This paper explores ethical questions arising from the work of health practitioners in immigration detention centres in Australia. It raises questions about the roles of professional disciplines and the ways in which they confront dual loyalty issues. The exploration is guided by interviews conducted with health professionals who have worked in asylum seeker detention and an examination of the outsider advocacy role undertaken by the social work profession. The paper discusses the stance taken by individuals and professional associations on participation (...) in controlled settings, including as participant, bystander and advocate, and asks when the provision of care becomes collusion with oppression. Australia has one of the harshest regimes for the processing of asylum seekers, people who have applied for refugee status but are still awaiting an answer. It has received sharp rebuke for its policies from international human rights bodies but continues to exercise its resolve to protect its borders from those seeking protection. One means of doing so is the detention of asylum seekers who arrive in Australia by boat. Health care providers who care for asylum seekers in these conditions (...) experience a conflict of “dual loyalty,” whereby their role in preserving and maintaining the health of patients can run counter to their employment in detention facilities. This article investigates whether or not theories of justice from political philosophy, first, support the position that health research should contribute to justice in global health, and second, provide guidance about what is owed by international clinical research (ICR) actors to parties in low- and middle-income countries. Four theories—John Rawls's theory of justice, the rights-based cosmopolitan theories of Thomas Pogge and Henry Shue, and Jennifer Ruger's health capability paradigm—are evaluated. The article shows that three of the four theories require the (...) conduct of health research for justice in global health. The theories help identify the ends of justice to which ICR is to contribute, but they cannot tell us how to organize ICR to promote these ends. Aside from Ruger's health capability paradigm, the theories also lack an allocative principle for assigning specific duties to specific actors. This creates difficulties for establishing obligations for certain types of ICR actors. (shrink)

Debates about justice in international clinical research problematically conflate two quite different forms of obligation. International research ethics guidelines were intended to describe how to conduct biomedical research in a just manner at the micro or clinical level (within the researcher-participant interaction) but have come to include requirements that are clearly intended to promote justice at the global level. Ethicists have also made a variety of claims regarding what international research should contribute to global justice. This paper argues that the (...) conflation of debates about justice at the micro and macro-levels has not only resulted in the placement of obligations upon the wrong actors but has also served to exclude relevant actors from the ethical picture. Suggestions for who should properly bear macro-level obligations of justice in international clinical research are offered. The paper further contends that, unlike researchers who violate informed consent requirements, no similar type of accountability exists for obligations of global justice, even for those obligation-bearers (incorrectly) identified by current ethics guidelines. Health research has been identified as a vehicle for advancing global justice in health. However, in bioethics, issues of global justice are mainly discussed within an ongoing debate on the conditions under which international clinical research is permissible. As a result, current ethical guidance predominantly links one type of international research (biomedical) to advancing one aspect of health equity (access to new treatments). International guidelines largely fail to connect international research to promoting broader aspects of health equity – namely, healthier (...) social environments and stronger health systems. Bioethical frameworks such as the human development approach do consider how international clinical research is connected to the social determinants of health but, again, do so to address the question of when international clinical research is permissible. It is suggested that the narrow focus of this debate is shaped by high-income countries' economic strategies. The article further argues that the debate's focus obscures a stronger imperative to consider how other types of international research might advance justice in global health. Bioethics should consider the need for non-clinical health research and its contribution to advancing global justice. (shrink)

Many assumptions are made about public willingness to participate in epidemiological research, yet few empirical studies have been conducted to ascertain whether such assumptions are correct. Our qualitative study of the public and of expert stakeholders leads us to suggest that people are generally prepared to participate in epidemiological research, particularly if it is conducted by a trusted public institution such as a government health department, charity, or university. However, there is widespread community distrust of research conducted or sponsored by (...) pharmaceutical companies. Individuals are prompted to take part if the study concerns an illness they or a family member or friend have personally experienced or if they believe the research will confer a widespread public benefit. Preferences vary about the mode of contact for the research to be conducted. Willingness to participate in telephone surveys has decreased in recent years, and this may be a consequence of an increase in calls to homes by telemarketers and market researchers. Participants also stressed the importance of knowing where their names and contact details were sourced and suggested that this information be available to prospective study participants as a matter of course in the first approach or letter. We provide valuable information to epidemiologists in designing studies. (shrink)

It has been suggested that community advisory boards can play a role in minimising exploitation in international research. To get a better idea of what this requires and whether it might be achievable, the paper first describes core elements that we suggest must be in place for a CAB to reduce the potential for exploitation. The paper then examines a CAB established by the Shoklo Malaria Research Unit under conditions common in resource-poor settings – namely, where individuals join with a (...) very limited understanding of disease and medical research and where an existing organisational structure is not relied upon to serve as the CAB. Using the Tak Province Border Community Ethics Advisory Board as a case study, we assess the extent to which it might be able to take on a role minimising exploitation were it to decide to do so. We investigate whether, after two years in operation, T-CAB is capable of assessing clinical trials for exploitative features and addressing those found to have them. The findings show that, although T-CAB members have gained knowledge and developed capacities that are foundational for one-day taking on a role to reduce exploitation, their ability to critically evaluate studies for the presence of exploitative elements has not yet been strongly demonstrated. In light of this example, we argue that CABs may not be able to perform such a role for a number of years after initial formation, making it an unsuitable responsibility for many short-term CABs. Bioethicists have long debated the content of sponsors and researchers' obligations of justice in international clinical research. However, there has been little empirical investigation as to whether and how obligations of responsiveness, ancillary care, post-trial benefits and research capacity strengthening are upheld in low- and middle-income country settings. In this paper, the authors argue that research ethics guidelines need to be more informed by international research practice. Practical guidance on how to fulfil these obligations is needed if research groups and (...) other actors are to successfully translate them into practice because doing so is often a complicated, context-specific process. Case study research methods offer one avenue for collecting data to develop this guidance. The authors describe how such methods have been used in relation to the Shoklo Malaria Research Unit's vivax malaria treatment (VHX) trial (NCT01074905). Relying on the VHX trial example, the paper shows how information can be gathered from not only international clinical researchers but also trial participants, community advisory board members and research funder representatives in order to: (1) measure evidence of responsiveness, provision of ancillary care, access to post-trial benefits and research capacity strengthening in international clinical research; and (2) identify the contextual factors and roles and responsibilities that were instrumental in the fulfilment of these ethical obligations. Such empirical work is necessary to inform the articulation of obligations of justice in international research and to develop guidance on how to fulfil them in order to facilitate better adherence to guidelines' requirements. In response to calls to expand the scope of research ethics to address justice in global health, recent scholarship has sought to clarify how external research actors from high-income countries might discharge their obligation to reduce health disparities between and within countries. An ethical framework—‘research for health justice’—was derived from a theory of justice (the health capability paradigm) and specifies how international clinical research might contribute to improved health and research capacity in host communities. The class of recursive functions over the reals, denoted by , was introduced by Cristopher Moore in his seminal paper written in 1995. Since then many subsequent investigations brought new results: the class was put in relation with the class of functions generated by the General Purpose Analogue Computer of Claude Shannon; classical digital computation was embedded in several ways into the new model of computation; restrictions of were proved to represent different classes of recursive functions, e.g., recursive, primitive recursive (...) and elementary functions, and structures such as the Ritchie and the Grzergorczyk hierarchies.The class of real recursive functions was then stratified in a natural way, and and the analytic hierarchy were recently recognised as two faces of the same mathematical concept.In this new article, we bring a strong foundational support to the Real Recursive Function Theory, rooted in Mathematical Analysis, in a way that the reader can easily recognise both its intrinsic mathematical beauty and its extreme simplicity. The new paradigm is now robust and smooth enough to be taught. To achieve such a result some concepts had to change and some new results were added. (shrink)

How international research might contribute to justice in global health has not been substantively addressed by bioethics. This article describes how the provision of ancillary care can link international clinical research to the reduction of global health disparities. It identifies the ancillary care obligations supported by a theory of global justice, showing that Jennifer Ruger’s health capability paradigm requires the delivery of ancillary care to trial participants for a limited subset of conditions that cause severe morbidity and mortality. Empirical research (...) on the Shoklo Malaria Research Unit’s (SMRU) vivax malaria treatment trial was then undertaken to demonstrate whether and how these obligations might be upheld in a resource-poor setting. Our findings show that fulfilment of the ancillary care obligations is feasible where there is commitment from chief investigators and funders and is strongly facilitated by SMRU’s dual role as a research unit and medical non-governmental organization. As características do ambiente no qual o organismo é exposto podem exercer influências que favorecem ou prejudicam seu desenvolvimento. A família pode ser considerada como o primeiro grupo social com o qual a criança interage e exibe papel privilegiado neste processo. O presente estudo pretendeu descrever e comparar a interação entre as díades mãe-bebê em situação lúdica e na troca de fraldas. Participaram 20 mães de bebês de quatro a seis meses de idade. Para a codificação das interações utilizou-se o (...) Sistema de Codificação da Interação Mãe-Criança Revisado (CITMI-R). Os resultados apontaram que nas duas situações o comportamento materno mais frequente foi o Sensível Neutro e dos bebês foram os de Aproximação Social Neutra e de Jogo. Na situação de cuidado as mães apresentaram, também, o comportamento Protetor Positivo. Conclui-se que houve baixa frequência de comportamentos interativos positivos, apontando para a importância de intervenções junto às mães de bebês. (shrink)

The main objective of this work is to discuss the materiality of relationships and experiences of infants in early childhood education, based on a dialogue with the Brazilian curriculum guidelines for Early Childhood Education in 3 municipalities in the state of São Paulo, and with the concepts of technical objects and individuation developed by Simondon. This is a qualitative research that uses bibliographic and documentary research as a methodology and the production of cartographies of scenes in which relationships between babies (...) and objects are observed. The cartography used here is inspired by the records developed by Fernand Deligny (2015) and its appropriation for research with babies, as proposed by Julia Oliveira (2016). Research was carried out on this topic in the field of education, going through scientific articles and guiding documents for early childhood education. The authors mapped the paths and affections of babies and their relationships with objects in two distinct scenes that were collected within the scope of a collective project. It is a work that contributes to early childhood education in general and, specifically, to baby studies (Infantology, Babyhood studies). (shrink)

O ato de amamentar apresenta diversos benefícios tanto à mãe quanto ao bebê. No entanto, amamentar um bebê com cardiopatia requer inúmeros desafios. Este estudo tem o propósito de compreender a vivência do aleitamento materno de bebês com cardiopatia congênita e as propostas dos serviços de saúde à paciente com essas condições. Estudo descritivo, qualitativo, realizado por meio de entrevista semiestruturada com mães de bebês internados em hospital de referência em cardiopediatria. Utilizou-se a técnica de análise de conteúdo de Minayo, (...) com referencial teórico específico para embasamento da discussão. Os resultados encontrados apontam para a dura realidade dessas mães que se deparam com inúmeras dificuldades diante do enfrentamento da doença e das repercussões emocionais e físicas advindas dela. O impacto emocional e inviabilização dos cuidados com a amamentação exige uma equipe de saúde especializada para oferecer suporte afetivo, fortalecer vínculo mãe-bebê e minimizar prejuízos que a doença pode ocasionar. Les problèmes de classification des noms de parenté tels que papa, maman, tonton, fiston, etc., comme noms propres ou comme noms communs, prennent une nouvelle dimension si l’on intègre, d’une part, certains emplois sans déterminant du nom commun tel que bébé (ex : Si maman va bien, bébé va bien), et d’autre part certains emplois sans déterminant de noms déposés (ex : Restez zen, Twingo maintient la pression de freinage automatiquement). L’objectif principal de cet article est d’utiliser ces emplois pour (...) faire évoluer les théories des noms, propres et communs. A partir de la proposition d’Alan Gardiner, qui fait jouer un rôle central au signifiant du nom propre, complétée par le concept de « sphère de validité » inspiré de Benoit de Cornulier, ce travail jette les bases d’une description unifiée. Reuben Louis Goodstein (1912-1985) foi aluno de Wittgenstein em Cambridge de 1931 a 1934. Neste artigo, faço uma breve descrição de seu trabalho na lógica matemática, no qual se percebe a influência das idéias de Wittgenstein, inclusive a substituição, em seu cálculo equacional, da indução matemática por uma regra de unicidade de uma função definida por uma função recursiva. Esse último aspecto se encontra no Big Typescript de Wittgenstein. Também mostro que as idéias fundamentais do cálculo equacional podem ser encontradas (...) não apenas no período intermediário, mas, in nuce, nas observações sobre matemática do Tractatus Logico-philosophicus. A partir disso, procuro desenvolver um argumento contra uma leitura corrente daquele livro, o assim chamado “Novo Wittgenstein”. Outra conexão entre Goodstein e Wittgenstein se encontra na rejeição da teoria da quantificação; na parte final do artigo, recorro às observações críticas de Goodstein sobre a Lei do Terceiro Excluído (que também incluem uma crítica a Brouwer e à sua rejeição “pela metade” dessa lei) para lançar luz sobre as observações do próprio Wittgenstein a esse respeito. À partir d’une étude de cas issue de la clinique de la dépression postpartum parentale, l’article met en lumière les enjeux de « l’effet bébé » sur l’intrapsychique de la mère et du père mais aussi sur l’intersubjectivité active au sein du couple. La naissance de l’enfant réel et la « renaissance » de l’enfant en soi pour le père et la mère exposent le couple au surgissement de l’histoire infantile et bébé de chacun, avec le risque de la réactivation (...) d’événements traumatiques ou névrotiques. On observe alors dans la dynamique du couple une intersubjectivité qui s’organise à partir de mouvements d’intrication ou de mise en conflit, voire de désintrication, des subjectivités respectives, un peu à l’image de la rythmicité de l’intersubjectivité primaire, pour « le meilleur ou le pire » de leur parentalité ou/et de leur conjugalité. À partir d’une approche psychanalytique lacanienne de la question du symptôme chez l’enfant et de vignettes cliniques, il met au travail l’hypothèse qui soutient ce choix singulier. À savoir : faire du bébé le sujet central du dispositif et orienter le désir (...) soignant vers ses « dires », son corps, ses inventions symptomatiques a des effets thérapeutiques qui permettront au bébé de « déplier son livre » en bricolant de nouveaux nouages pulsionnels à partir de la rencontre avec le corps du soignant. Resumo: Discuto a abordagem de Darwin sobre meios e finalidades de expressões emocionais, com destaque para o choro e sorriso dos bebês; e apresento algumas contribuições, que se depreendem desse conteúdo, para a reflexão das práticas educativas direcionadas à estas crianças que se encontram na primeira etapa da vida. Para tanto, utilizo de descrições e análises apresentadas por Darwin em “A expressão das emoções nos homens e animais”, publicado em 1872 (2009); apresento perspectivas de estudiosos contemporâneos, que tratam das expressões (...) emocionais, tais como: Tomasello (2003) e Damásio (2011). Com base nessas considerações, abordo sobre a importância das expressões emocionais no desenvolvimento de práticas educativas dirigida aos bebês, adicionando ao debate comentários de estudiosos que tratam da primeira infância. Enfatizo o choro e o sorriso como meios naturais de expressão das emoções, que são acionados por estímulos e se dirigem à manifestação de necessidades básicas e estados emocionais, quer sejam excitantes ou deprimentes. Nos bebês, estes meios se apresentam, inicialmente, como ações reflexões; progressivamente, tais meios se mostram cada vez mais complexos. As contribuições que se depreendem dessa discussão podem ser refletidas por cuidadores, no desempenho de suas práticas educativas junto aos bebês, em sintonia com a relação corpo-mente. Darwin's perspective on babies' emotional expressions: contributions to the development of educational practices Abstract: I discuss Darwin's approach to the means and ends of emotional expressions, with emphasis on babies' crying and smiling; and I present some contributions, which are deduced from this content, for the reflection of educational practices aimed at these children who are in the first stage of life. For that, I use descriptions and analyzes presented by Darwin in “The expression of emotions in men and animals”, published in 1872 (2009); I present perspectives of contemporary scholars who deal with emotional expressions, such as: Tomasello (2003) and Damásio (2011). Based on these considerations, I address the importance of emotional expressions in the development of educational practices aimed at babies, adding comments from scholars who deal with early childhood to the debate. I emphasize crying and smiling as natural means of expressing emotions, which are triggered by stimuli and are directed towards the manifestation of basic needs and emotional states, whether they are exciting or depressing. In babies, these means appear, initially, as reflective actions; progressively, such means are becoming more and more complex. The contributions that emerge from this discussion can be reflected by caregivers, in the performance of their educational practices with babies, in tune with the body-mind relationship. La perspectiva de Darwin sobre las expresiones emocionales de los bebés: aportes al desarrollo de las prácticas educativas Abstracto: Analizo el enfoque de Darwin sobre los medios y fines de las expresiones emocionales, con énfasis en el llanto y la sonrisa de los bebés; y presento algunos aportes, que se deducen de este contenido, para la reflexión de las prácticas educativas dirigidas a estos niños que se encuentran en la primera etapa de la vida. Destaco el llanto y la sonrisa como medios naturales de expresión de emociones, que son desencadenados por estímulos y se dirigen a la manifestación de necesidades básicas y estados emocionales, ya sean estos excitantes o deprimentes. En los bebés, estos medios aparecen, inicialmente, como acciones reflexivas; progresivamente, tales medios son cada vez más complejos. Los aportes que emergen de esta discusión pueden ser reflejados por los cuidadores, en el desempeño de sus prácticas educativas con los bebés, en sintonía con la relación cuerpo-mente. La dépression hante notre contemporanéité. « Mal du siècle », elle envahit notamment les lieux de consultation et services hospitaliers accueillant les bébés et leurs parents. Les recherches montrent les impacts de la dépression maternelle sur le développement de l’enfant et la nécessité d’accompagner le processus de parentalité dans ce cas de figure. Après quelques repères théoriques autour de la dépression maternelle et/ou conjointe, l’article propose une thérapeutique qui s’est révélée efficace. Il s’agit d’Accordanse : un dispositif de soin et (...) de recherche accueillant des mères et leurs bébés en souffrance psychique centré sur la danse. Quelques séquences cliniques permettront de mieux mesurer dans quelle mesure la danse soutient la construction d’une chorégraphie des interactions harmonieusement accordée lorsque celle-ci est « empêchée » par la lourdeur de la dépression. Cet article s’inscrit dans le champ de la psychologie transculturelle du bébé et du jeune enfant. Lorsque la pathologie s’invite dans la vie de la jeune mère martiniquaise, cette dernière se trouve confrontée à la fois au discours médico-social (médecine visible) et au discours populaire (médecine invisible), mobilisant alors le double ancrage de sa propre parentalité. L’inscription ancestrale de la parentalité est façonnée par le personnage emblématique du doktè fey (guérisseur). Ceci sera illustré par l’étude de cas d’un bébé de (...) 20 mois face à une mère dépressive, qui est dans un rapport ambivalent avec sa propre mère. Cet article s’inscrit dans le champ de la psychologie transculturelle du bébé et du jeune enfant. Lorsque la pathologie s’invite dans la vie de la jeune mère martiniquaise, cette dernière se trouve confrontée à la fois au discours médico-social (médecine visible) et au discours populaire (médecine invisible), mobilisant alors le double ancrage de sa propre parentalité. L’inscription ancestrale de la parentalité est façonnée par le personnage emblématique du doktè fey (guérisseur). Ceci sera illustré par l’étude de cas d’un bébé de (...) 20 mois face à une mère dépressive, qui est dans un rapport ambivalent avec sa propre mère. El bebé desde que nace, presenta unas necesidades y unas habilidades que quienes están a su cargo han de fomentar y fortalecer. La salud física es fundamental, por supuesto, pero no podemos olvidar la salud afectiva y la salud emocional que permite crear un vínculo entre los bebés y sus cuidadores. Uno de estos aspectos es la lectura.

ABSTRACT The purpose of this paper is to reveal how the responses of the others compose and expand the narratives of an early years teacher and also how these responses appear in narratives in the context of non-face-to-face and asynchronous support, due to the covid-19 pandemic. The narrative methodology of research in education, in dialogue with the contributions of the Bakhtinian philosophy, was used to interpret the responses materialized in the narratives and the way the teacher reorganizes her work based (...) on them. It was shown that whereas the narratives were related to face-to-face work, the babies’ responses came up from the unfinishedness utterances that the teacher produced from her experience with them; as regarding the narratives about non-face-to-face work, the responses came from the utterances produced by the families, based on what they experienced in their relationship with the babies. In both situations, the teacher tried to reorganize her work in relation to the responses of her others. (shrink)

Résumé Les liens psyché-soma retiennent toute l’attention au cours des premiers mois de la vie. Le bébé, placé sous la dépendance étroite de ses parents, bénéficie de soins et d’une attention la plupart du temps adaptés, mais il faut pouvoir prendre en compte le travail psychique que réalisent les parents pour parer à ses états de tension et d’excitation. Ainsi, pour le bébé dont l’organisation psychique est en cours de développement, le système pare-excitation maternel et plus largement parental joue un (...) rôle central dans le développement psychosomatique. La présentation d’une situation clinique permet d’illustrer les propos de l’auteur, de mesurer l’importance du système pare-excitation parental durant les premiers mois de la vie et ses effets, au sein des régulations précoces, en cas de défaillance.
