Background: There is a shortage of reports on what potential recipients of implantable cardioverter–defibrillators need to be informed about and what role they can and want to play in the decision-making process when it comes to whether or not to implant an ICD.Aims: To explore how patients with heart failure and previous episodes of malignant arrhythmia experience and view their role in the decision to initiate ICD treatment.Patients and methods: A qualitative content analysis of semistructured interviews was used. The study (...) population consisted of 31 outpatients with moderate heart failure at the time of their first ICD implantation.Setting: The study was performed at Sahlgrenska University Hospital, Göteborg, Sweden.Results: None of the respondents had discussed the alternative option of receiving treatment with anti-arrhythmic drugs, the estimated risk of a fatal arrhythmia, or the expected time of survival from heart failure in itself. Even so, very little criticism was directed at the lack of information or the lack of participation in the decision-making process. The respondents felt that they had to rely on the doctors’ recommendation when it comes to such a complex and important decision. None of them regretted implantation of the ICD.Conclusions: The respondents were confronted by a matter of fact. They needed an ICD and were given an offer they could not refuse, simply because life was precious to them. Being able to give well-informed consent seemed to be a matter of less importance for them. The Machiavellian Moment is a classic study of the consequences for modern historical and social consciousness of the ideal of the classical republic revived by Machiavelli and other thinkers of Renaissance Italy. J.G.A. Pocock suggests that Machiavelli's prime emphasis was on the moment in which the republic confronts the problem of its own instability in time, and which he calls the "Machiavellian moment." After examining this problem in the thought of Machiavelli, Guicciardini, and Giannotti, Pocock turns to the revival of (...) republican thought in Puritan England and in Revolutionary and Federalist America. He argues that the American Revolution can be considered the last great act of civic humanism of the Renaissance. The Delegation for Medical Ethics within the Swedish Society of Medicine has taken the initiative to create national ethical guidelines on cardiopulmonary resuscitation. The reasons behind this initiative were indications of differences in the way decisions about cardiopulmonary resuscitation were made and documented and requests expressed by health-care professionals for new national ethical guidelines. During the process of creating the guidelines, a number of workshops were held with representatives from the delegation and clinical experts from various branches of medicine. Several (...) versions of the working document were sent to consultation bodies with requests for comments. We therefore believe that the final guidelines are well supported by the medical profession in Sweden. The purpose of this article is to present ethical issues on which it was difficult to reach consensus due to divergent opinions expressed by the people and organisations involved. In this paper I examine Grace A. de Laguna’s theory of universals in its historical context and in relation to contemporary debates in analytic metaphysics. I explain the central features of her theory, arguing that her theory should be classified as a form of immanent realism and as a powers ontology. I then show in what ways her theory affords a theory of modality in terms of potentialities and discuss some of its consequences along the way.

This book argues for the use of nonideal theory in social ontology. The central claim is that a paradigm shift is underway in contemporary social ontology, from ideal to nonideal, and that this shift should be fully followed through. To develop and defend this central claim, the first step is to show that the key questions and central dividing lines within contemporary social ontology can be fruitfully reconstructed as a clash between two worlds, referred to as ideal and nonideal social (...) ontology. Ideal social ontology is characterized by consensus and cooperation, while nonideal social ontology is characterized by conflict and contestation. The second step is to show that, when taken together, objections to the standard model of ideal social ontology (the dominant version of ideal social ontology) imply that this model needs to be given up in favor of nonideal social ontology. In other words, we should look to nonideal rather than ideal social ontology for core concepts. The third step is to offer a positive account, called the power view, of nonideal social ontology. This account places the concept of social power at the core of a general theory of the social world and replaces the flat and narrow conception of power in ideal social ontology with a richer and more extensive conception. In addition, it rectifies a shortcoming in nonideal social ontology by attending to class, which has been notably and oddly overlooked in the literature. (shrink)
