There has long been tension between bioethicists whose work focuses on classical philosophical inquiry and those who perform empirical studies on bioethical issues. While many have argued that empirical research merely illuminates current practices and cannot inform normative ethics, others assert that research-based work has significant implications for refining our ethical norms. In this essay, I present a novel construct for classifying empirical research in bioethics into four hierarchical categories: Lay of the Land, Ideal Versus Reality, Improving Care, and Changing (...) Ethical Norms. Through explaining these four categories and providing examples of publications in each stratum, I define how empirical research informs normative ethics. I conclude by demonstrating how philosophical inquiry and empirical research can work cooperatively to further normative ethics. (shrink)

This dissertation is about prejudice. In particular, it examines the theoretical and ethical questions raised by research on implicit social biases. Social biases are termed "implicit" when they are not reported, though they lie just beneath the surface of consciousness. Such biases are easy to adopt but very difficult to introspect and control. Despite this difficulty, I argue that we are personally responsible for our biases and obligated to overcome them if they can bring harm to ourselves or to others. (...) My dissertation addresses the terms of their removal. It is grounded in a comprehensive examination of empirical research and, as such, is a contribution to social psychology. Although implicit social biases significantly influence our judgment and action, they are not reducible to beliefs or desires. Rather, they constitute a class of their own. Understanding their particular character is vital to determining how to replace them with more preferable habits of mind. I argue for a model of interpersonal fluency, a kind of ethical expertise that requires transforming our underlying dispositions of thought, feeling, and action. (shrink)

In this article, the authors explore the impact of a potential future regulatory decision by FDA whether or not to continue its enforcement discretion policy allowing physicians to perform, and stool banks to sell, stool product for fecal microbiota transplantation as a treatment for recurrent Clostridium Difficile infection without an Investigative New Drug application. The paper looks at the Agency's regulatory options in light of the current gut microbiota based products that are in the FDA pipeline for drug approval and (...) the potential impact and repercussions of their approval on FDA action. In laying out FDA's options we consider the implications of market exclusivity and off-label use of newly approved drugs. Ultimately, we explore the potential impact of FDA's decision on patients, research, and innovation. (shrink)

Whether the initial question is phrased in the horizon of faith, in an existential or abstract perspective, whether it directly adresses God, whether it is approached by means of creation theology, a theology of the cross or eschatological theology, whether it is based on a traditional theistical idea of god, agnostic or atheistic, logical or empirical, ultimately determines the actual existence of an answer, which possible answers can be brought to the questioner's attention and which of his or her needs can be satisfied in this regard; in short, what can be expected from the treatment of the problem and what cannot. Mechanisms play an important role in many sciences when it comes to questions concerning explanation, prediction, and control. Answering such questions in a quantitative way requires a formal represention of mechanisms. Gebharter (2014) suggests to represent mechanisms by means of one or more causal arrows of an acyclic causal net. In this paper we show how this approach can be extended in such a way that it can also be fruitfully applied to mechanisms featuring causal feedback.

This book shows that the links with France stretch back deep into the Middle Ages, and continue without a break into the 18th century, the Age of Enlightenment.

Some philosophers and physicians have argued that alcoholic patients, who are responsible for their liver failure by virtue of alcoholism, ought to be given lower priority for a transplant when donated livers are being allocated to patients in need of a liver transplant. The primary argument for this proposal, known as the Responsibility Argument, is based on the more general idea that patients who require scarce medical resources should be given lower priority for those resources when they are responsible for (...) needing them and when they are competing with patients who need the same resources through no fault of their own. Since alcoholic patients are responsible for needing a new liver and are in direct competition with other patients who need a new liver through no fault of their own, it follows that alcoholic patients ought to be given lower priority for a transplant. In this article, I argue against the Responsibility Argument by suggesting that in order for it to avoid the force of plausible counter examples, it must be revised to say that patients who are responsible for needing a scarce medical resource due to engaging in behavior that is not socially valuable ought to be given lower priority. I'll then argue that allocating organs according to social value is inconsistent or in tension with liberal neutrality on the good life. Thus, if one is committed to liberal neutrality, one ought to reject the Responsibility Argument. (shrink)

Semantic realism, as I shall understand it it in this paper, is the combination of the views that sentential understanding is constituted by grasp of truth conditions and that the notion of truth which figures therein is essentially epistemically unconstrained. In a single slogan, understanding a sentence consists in some cases in grasp of potentially recognition-transcendent truth conditions. For example, a semantic realist about the past holds that our understanding of 'Caesar sneezed fifteen times on his 19th birthday' consists in (...) grasp of its truth condition, where this is capable of obtaining even though there is no guarantee that we shall be able, even in principle, to recognize that that is so. ObjectiveCo-morbid insomnia and sleep apnea is a common and debilitating condition that is more difficult to treat compared to insomnia or sleep apnea-alone. Emerging evidence suggests that cognitive behavioral therapy for insomnia is effective in patients with COMISA, however, those with more severe sleep apnea and evidence of greater objective sleep disturbance may be less responsive to CBTi. Polysomnographic sleep study data has been used to predict treatment response to CBTi in patients with insomnia-alone, but not in patients with COMISA. (...) We used randomized controlled trial data to investigate polysomnographic predictors of insomnia improvement following CBTi, versus control in participants with COMISA.MethodsOne hundred and forty five participants with insomnia and sleep apnea [apnea-hypopnea index ≥ 15] were randomized to CBTi or no-treatment control. Mixed models were used to investigate the effect of pre-treatment AHI, sleep duration, and other traditional, and novel [quantitative electroencephalography ] polysomnographic predictors of between-group changes in Insomnia Severity Index scores from pre-treatment to post-treatment.ResultsCompared to control, CBTi was associated with greater ISI improvement among participants with; higher AHI, less wake after sleep onset, and less N3 sleep. No quantitative electroencephalographic, or other traditional polysomnographic variables predicted between-group ISI change.DiscussionAmong participants with COMISA, higher OSA severity predicted a greater treatment-response to CBTi, versus control. People with COMISA should be treated with CBTi, which is effective even in the presence of severe OSA and objective sleep disturbance. (shrink)

Several proponents of the interventionist theory of causation have recently argued for a neo-Russellian account of causation. The paper discusses two strategies for interventionists to be neo-Russellians. Firstly, I argue that the open systems argument – the main argument for a neo-Russellian account advocated by interventionists – fails. Secondly, I explore and discuss an alternative for interventionists who wish to be neo-Russellians: the statistical mechanical account. Although the latter account is an attractive alternative, it is argued that interventionists are not (...) able to adopt it straightforwardly. Hence, to be neo-Russellians remains a challenge to interventionists. (shrink)

This paper compares two ‘relativist’ theories about deliciousness: truth-relativism, and Kit Fine’s non-factualism about a subject-matter. Contemporary truth-relativism is presented as a linguistic thesis; its metaphysical underpinning is often neglected. I distinguish three views about the obtaining of worldly states of affairs concerning deliciousness, and argue that none yields a satisfactory version of truth-relativism. Finean non-factualism about deliciousness is not subject to the problems with truth-relativism. I conclude that Finean non-factualism is the better relativist theory. As I explain, non-facualism about (...) deliciousness is happily combined with an invariantist semantics for the word “delicious”. On this approach, relativism is a matter for a metaphysical theory, not a linguistic one. (shrink)

Two sources of possible disagreement in bioethics may be associated with pessimism about what bioethics can achieve. First, pluralism implies that bioethics engages with interlocutors who hold divergent moral beliefs. Pessimists might believe that these disagreements significantly limit the extent to which bioethics can provide normatively robust guidance in relevant areas. Second, the interdisciplinary nature of bioethics suggests that interlocutors may hold divergent views on the nature of bioethics itself—particularly its practicality. Pessimists may suppose that interdisciplinary disagreements could frustrate the (...) goals of bioethics. In this article, I explore how wonder may alleviate the concerns of the first group of pessimists regarding problems associated with pluralism, provided that we are willing to accept some interdisciplinary frustrations. Then, I invite readers of this issue of The Journal of Medicine & Philosophy to test these intuitions by considering the role of wonder in these articles. (shrink)

I defend a simple argument for why considerations of epistemic probability should lead us away from Open Future views according to which claims about the future are never true.

The rise of modernity in the 19th century can, among other things, be vividly illustrated by the phenomenal advance of medical profession and, in particular, surgery as its most radical form. In the 20th century, the doctor has already been steadily associated with the phenomenon of power. Medical experiments on human subjects are generally recognized as one of the most extreme manifestations of this discursive nexus. Despite considerable amount of historical research, predominantly dealing with the experiences of Nazi medicine and (...) its patients in concentration camps and some other disciplinary institutions, as well as somewhat gloomy attraction providing for unabated fascination on the part of trivial literature, such experiments have not yet become an established literary topos. The elusive nature of this phenomenon which belongs to the kern of post-war anxiety, but is rarely addressed to, allows me to attest to the fact that human experiments, especially those conducted in Nazi concentration camps, have become a cultural «exclusion zone». Nonetheless, there is a whole range of texts in which attempts has been made to overcome the cultural taboo and to give voice to the victims of human vivisection. Two of them – Friedrich Dürrenmatt’s Suspicion and Marcel Beyers The Flying Foxes – are analyzed in this paper against the backdrop of the so called «intellectual witness» – a concept proposed by Geoffrey Hartman and meaning a distanced witnessing, mostly on behalf of younger generations or, as in case with Dürrenmatt, of those who stayed aside of the World War II. In both novels, the narrative structures of intellectual witnessing serve to surmount the metaphorizing tendency which turns the reality of human experiments into tropes corroborating the author’s idea or concept. Combining documentary elements with the elements of trivial fiction, Dürrenmatt and Beyer make significant strides in deconstructing the naïve «over-identifying» with the victims, as well as their overt fictionalization. As a result, they manage to open the possibility of witness beyond the perspective of eyewitnesses, and at the same time subjugate this rearticulation of the past to the demands of the present situation that facilitates it. This perilous duality reveals the ethical double bind which seems to be inherent to the most reactualizations of utterly traumatic historical experiences, such as human experimentation, transformed into «exclusion zones» and then voiced through intellectual witness and other forms of «secondary witness». In order to become an object of witnessing, those traumatic experiences must correspond to the anxieties of the present originating from them. While triggering the reactualization process, such anxieties, however, also subject the experience of concrete victims to the desiderata of a traumatized «(intellectual) witness» and, in such a way, misuse the tragedy as a means of the witness’s coming to terms with his or her own trauma. This study explored LaRose and Eastin's model of media attendance, within a European context. It extended the uses and gratifications paradigm within the framework of social cognitive theory by instituting new operational measures of gratifications sought, reconstructed as outcome expectations. Although the model of media attendance offers some promising steps forward in measuring media selectivity and usage, and to some extent is applicable to another context of media use, the relative importance of outcome expectancies in explaining media usage and selectivity (...) is not fully supported. Even in its early years, the Internet was recognized as a medium with great potential for lesbians, gay men, and bisexual individuals, especially for LGB youths struggling with their sexual identity. Yet, Internet research related to coming out tends to focus on particular cases or Internet use before and during coming out. Consequently, as such research emphasizes the opportunities and positive aspects of the Internet for LGBs, it may lead to an overestimation of the importance of sexual identity in terms (...) of LGB Internet use. Therefore, in this paper we explore the LGB-specific Internet use of a broad crosssection of the LGB community both before or during and after coming out. Our quantitative online survey and in-depth interviews show that LGBs use the Internet for LGB-oriented purposes less after coming out than before or during it. The results suggest that sexual identity becomes a less salient topic in terms of everyday Internet use after coming out. (shrink)

When young children confront a vast array of adults' testimonial claims, they should decide which testimony to endorse. If they are unable to immediately verify the content of testimonial assertions, children adopt or reject their informants' statements on the basis of forming trust in the sources of testimony. This kind of trust needs to be based on some underlying reasons. The rational choice theory, which currently dominates the social, cognitive, and psychological sciences, posits that trust should be formed on a (...) rational basis, as a result of probabilistic assessments and utility‐maximizing calculations. In this paper, the predictions stemming from the rational choice approach to trust are systematically compared with the empirical evidence from the field of developmental psychology on how children establish their trust in testimonial statements. The results of this comparison demonstrate an obvious inadequacy of the rational choice explanation of the emergence and development of children's testimonial trust, regardless of which form of trust rationality—weighting, threshold, or ordering—is examined. As none of the three forms of rationality of children's trust in testimony squares with the empirical data, this paper introduces a new version of trust rationality, adaptively rational trust. It explores the compatibility of the concept of adaptively rational trust with the recent empirical findings in the area of developmental psychology and addresses some avenues for future research on the rationality of testimonial trust. This article analyzes some prospects for the economic and political development of the United States and China. The first part of the article is devoted to the consideration of strengths and weaknesses of the U.S. model and of the Chinese one. The second part of the article considers the most probable scenarios of the future struggle for world leadership. The first scenario suggests that China will continue developing at a faster rate in the several coming decades and will be gradually (...) catching up with the United States in terms of Gross Domestic Product (GDP) production. However, after several crises in 2008?2020 the United States will likely regain a number of advantages over China as a result of assimilation of the newest technologies. The second scenario suggests that the United States will come across a whole range of internal social and political problems related to internal political splits. In this case, the United States will have to share its global leadership with China. The article attempts to answer the following questions: What scenario is more probable? Will the old world order change? What are the main conclusions for the European Union and for Russia? (shrink)
