-six IRBs across Saudi Arabia responded to the survey. Overall, the IRBs in this study scored a total of 150/200 of the points on the self-assessment tool. Relatively newer IRBs (established less than ten years ago) conducted meetings at least once in a month, had annual funding, had more balanced gender representation, tended to score higher than older IRBs. The organizational aspect score was the lowest among all items in the survey (14.3 score difference, p-value < 0.01). The average turnaround time for expedited research from proposal submission to final decision was 7 days, while it was 20.5 days for the full committee review.ConclusionSaudi IRBs performed generally well. However, there is room for focused improvement with respect to extra resources and organizational issues that require closer evaluation and guidance from the regulatory bodies. (shrink)

This article experiments with multimedia storytelling to re-vision difference outside biomedical and humanistic frames by generating new understandings of living dis/artfully with illness. We present and analyze seven short videos created by women and trans people living with illness as part of an arts-based research project that aimed to speak back to hegemonic concepts of disability that create barriers to healthcare.1 We call for a welcoming in of disability studies’ disruptive and re-imaginative orientations to bodily difference to unsettle medicine’s humanistic (...) accounts. As emphasised in Islamic sources, justice is one of the most important issues covered in the religion of Islam. In fact, justice is a central theme in Islam and has a special value in this regard. Conversation about justice and its nature, as well as its realisation in human communities, has been thus far a necessity in human life. Actually, the establishment and implementation of justice in all areas are crucial for the utopia. Given the importance of this subject, the (...) present study aims to determine and explain the components of the justice-oriented Islamic community based on the teachings of Nahj al-Balaghah. Therefore, the components that introduce justice orientation in the Islamic community are extracted from the sermons, letters and wise sayings (viz. narrations) of Nahj al-Balaghah. Afterward, the extracted components are classified based on the similarity of the themes and concepts. In the end, five classifications are introduced, including distributive justice, procedural justice, interactional justice, social justice and fairness.Contribution: Based on the determined indices, the model of justice-oriented Islamic community is developed. It is thus recommended to pay more attention to the realisation of a justice-oriented Islamic community by researchers and leaders. This is mainly because of the fact that the opposite of a justice-oriented community is one with injustice, where oppression and disrespect for the rights of others occur, divine blessings change, divine wrath and anger are provoked and destruction approaches.
