Checklists have been used to improve quality in many industries, including healthcare. The use of checklists, however, has not been extensively evaluated in clinical ethics consultation. This article seeks to fill this gap by exploring the efficacy of using a checklist in ethics consultation, as tested by an empirical investigation of the use of the checklist at a large academic medical system (Cleveland Clinic). The specific aims of this project are as follows: (1) to improve the quality of ethics consultations (...) by providing reminders to ethics consultants about process steps that are important for most patient-centered ethics consultations, (2) to create consistency in the ethics consultation process across the medical system, and (3) to establish an effective educational tool for trainers and trainees in clinical ethics consultation. The checklist was developed after a thorough literature review and an iterative process of revising and testing by a group of experienced ethics consultants. To pilot test the checklist, it was distributed to 46 ethics consultants. After a six-month pilot period in which ethics professionals used the checklist during their clinical activities, a survey was distributed to all of those who used the checklist. The 10-item survey examined consultants’ perceptions regarding the three aims listed above. Of the 25 survey respondents, 11 self-reported as experts in ethics consultation, nine perceived themselves to have mid-level expertise, and five self-reported as novices. The majority (68 percent) of all respondents, regardless of expertise, believed that the checklist could be a “helpful” or “very helpful” tool in the consultation process generally. Novices were more likely than experts to believe that the checklist would be useful in conducting consultations. The limitations of this study include: reduced generalizability given that this project was conducted at one medical system, utilized a small sample size, and used self-reported quality outcome measures. Despite these limitations, to the authors’ knowledge this is the first investigatation of the use of a checklist systematically to improve quality in ethics consultation. Importantly, our findings shed light on ways this checklist can be used to improve ethics consultation, including its use as an educational tool. The authors hope to test the checklist with consultants in other healthcare systems to explore its usefulness in different healthcare environments. (shrink)

This early work by Sydney Smith was originally published in 1854 and we are now republishing it with a brand new introductory biography. 'Elementary Sketches of Moral Philosophy' is a collection of lectures delivered by Smith at the Royal Institution in the years 1804, 1805, and 1806. Sydney Smith was born on 3rd June 1771 in Woodford, Essex, England. Smith's first book 'Six Sermons, preached in Charlotte Street Chapel, Edinburgh' was published in 1800. He (...) married Catharine Amelia Pybus in the same year and the couple settled in Edinburgh. While there, he helped set up the 'Edinburgh Review' and became its first editor in 1802. He continued to write articles for the review for the next quarter of the century which were a key element to the publication's success. (shrink)

In regard to the latter, Alexander was impressed by Adam Smith's treatment of the Moral Sentiments and especially with what he considered his sure insight in seeking for the ground of obligation in the causes of conduct, rather than in its effects. These causes were the passions. In this he was in sympathy with his (...) contemporary Hobhouse, who asserted with conviction that action rested on impulse feeling, and that it was useless to look for anything whether it was called Practical Reason or anything else, that stood outside the body of impulse feeling and controlled it. I think it was the considered opinion of Alexander, as it was that of Hobhouse, that if Psychology had anything to teach us bearing upon Morals, it was that the springs of human conduct must be looked for in our inherited tendencies with their associated emotions. Fear, sorrow, joy, repulsion, curiosity, pugnacity, self-assertion, the sex instinct, and acquisitiveness and constructiveness, are present, not only in men of all living races, but also in most of the higher animals. These specific tendencies and emotions, with certain complex passions such as admiration, reverence, scorn and the rest; together with one or two general tendencies such as sympathy and imitativeness, arising out of the nature of mental processes, must have played a leading role in the determination of human conduct, both in its lower and higher grades. First, he thinks his representationalist position is consis- tent with the possibility of spectrum inversion, and second, he thinks there are qualia. Thus, we can think of his position in the qualia debate as moderate representationalism (or, equally, moderate qualia realism) by taking up some middle ground be- tween these two (...) major camps. This \moderate" view faces several problems. Here I will very briey explain Shoemaker's represen- tationalist account of spectrum inversion in which he appeals to the existence of a certain sort of subjective property of objects, namely, what he calls appearance properties (formerly he called these phenomenal properties). I will argue that an alternative ver- sion of representationalism provides a more plausible explanation of both inversion-type scenarios and Shoemaker's color constancy case, which he uses to motivate the existence of these subjective properties, without positing appearance properties at all. Qualia realists hold that experience’s phenomenal character is a non-representational property of experience, what they call qualia. Representationalists hold that phenomenal character is a representational property of experience—there are no qualia (in this particular sense of the word). The transparency of qualia to introspection would seem to count as reason for rejecting qualia realism and favoring representationalism. Sydney Shoemaker defends a middle ground, call it moderate qualia realism, which seems to provide a response to the problem of transparency that (...) in consistent with qualia realism. According to this view, while phenomenal character is a representational property of experience, it is determined by certain non-representational properties of experience, namely qualia. Shoemaker explains the apparent transparency of qualia by claiming that, while qualia are not directly introspectable, they are indirectly introspectable. I argue that neither Shoemaker’s moderate qualia realism nor his account of indirect introspection provide the qualia realist with a plausible solution to the problem of transparency. (shrink)

Excerpt from Studies in Philosophical Criticism and Construction In the following pages my aim is to illustrate the principles of philosophic method by endeavouring critically to establish certain fundamental principles or Grundbegriffe in the spheres of Psychology, Logic and Epistemology, Ethics and Metaphysics; in other words, to lay the foundation for a more complete structure in each of these three branches of Philosophy.
