Progress in psychiatry depends on accurate definitions of disorders. As long as there are no known biologic markers available that are highly specific for a particular psychiatric disorder, clinical practice as well as scientific research is forced to appeal to clinical symptoms. Currently, the nosology of obsessive-compulsive disorder is being reconsidered in view of the publication of DSM-V. Since our diagnostic entities are often simplifications of the complicated clinical profile of patients, definitions of psychiatric disorders are imprecise and always indeterminate. (...) This urges researchers and clinicians to constantly think and rethink well-established definitions that in psychiatry are at risk of being fossilised. In their recent paper, Natalie Banner and Tim Thornton evaluate seven volumes of the Oxford University Press series “International Perspectives in Philosophy and Psychiatry,” an international book series begun in 2003 focusing on the emerging interdisciplinary field at the interface of philosophy and psychiatry. According to Natalie Banner and Tim Thornton, the series represents a clear indication that the interdisciplinary field of philosophy of psychiatry has been flourishing lately. Philosophers and psychiatrists face a “new philosophy of psychiatry”. However, the optimism (...) which the “new” philosophy of psychiatry celebrates is precisely the exiling of philosophy from the foundations of psychiatry. The 150 year old belief that psychopathology cannot do without philosophical reflection has virtually disappeared from common psychiatric education and daily clinical practice. Though the discipline of psychiatry is particularly suited to contributions from philosophy, the impact of philosophy on psychiatry nowadays remains limited. With some exceptions, philosophical papers are embedded in a philosophical context inscrutable to ordinary psychiatrists. Much current philosophical work is perceived by psychiatrists as negativistic. I would encourage the field of psychiatry to incorporate once again basic philosophical attitudes which render possible true dialogue with philosophy and enrich both disciplines. The views developed here should not discredit the value and importance of Natalie Banner and Tim Thornton’s paper and the excellent series “International Perspectives in Philosophy and Psychiatry.” As Jaspers said “Everybody inclined to disregard philosophy will be overwhelmed by philosophy in an unperceived way”. In this chapter we give an overview of current and historical conceptions of the nature of obsessions and compulsions. We discuss some open questions pertaining to the primacy of the affective, volitional or affective nature of obsessive-compulsive disorder (OCD). Furthermore, we add some phenomenological suggestions of our own. In particular, we point to the patients’ need for absolute certainty and the lack of trust underlying this need. Building on insights from Wittgenstein, we argue that the kind of certainty the patients (...) strive for is unattainable in principle via the acquisition of factual knowledge. Moreover, we suggest that the patients’ attempts to attain certainty are counter-productive as their excessive conscious control in fact undermines the trust they need. (shrink)

We propose to understand social affordances in the broader context of responsiveness to a field of relevant affordances in general. This perspective clarifies our everyday ability to unreflectively switch between social and other affordances. Moreover, based on our experience with Deep Brain Stimulation for treating obsessive-compulsive disorder (OCD) patients, we suggest that psychiatric disorders may affect skilled intentionality, including responsiveness to social affordances.

People suffering from Obsessive-Compulsive Disorder (OCD) do things they do not want to do, and/or they think things they do not want to think. In about 10 percent of OCD patients, none of the available treatment options is effective. A small group of these patients is currently being treated with deep brain stimulation (DBS). Deep brain stimulation involves the implantation of electrodes in the brain. These electrodes give a continuous electrical pulse to the brain area in which they are implanted. (...) It turns out that patients may experience profound changes as a result of DBS treatment. It is not just the symptoms that change; patients rather seem to experience a different way of being in the world. These global effects are insufficiently captured by traditional psychiatric scales, which mainly consist of behavioural measures of the severity of the symptoms. In this article we aim to capture the changes in the patients’ phenomenology and make sense of the broad range of changes they report. For that we introduce an enactive, affordance-based model that fleshes out the dynamic interactions between person and world in four aspects. The first aspect is the patients’ experience of the world. We propose to specify the patients’ world in terms of a field of affordances, with the three dimensions of broadness of scope (‘width’ of the field), temporal horizon (‘depth’), and relevance of the perceived affordances (‘height’). The second aspect is the person-side of the interaction, that is, the patients’ self-experience, notably their moods and feelings. Thirdly, we point to the different characteristics of the way in which patients relate to the world. And lastly, the existential stance refers to the stance that patients take towards the changes they experience: the second-order evaluative relation to their interactions and themselves. With our model we intend to specify the notion of being in the world in order to do justice to the phenomenological effects of DBS treatment. (shrink)

According to the traditional Western concept of freedom, the ability to exercise free will depends on the availability of options and the possibility to consciously decide which one to choose. Since neuroscientific research increasingly shows the limits of what we in fact consciously control, it seems that our belief in free will and hence in personal autonomy is in trouble. -/- A closer look at the phenomenology of Obsessive-Compulsive Disorder (OCD) gives us reason to doubt the traditional concept of freedom (...) in terms of conscious control. Patients suffering from OCD experience themselves as unfree. The question is whether their lack of freedom is due to a lack of will power. Do they have too little conscious control over their thoughts and actions? Or could it be the opposite: are they exerting too much conscious control over their thoughts and actions? -/- In this chapter, we will argue that OCD patients testify to the general condition that exercising an increased conscious control over actions can in fact diminish the sense of agency rather than increase the experience of freedom. The experiences of these patients show that the traditional conception of freedom in terms of ‘free will’ has major shortcomings. There is an alternative, however, to be found in the work of Hannah Arendt. She advocates a conception of freedom as freedom in action. Combined with phenomenological insights on action, Arendt’s account of freedom helps us to get a more adequate understanding of the role of deliberation in the experience of freedom. We argue that the experience of freedom depends on the right balance between deliberate control and unreflective actions. (shrink)

Deep Brain Stimulation (DBS) is a relatively new, experimental treatment for patients suffering from treatment-refractory Obsessive Compulsive Disorder (OCD). The effects of treatment are typically assessed with psychopathological scales that measure the amount of symptoms. However, clinical experience indicates that the effects of DBS are not limited to symptoms only: patients for instance report changes in perception, feeling stronger and more confident, and doing things unreflectively. Our aim is to get a better overview of the whole variety of changes that (...) OCD patients experience during DBS treatment. For that purpose we conducted in-depth, semi-structured interviews with 18 OCD patients. In this paper, we present the results from this qualitative study.We list the changes grouped in four domains: with regard to (a) person, (b) (social) world, (c)characteristics of person-world interactions, and (d) existential stance. We subsequently provide an interpretation of these results. In particular, we suggest that many of these changes can be seen as different expressions of the same process; namely that the experience of anxiety and tension gives way to an increased basic trust and increased reliance on one’s abilities. We then discuss the clinical implications of our findings, especially with regard to properly informing patients of what they can expect from treatment, the usefulness of including CBT in treatment, and the limitations of current measures of treatment success. We end by making several concrete suggestions for further research. (shrink)

Does DBS change a patient’s personality? This is one of the central questions in the debate on the ethics of treatment with Deep Brain Stimulation (DBS). At the moment, however, this important debate is hampered by the fact that there is relatively little data available concerning what patients actually experience following DBS treatment. There are a few qualitative studies with patients with Parkinson’s disease and Primary Dystonia and some case reports, but there has been no qualitative study yet with patients (...) suffering from psychiatric disorders. In this paper, we present the experiences of 18 patients with Obsessive-Compulsive Disorder (OCD) who are undergoing treatment with DBS. We will also discuss the inherent difficulties of how to define and assess changes in personality, in particular for patients with psychiatric disorders. We end with a discussion of the data and how these shed new light on the conceptual debate about how to define personality. (shrink)

Despite technological innovations, clinical expertise remains the cornerstone of psychiatry. A clinical expert does not only have general textbook knowledge, but is sensitive to what is demanded for the individual patient in a particular situation. A method that can do justice to the subjective and situation-specific nature of clinical expertise is ethnography. Effective deep brain stimulation (DBS) for obsessive-compulsive disorder (OCD) involves an interpretive, evaluative process of optimizing stimulation parameters, which makes it an interesting case to study clinical expertise. The (...) aim of this study is to explore the role of clinical expertise through an ethnography of the particular case of DBS optimization in OCD. In line with the topic of the special issue this article is a part of, we will also use our findings to reflect on ethnography as a method to study complex phenomena like clinical expertise. This ethnography of DBS optimization is based on 18 months of participant observation and nine in-depth interviews with a team of expert clinicians who have been treating over 80 OCD patients since 2005. By repeatedly observing particular situations for an extended period of time, we found that there are recurrent patterns in the ways clinicians interact with patients. These patterns of clinical practice shape the possibilities clinicians have for making sense of DBS-induced changes in patients’ lived experience and behavior. Collective established patterns of clinical practice are dynamic and change under the influence of individual learning experiences in particular situations, opening up new possibilities and challenges. We conclude that patterns of clinical practice and particular situations are mutually constitutive. Ethnography is ideally suited to bring this relation into view thanks to its broad temporal scope and focus on the life-world. Based on our findings, we argue that clinical expertise not only implies skillful engagement with a concrete situation but also with the patterns of clinical practice that shape what is possible in this specific situation. Given this constraining and enabling role of practices, it is important to investigate them in order to find ways to improve diagnostic and therapeutic possibilities. (shrink)

We whole-heartedly agree with Mecacci and Haselager(2014) on the need to investigate the psychosocial effects of deep brain stimulation (DBS), and particularly to find out how to prevent adverse psychosocial effects. We also agree with the authors on the value of an embodied, embedded, enactive approach (EEC) to the self and the mind–brain problem. However, we do not think this value primarily lies in dissolving a so-called “maladaptation” of patients to their DBS device. In this comment, we challenge three central (...) claims of the authors on the basis of our direct experience with psychosocial effects of DBS in 45 obsessive- compulsive disorder (OCD) patients treated at the AMC in Amsterdam, The Netherlands, and our indepth qualitative interviews with 18 of them (de Haan et al. 2013). Franz Brentano’s impact on the philosophy of his time and on 20th-century philosophy is considerable. The “sharp dialectician” (Freud) and “genial master” (Husserl) influenced philosophers of various allegiances, being acknowledged not only as the “grandfather of phenomenology” (Ryle) but also as an analytic philosopher “in the best sense of this term” (Chisholm). The fourteen new essays gathered together in this volume give an insight in three core issues of Brentano’s philosophy: consciousness (sect.1), intentionality (sect. 2) and ontology and metaphysics (sect. (...) 3). Two further sections of the volume deal with the posterity of his philoso¬phy: in section 4, the legacy of his account of sense perception and feeling is discussed, while the history of Brentano’s unpublished manuscripts is discussed in section 5. This section also presents an edition of a manuscript from 1899 on relations, along with the letters from Brentano to Marty which discuss this manuscript. The central goal of this paper is to investigate the injustices that can occur with the use of new technologies, especially Artificial Intelligence (AI), focusing on the issues concerning respect to human rights and the protection of victims and the most vulnerable. We aim to study the impacts of AI in daily life and the possible threats to human dignity imposed by it, such as discrimination based on prejudices, identity-oriented stereotypes, and unequal access to health services. We characterize such cases (...) as algorithmic injustices. In the final part of the text, we propose some strategies to confront this specific type of injustice. (shrink)

In 1484, humanist philosopher and theologian Marsilio Ficino published the first complete Latin translation of Plato's extant works. Students of Plato now had access to the entire range of the dialogues, which revealed to Renaissance audiences the rich ancient landscape of myths, allegories, philosophical arguments, etymologies, fragments of poetry, other works of philosophy, aspects of ancient pagan religious practices, concepts of mathematics and natural philosophy, and the dialogic nature of the Platonic corpus's interlocutors. By and large, Renaissance readers in the (...) Latin West encountered Plato's text through Ficino's translations and interpretation. -/- In Plato's Persona, Denis J.-J. Robichaud provides the first synthetic study of Ficino's interpretation of the Platonic corpus. Robichaud analyzes Plato's works in their original Greek and in Ficino's Latin translations, as well as Ficino's non-Platonic writings and correspondence, in the process uncovering new aspects of Ficino's intellectual work habits. In his letters and works, Ficino self-consciously imitated a Platonic style of prose, in effect devising a persona for himself as a Platonic philosopher. Plato's dialogues are populated with a wealth of literary characters with whom Plato interacts and against whom Plato refines his own philosophies. Reading through Ficino's translations, Robichaud finds that the Renaissance philosopher seeks an understanding of Plato's persona(e) among all the dialogues' interlocutors. In effect, Ficino assumed the role of Plato's Latin spokesperson in the Renaissance. -/- Plato's Persona is grounded in an extensive study of scholarship in Renaissance humanism, classics, philosophy, and intellectual history, and contextualizes Ficino's intellectual achievements within the contemporary Christian orthodox view of Platonism. Ficino was an influential figure in the early Italian Renaissance: the key intermediary between Greek and Latin, and between manuscript and print, giving voice to Plato and access to the ancient frameworks needed to interpret his dialogues. Since the 1970s, scholars have begun to pay special attention to the questions of whether democracy guarantees peace, whether freedom should be sacrificed in the name of security during war, how sustainable peace is possible, and what threats war poses to democracy. In the same period, influenced by the legacy of Immanuel Kant and David Hume, the democratic peace hypothesis began to be developed. This article discusses the theoretical debate concerning this hypothesis, as well as the question of whether the (...) type of political regime affects the state's success in war. An examining of the theoretical debates has shown that the proponents of the democratic peace hypothesis have not been able to provide convincing evidence of a direct link between the type of political regime and the willingness to initiate war or maintain peace. At the same time, the debate disproves another common belief, that of the military weakness of democracies. The article notes that the prevalent theoretical approach to studying the issue of war and democracy is based on an eschatological idea of the future democratic world. In contrast to this view, Gunther Anders’ idea of an “apocalypse without a Kingdom” opens up a new perspective for understanding war and democracy, which is that the destruction that war brings is not followed by the construction of a new world. In this case, the main task is not to achieve democracy as a result of war, but to preserve democracy during war. (shrink)

Nous explorons l'espace en le traversant, mais aussi en écoutant les descriptions qui nous en sont faites, en étudiant cartes, atlas ou supports numériques. Nous mémorisons des itinéraires, nous comparons des distances, nous retrouvons notre point de départ après un long trajet. Nos capacités de raisonnement nous permettent d'imaginer des raccourcis, de créer de nouveaux parcours, en un mot, de manifester notre adaptation (...) à l'environnement. Les technologies numériques étendent cette capacité en offrant à l'individu de nouvelles formes d'assistance au déplacement. Ces dispositifs sont particulièrement utiles pour les personnes souffrant de déficits visuels ou encore d'atteintes neurologiques spécifiques. Quelles capacités mentales l'individu doit-il mobiliser lorsqu'il est confronté à l'espace? Quelles fonctions cérébrales met-il en oeuvre? La psychologie et les neurosciences jouent un rôle primordial pour répondre à ces questions, comme en témoignent plusieurs chapitres de ce livre. Mais un ouvrage consacré à l'espace doit aussi donner leur place à d'autres disciplines concernées par la question : la géométrie, la géographie, l'urbanisme, l'architecture, la peinture, le cinéma, etc. Les sciences du langage participent elles aussi à ce concert scientifique, lorsqu'elles analysent la manière dont l'art littéraire est mis au service de la description de l'espace. Goncharov and Peretyat'kin independently gave necessary and sufficient conditions for when a set of types of a complete theory is the type spectrum of some homogeneous model of. Their result can be stated as a principle of second order arithmetic, which is called the Homogeneous Model Theorem (HMT), and analyzed from the points of view of computability theory and reverse mathematics. Previous computability theoretic results by Lange suggested a close connection between HMT and the Atomic Model Theorem (AMT), which states (...) that every complete atomic theory has an atomic model. The authors show that HMT and AMT are indeed equivalent in the sense of reverse mathematics, as well as in a strong computability theoretic sense and do the same for an analogous result of Peretyat'kin giving necessary and sufficient conditions for when a set of types is the type spectrum of some model. Grand philosophe, grand mathématicien, Descartes a-t-il été aussi un grand écrivain? La question est classique. A l'évidence, il existe un style cartésien, dont le Discours de la Méthode offre une sorte de bouquet, avec ses longues phrases à la construction puissante et aux savantes modulations. Ce style résulte d'une éducation littéraire que Descartes n'a en vérité jamais répudiée. De quoi cette éducation a-t-elle été faite? Quelle relation le philosophe entretient-il avec le langage? Comment passe-t-il du latin, " langue de ses (...) précepteurs ", au français, " langue de son pays "? De quel travail de la pensée la phrase cartésienne est-elle le théâtre? Ces questions n'ont rien de subsidiaire. Se pencher sur le style de Descartes, c'est s'engager dans un voyage à travers ses textes. Des notes de jeunesse jusqu'aux dernières lettres, on y rencontrera des paysages presque ignorés, où s'incarne, au plus loin des simplifications scolaires, une pensée d'une audace restée inouïe. Il existe, en français, un ouvrage de référence sur la pensée anthropologique de Humboldt (Jean Quillien, 1991, réédité en 2015) mais rien de tel sur sa pensée du langage. Ce livre souhaite donc combler cette lacune tout en remettant à leur place certaines idées fausses. Il se révèle, à ce titre, triplement provocateur. D'une part, cet ouvrage rappelle que Humboldt se situe entièrement dans le prolongement de la tradition généraliste des grammaires philosophiques qu'il ne renie jamais entièrement. Il relit au (...) contraire cette tradition au moyen des conceptions de la philosophie allemande de Kant et Fichte. D'autre part, il souligne que Humboldt ne s'est pas contenté de formuler quelques idées générales sur le langage, mais a effectué un immense travail empirique sur près de 80 langues, rédigeant plus de 30 grammaires. Denis Thouard donne un aperçu de ce travail en évoquant l'édition des grammaires américaines effectuée ces dernières années à l'Académie de Berlin-Brandebourg, et développe le cas du basque, du chinois et de l'égyptien. C'est l'occasion de découvrir un linguiste véritable. Enfin, il permet de remettre en lumière le rôle essentiel et le plus souvent ignoré qu'a joué pour la pensée de Humboldt sa réflexion sur la traduction en analysant son travail de traducteur de l'Agamemnon d'Eschyle dans son contexte culturel. Une réflexion sur la question de la diversité des langues dans l'Europe actuelle, le renouveau des études sur l'origine des langues et les mésusages des idées de Humboldt accompagnent une présentation sélective et approfondie des points essentiels de sa pensée. Ils se répandent dans les sciences économiques et sociales - comme dans la plupart des sciences des systèmes complexes. Des énigmes épistémologiques (ré)apparaissent. On a souvent opposé modèles et investigations empiriques : d’un côté, on considère les sciences empiriques fondées sur une observation méthodique (enquêtes, expériences) tandis que de l’autre, on conçoit les approches théoriques et la modélisation comme s’appuyant sur une (...) approche conceptuelle ou hypothético-déductive. la simulation a de son coté souvent été comparée à une forme d’expérience ou à une méthode intermédiaire entre la théorie et l’expérience. Tesfatsion a parlé de « laboratoire computationnel » comme d’une manière d’étudier « les comportements des systèmes complexes au moyen d’expériences sous contrôle qu’on peut répéter (réplicable)» et Axelrod a proclamé que la simulation serait « une troisième manière de pratiquer la science dans les sciences sociales », entre l’induction et la déduction. Nous introduisons de nouveaux concepts épistémologiques afin de montrer dans quelle mesure les auteurs ont raison lorsqu'ils se concentrent sur une signification empirique, instrumentale ou conceptuelle de leur modèle ou simulation. La première partie du chapitre discute plus particulièrement de l'épistémologie des modèles économiques à partir des travaux d’Hausman, Guala, Mäki, Sugden, Schelling etc. pour expliquer les divergences entre modèles et simulations. En distinguant entre modèles et simulations, comme entre entre types de modèles, types de simulations informatiques et types d'empiricité obtenue par une simulation, la seconde partie donne la possibilité de comprendre plus précisément - et ensuite de justifier - la diversité des positions épistémologiques présentées dans la première partie. En utilisant les notion de sous-symbolisation (due à Smolensky) et, de hiérarchie dénotationnelle (due à Goodman), une attention particulière est accordée à la multiplicité des pouvoirs dénotationnels des symboles en jeu dans les modèles complexes et les simulations informatiques. In Pascal’s Pensées, at the very end of the so-called “wager” fragment, we find this intriguing remark intended to win once and for all the whole conviction of the “libertine” the apologist is addressing to : “I will tell you that you will win thereby in this life”. This article endeavours to reconsider this remark seriously, as it has been somewhat neglected by the canonical commentaries on the “wager”. We begin by determining the reasons that lead the apologist to this (...) final remark, then we attempt to define its meaning and scope in Pascal’s apologetic project. Next we seek to identify the “gain in this life” that is promised to the libertine who adopts a Christian lifestyle. This leads to study how Pascal analyses joy. To conclude we propose some more general thoughts about the strategies this fragment of Les Pensées suggests to today’s Christian apologetics. (shrink)

This paper is about the question: what is it for a mental state to mean (or present) something as an intentional object? This issue is addressed from a broad perspective, against the background of Brentano’s philosophical programme in Psychology from an empirical standpoint, and the controversy between the proponents of a non-canonical interpretation of Brentano’s theory of intentionality, and the so-called orthodox interpretation advocated namely by R. Chisholm. My investigation is divided into six parts. In the first section, I explain (...) the meaning and function of the notion of phenomenon in light of Brentano’s philosophical programme, and I briefly elucidate the notion of physical phenomenon which, in Brentano’s Psychology, constitutes the primary object of consciousness. In the next two sections, I look at two aspects of Brentano’s criticism of the identity thesis that he attributes to British empiricism, namely the psychological aspect, which concerns the identification of the two classes of phenomena, and the metaphysical aspect relating to the relationship between physical phenomena and the reality of an extramental world. Once this double distinction will be established, I will turn to the relation of intentional objects to presentations and put forward the hypothesis that intentional objects are conceptually dependent upon presentations and that this dependence rests, in turn, upon the content of the mental phenomena. The next step concerns Brentano’s theory of primary and secondary objects, and one of the main non-orthodox arguments against the canonical interpretation, namely that the latter conflates primary and secondary objects. In this context, I examine a second hypothesis: that the secondary object, or intentional correlate of an act, is an intentional content, which is distinct both from the intentional object and from the reality to which it relates, and I maintain that the intentional content has the function of mediating the mental acts’ relation to their objects. Finally, I shall examine some objections against the hypothesis of intentional content in Brentano’s Psychology, and I will conclude with a brief commentary on the bearing of this investigation with regard to the interpretation of the genesis of Brentano’s theory of intentionality, before and after the reistic turn of his philosophy.
