Background Informed consent is perhaps more relevant to surgical specialties than to other clinical disciplines. Fundamental to this concept is the provision of relevant information for the patient to make an informed choice about a surgical intervention. The opinions of surgeons in Nigeria about informed consent in their practice were surveyed. Methods A cross-sectional survey of surgeons in Nigeria was undertaken in 2004/5 using self-administered semistructured questionnaires. Results There were 102 respondents, 85.3% of whom were men and 58.8% were aged (...) 31–40 years. 43.1% were consultants and 54.0% were surgical trainees. 27.4% were in surgical subspecialties, 26.5% in general surgery and 21.6% were obstetricians and gynaecologists. 54.9% agreed that sufficient information is not provided to patients while obtaining their consent for surgical procedures. They listed medicolegal reasons, informing patients about benefits, risks and alternatives and hospital policy as some reasons for obtaining consent for surgical procedures. When patients decline to give consent for surgery, 84.3% of them thought that poor communication between surgeons and patients may be contributory. They identified taking a course in bioethics during surgical training and compulsory communication skills course as some ways to improve communication between surgeons and patients. Conclusion Most Nigerian surgeons seemed to have a good knowledge of the informed consent requirements and process but fall short in practice. There is a need to improve the surgeon–patient relationship in line with modern exigencies to provide interactive environments for fruitful patient communication and involvement. (shrink)

The sequencing of the human genome with the science and technology by which it occurs is seen as a potential gateway to man's final conquest of most of the health and health related disorders that have for long plagued the human race.While some developing nations (...) like Cuba, Mexico and India have taken the initiative to be major players in the genomics arena by exploring its potentials towards enhancing improved quality of life for their citizens, the majority of others, especially in Africa, still occupy the spectators' seat. If this apparent lukewarm attitude continues, it implies that the present dependence on the developed nations as remote beneficiaries of gains of scientific breakthroughs will persist. It also means that the expectation that genomics should, among other benefits, re-dress the inequalities of access to health care between the rich and poor nations may be a mirage for a long time to come.The expected benefits from the human genome project and genomics technologies are fascinating and hold the ace for improving the standards of living of the African people. However, these expectations are futuristic, time-dependent and capital intensive. They require commitments of national governments to policy re-orientation about research and development, strategic planning, resource mobilization, priority setting; and establishing, promoting and sustaining enabling environments for scientific and technological breakthrough. No doubt, there is dire need for assistance from the developed and the frontline developing nations in this regard. However, great initiatives and deep commitments to making significant scholarly contributions to the advancement of biotechnology and its potentials in the near future must come from within. Background: The formulation and implementation of national ethical regulations to protect research participants is fundamental to ethical conduct of research. Ethics education and capacity are inadequate in developing African countries. This study was designed to develop a module for online training in research ethics based on the Nigerian National Code of Health Research Ethics and assess its ease of use and reliability among biomedical researchers in Nigeria.MethodologyThis was a three-phased evaluation study. Phase one involved development of an online training module (...) based on the Nigerian Code of Health Research Ethics (NCHRE) and uploading it to the Collaborative Institutional Training Initiative (CITI) website while the second phase entailed the evaluation of the module for comprehensibility, readability and ease of use by 45 Nigerian biomedical researchers. The third phase involved modification and re-evaluation of the module by 30 Nigerian biomedical researchers and determination of test-retest reliability of the module using Cronbach’s alpha. Results: The online module was easily accessible and comprehensible to 95% of study participants. There were significant differences in the pretest and posttest scores of study participants during the evaluation of the online module (p = 0.001) with correlation coefficients of 0.9 and 0.8 for the pretest and posttest scores respectively. The module also demonstrated excellent test-retest reliability and internal consistency as shown by Cronbach’s alpha coefficients of 0.92 and 0.84 for the pretest and posttest respectively. Conclusion: The module based on the Nigerian Code was developed, tested and made available online as a valuable tool for training in cultural and societal relevant ethical principles to orient national and international biomedical researchers working in Nigeria. It would complement other general research ethics and Good Clinical Practice modules. Participants suggested that awareness of the online module should be increased through seminars, advertisement on government websites and portals used by Nigerian biomedical researchers, and incorporation of the Code into the undergraduate medical training curriculum. This article featuring Nigeria constitutes one of five articles in a collection of essays on local capacity-building in research ethics by graduates from the University of Toronto’s Joint Centre for Bioethics MHSc in Bioethics, International Stream programme funded by the Fogarty International Center for Advanced Study in the Health Sciences (FIC). The first National Health Research Ethics Committee was inaugurated in 2006. Since then, more institutional health research ethics committees continue to be formed. However, research ethics challenges in Nigeria are (...) systemic and require a systems approach to address them effectively. Nigeria requires capacity-building for authentic acculturation of health systems as well as for health research, education, and advocacy within the research community and the general public. Studies on informed consent to medical research conducted in low or middle-income settings have increased, including empirical investigations of consent to genetic research. We investigated voluntary participation and comprehension of informed consent among women involved in a genetic epidemiological study on breast cancer in an urban setting of Nigeria comparing women in the case and control groups.

_The goal of this study is to investigate the ethnic conflict in southern Kaduna in connection to eastern Ukraine, which has now spread internationally as a result of major countries' action on Ukraine's behalf. The Hausa-Fulani are Muslims, whereas indigenous Christian communities make up the bulk of the population in southern Kaduna. Although the root causes of the situation in southern Kaduna are multifaceted given their theological and sociopolitical undertones, they are primarily cultural and patriotic in the case of Ukraine. (...) The special status of the Russian language is a subject of heated debate among those on the side of Ukraine's government and opposition nationalists who are pro-Russia. The study's purpose dictated that premordialism theory be used as a working model. Ethnic activists and nationalists commonly employ primordialist ideas to explain and defend ethnic wars, like the ones under study (in southern Kaduna and eastern Ukraine). In addition to hoping that the United Nations is permitted to fulfil the purpose for which it was established so that peace can return to Ukraine, we also hope that the same can be achieved in southern Kaduna if the recommendations of the state and federal governments assigned to investigate the crisis are dusted off and implemented._ _In conclusion, it is hard to discuss peace in an environment rife with injustice and nepotism. More peace advocacy is needed from the main international political powers, but they can only do that if they allow the United Nations (UN) to function freely. Primordialist theories are frequently used by ethnic activists and nationalists to explain and justify ethnic conflicts, such as those in southern Kaduna and eastern-southern Ukraine, which is why this research supports the use of the primordial theory of ethnic conflict despite its flaws._. This paper addresses the question “what connection is there between our answer to the question of what we are, and the question, what our actions are?” Suppose that actions are reflexive changes of agents. On that supposition, there would be a direct connection between the answers to those two questions. An action of mine will be a reflexive change of me, and what I am will fix the nature of those changes. I hold that supposition to be true and consider (...) reasons in favor of believing it. However, the paper is not primarily aimed at defense of that thesis. It rather concerned with exploring what consequences accepting it has for the competing notions of what we are, given what we ordinarily think actions are, and bringing to light a tension between thinking of actions as reflexive changes of agents in this way, and a kind of causal understanding of actions that is prevalent. What emerges is that we should shift where we start our theorizing: we cannot assume that action theory primarily involves the task of characterizing the relation between an agent and changes caused, rather than a characterization of a particular kind of relation between the agent and herself. This paper advances the thesis that sensory concepts have as a semantic component the first-person indexical. It is argued that the private nature of our access to our own sensations forces, in our talking about them, an indexical reference to the inner states of the speaker in lieu of publicly accessible properties by which reference is usually fixed. Indexicals, such as ‘here’, can be understood despite ignorance of their referent. Such is the case with sensory terms. Furthermore, the thesis that (...) sensory terms are indexical has considerable explanatory power. I give two examples: firstly, I argue that clashes of intuition over Block's ‘inverted spectrum’ thought experiment can be explained by appeal to semantic properties of indexicals. Secondly, I argue that multiple realisability intuitions can be shown to be consistent with the view that sensations are type-identical with brain states. (shrink)

Зазначено, що отримання православними віруючими України Томосу про визнання автокефалії і конституювання Православної Церкви України позитивно впливає не лише на розвиток самої ПЦУ, але й рівня патріотизму, відновлення і розвою (...) україномовного середовища, архетипних традицій, обрядів, поведінкових моделей тощо. Resumen El artículo se centra en el poema Seudoaraucana de Elvira Hernández (2010; 2017) a partir de la reescritura como recurso articulador de una mirada descentrada y amplificada del texto fuente, La Araucana de Ercilla y Zúñiga. Como hipótesis se plantea que la reescritura discute fenómenos sociales vinculados con las fisuras de la nación chilena y los procesos políticos de la actual coyuntura. Como objetivo, se busca establecer un diálogo con la tradición y el campo literario en torno a la (...) épica, por un lado, y con las reconsideraciones críticas acerca de la imaginación poética de la historia, por otro. Abstract The article focuses on the poem Seudoaraucana by Elvira Hernández (2010; 2017) based on the rewriting as an articulating resource of an off-center and amplified look at the source text, La Araucana de Ercilla y Zúñiga. As a hypothesis, it proposes that the rewriting discusses social phenomena linked to the fissures of the Chilean nation and the political processes of the current situation. As an objective, it tries to establish a dialogue with the tradition and the literary field around the epic, on the one hand, and with critical reconsiderations about the poetic imagination of history, on the other. _Purpose._ This article discusses researching the nature and basic structure of acts of empathy. Such research first requires answering the question: are empathic acts intentional acts of our consciousness? If the answer to this question is affirmative, then there is a need to answer the following questions: what are the features of acts of empathy as intentional ones? And can such acts be qualified as opening a special and complex type of access (epistemic, social, and ethical) to "other minds"? _Theoretical (...) basis._ The research is based on the problems set by the phenomenological tradition and the developed analytical philosophy, which found their continuation in the philosophy of mind. With the tendency to naturalization, representatives of these areas have another common sphere in the research of mental reality. The changes that both traditions are undergoing can be observed in the transformation of fundamental theoretical concepts such as "intentionality". The linguistic turn gives impetus to the development of new theoretical approaches to the understanding of consciousness, which give preference to the research of language rather than the research in the structure of the Self. The change in methodological guidelines is due to a few factors. In particular, this is a noticeable progress in empirical sciences and the dominance of Behaviorism guidelines in psychology, as well as the fact that the Anglo-American tradition inherits several features intrinsic to British empiricism. Today, three main directions of understanding intentionality can be distinguished. Starting from Brentano and Husserl, we have a classical theory – mental intentionality (phenomenological). As part of analytical philosophy and philosophy of language, we can talk about linguistically interpreted intentionality. Sellars’ model of intentionality has a definite linguistic component. Also, a modern version that functions in the philosophy of consciousness: attempts to naturalize intentionality (this is, for example, the research of Galen Strawson and Daniel Dennett). _Originality._ The problem is studied not in the context of the phenomenological or analytical traditions in their isolation, but in a problematic-conceptual way, which allows us to reach a new level of generalization and reveal the theoretical advantages of combining both traditions. _Conclusions._ Empathic Acts can be defined as intentional within the classical phenomenological tradition. As intentional acts, they have their specificity in that they are directed to the "other mind" and can form the basis for the research of intersubjectivity. For the analytic tradition and early philosophy of consciousness, such statements are not obvious. However, discussions about the role of corporality and the problem concerning embodied cognition are becoming increasingly common today. In such discussions, empathy, as an element in the knowledge of others and a possibility for grounding the social sciences, seems more promising than attempts to make epistemic access to other minds possible on the basis of the argument by analogy or on the basis of the inference to the best explanation. Appeals to nature are ubiquitous in Epicurean ethics and politics. The foundation of Epicurean ethics is its claim that pleasure is the sole intrinsic good and pain the sole intrinsic evil, and this is supposedly shown by the behavior of infants who have not yet been corrupted, "when nature's judgement is pure and whole." Central to their recommendations about how to attain pleasure is their division between types of desires: the natural and necessary ones, the natural but non-necessary ones, and (...) the vain and empty ones. Elsewhere, the Epicureans talk about the "natural goods" of political power and fame, and they contrast "natural wealth" with wealth as "defined by empty opinion." Finally, in their politics, Epicurus claims that the "the justice of nature is a pledge of reciprocal usefulness, [i.e.,] neither to harm one another nor to be harmed." This paper explores two questions regarding these various appeals to nature. The first is: what is it for these things to be natural, i.e., what notion of "natural" or "nature" is at play here? (Furthermore, is there a single notion being used across these appeals, and if not, how are they related?) The second is: what normative work does a thing's being natural do? That is, what reason, if any, does a desire's being natural give me for pursuing the object of that desire and trying to fulfill that desire, as opposed to not doing so and trying to eliminate it, and similarly for the other appeals to nature? Edmund Burke, long considered modern conservatism's founding father, is also widely believed to be an opponent of empire. However, Daniel O'Neill turns that latter belief on its head. This fresh and innovative book shows that Burke was a passionate supporter and staunch defender of the British Empire in the eighteenth century, whether in the New World, India, or Ireland. This book discusses Thomistic commentary on the topics of physical premotion, grace, and the permission of sin, especially as these relate to the mysteries of predestination and reprobation. The author examines the fundamental tenets of the classical Thomistic account, and on this basis critiques the 20th century revisionist theories of Domingo Banez, Reginald Garrigou-Lagrange, Francisco Marin-Sola, Jacques Maritain, Bernard Lonergan, and Jean-Herve Nicolas. In conclusion, the implications of the traditional view are considered in light of the spiritual life.

Semantic deference is the apparent phenomenon whereby some of -/- our concepts have their content fixed by the minds of others. The -/- phenomenon is puzzling both in terms of how such concepts are -/- supposed to work, but also in terms of why we should have -/- concepts whose content is fixed by others. Here I argue that if we -/- rethink semantic deference in terms of extended mind reasoning -/- we find answers to both of these questions: the (...) minds of others -/- can be understood to play a role in storing the semantic knowledge -/- underpinning our concepts without undermining their -/- functionality, and this ‘outsourcing’ of semantic knowledge greatly -/- expands our overall knowledge-bearing capacity, both at the level -/- of the individual and the community. I conclude that deference in -/- fact affords stronger grounds for a social expansion of knowledge -/- than standard extended mind reasoning permits. (shrink)

Anaxarchus accompanied Pyrrho on Alexander the Great’s expedition to India and was known as “the Happy Man” because of his impassivity and contentment. Our sources on his philosophy are limited and largely consist of anecdotes about his interactions with Pyrrho and Alexander, but they allow us to reconstruct a distinctive ethical position. It overlaps with several disparate ethical traditions but is not merely a hodge-podge; it hangs together as a unified whole. Like Pyrrho, he asserts that things are indifferent in (...) value and that realizing this indifference leads to contentment. But this doctrine of indifference is rooted in Democritean atomism. And in his pursuit of pleasure and dismissiveness of conventional standards of what is just, noble, and pious, Anaxarchus is closer to fifth century thinkers such as Aristippus, Antiphon, and Critias.
