Anosognosia for hemiplegia is a lack of awareness about paralysis following stroke. Recent explanations use a ‘forward model’ of movement to suggest that AHP patients fail to register discrepancies between internally- and externally-generated sensory information. We predicted that this failure would impair the ability to recall from memory whether information is internally- or externally-generated . Two experiments examined this prediction. Experiment 1 demonstrated that AHP patients exhibit a reality monitoring deficit for non-motor information , whilst hemiplegic controls without anosognosia perform (...) like age-matched healthy volunteers . Experiment 2 explored if this deficit occurs when AHP patients discriminate performed, imagined, or observed movement. Results showed impaired reality monitoring for movements in AHP and nonAHP patients relative to HVs. Findings suggest that reality monitoring processes not directly related to movement, together with a failure to reality monitor movements, contribute to the pathogenesis of AHP. (shrink)

Lane et al. propose an integrative model for the reconsolidation of traces in their timely and impressive article. This commentary draws attention to tradeoffs between accuracy and self-narrative integrity in the model. The tradeoffs concern the sense of agency in memory and its role in both implicit and explicit memory reconsolidation, rather than balances concerning degrees of emotional arousal.

Some monothematic types of delusions may arise because subjects have unusual experiences. The role of this experiential component in the pathogenesis of delusion is still not understood. Focussing on delusions of alien control, we outline a model for reality testing competence on unusual experiences. We propose that nascent delusions arise when there are local failures of reality testing performance, and that monothematic delusions arise as normal responses to these. In the course of this we address questions concerning the tenacity with (...) which delusions are maintained, their often bizarre content, the patients’ inability to dismiss them, and their often circumscribed character. The neuropathologies of the self are disorders of the self and identity that occur in association with neuropathology and include perturbations of the bodily, relational, and narrative self. Right, especially medial-frontal and orbitofrontal lesions, are associated with these conditions. The ego disequilibrium theory proposes this brain pathology causes a disturbance of ego boundaries and functions and the emergence of developmentally immature styles of thought, ego functioning, and psychological defenses including denial, projection, splitting, and fantasy that the NPS patient has in (...) common with the child. I hypothesize that during brain development between approximately ages 3 and 7 immature defensive functions and fantasies tend to be replaced by mature defenses and the inhibition of fantasy a process that depends upon maturational processes within the right hemisphere. I propose a four-tiered model of the NPS that emphasizes a multifactorial approach and includes both negative and positive, bottom up and top down, and neuropsychological and psychological factors. Consciousness is best understood in context, as one element of an interactive waking state in which the greater part of cognitive processing takes place in a nonconscious fashion. But if conscious and nonconscious processing are combined in the waking state, what distinguishes the former form the latter, what is consciousness, and what is its purpose? The answer to the second question depends crucially on our conclusion regarding the first. What is the property in virtue of which a state is conscious (...) rather than nonconscious? In the following, it will be argued that of the answers most frequently proposed—intentionality, subjectivity, accessibility, reflexivity—only the final characteristic, reflexive, autonoetic awareness, is unique to the conscious state. Reflexivity can best be explained not as the product of a self-representational data structure, but as the expression of a recursive processing regime, in which cognition registers the properties of the processing state to a greater extent than properties of the content represented. And the principal characteristic of a reflexive processing state is cognitive reflexivity or autonoetic awareness. (shrink)
