Equity in health and health care is animportant issue. It has been proposed that thepursuit of equity in health care is beinghampered by the dominance of individualism inhealth care practices. This paper explores theway in which communitarian ideals and practicesmight lend themselves to the pursuit of equity.Communitarians acknowledge, respect and fosterthe bonds that unite and identify communities.The paper argues that, to achieve equity inhealth care, these bonds need to be recognisedand harnessed rather than ignored. The notionof individual autonomy in the (...) context of thecommunity is examined. Alternative concepts ofautonomy – social autonomy and communityautonomy – are seen to be more respectful andnurturing of both the individual and thecommunity. Moreover, these concepts appeardesirable for the pursuit of health care equitygoals. The paper concludes with some thoughtsabout how equity in Australia's health caresystem can reasonably progress within acommunitarian vision. This is an assumption which most health economists at least implicitly appear to endorse. This paper questions that assumption. There are various outcomes beyond health and there are various processes involved in health care about which patients are not indifferent. This paper calls for a fuller investigation as (...) to what it is that patients want from their health services and the adoption of a more pluralistic conception of health care benefits. It is further argued that the objectives of health care systems are those in which citizensqua citizens also have interests and which may be different from those of patients. It is yet less likely that citizens’ interests in health care will be restricted to health. (shrink)

This study is part of a programme to elicit and examine community preferences for health care in different contexts. Data were obtained from a group of predominantly Australian health care decision-makers. A short questionnaire contained six valuation questions and four demographic questions. The six valuation questions posed choices where equal health gains were to be allocated to different population groups based upon: age; sex; current health; socio-economic status; across time; and across different numbers of individuals. The results provide some evidence (...) that respondents were prepared to discriminate between health gains derived in different contexts especially where health gains were to be allocated between groups of different health status and over time. Further research is planned and the possible implications for health policy, and in particular for resource allocation in health care, are briefly discussed. (shrink)

The justification for attempting to answer such questions lies in the de- sirability of injecting increased explicitness and rationality into decision-making in those areas of the public sector which are con- cerned with life saving. Given that resources are already being de- ployed to such (...) activities as crash-barriers on motorways, helicopters for air-sea rescue, kidney machines and other life-saving measures - although such activities result only in a reduction in risk of death, not itsillimination, in the policy fields affected - this means that already at the present time, at least by implication, values are be- ing placed by decision-makers on the saving of life. This paper introduces this mini-series on vertical equity in health care. It reflects on the fact that by and large equity policies in health care have failed and that there is a need for positive discriminationto promote equity better in future. This positive discrimination is examined under the heading of`vertical equity'. The paper considers Varian's notion of 'envy' as a basis for equity in health care but concludes that this is not a helpful route to go down. Better it would (...) seem to pursue the idea from Sen of 'freedoms' and 'communitarian claims' (as raised previously by this author). While it is argued that procedural justice is to be preferred in the longer run as a basis for equity there are gains in adopting a short run goal of promoting distributive justice. This paper argues that there is a need to move yet further than has already been suggested by some from the individual to the collective as a base for public health. A communitarian approach is one way to achieve this. This has the advantage of allowing not only the community’s voice to have a say in setting the values for public health but also more formally the development of a constitution on which public health might then be built. The problem of change recurs across Frantz Fanon's writings. As a philosopher, psychiatrist, and revolutionary, Fanon was deeply committed to theorizing and instigating change in all of its facets. Change is the thread that ties together his critical dialogue with Hegel, Marx, Freud, and Nietzsche and his intellectual exchange with Césaire, Kojève, and Sartre. It informs his analysis of racism and colonialism, négritude and the veil, language and culture, disalienation and decolonization, and it underpins his reflections on Martinique, Algeria, the (...) Caribbean, Africa, the Third World, and the world at large. Gavin Arnall traces an internal division throughout Fanon's work between two distinct modes of thinking about change. He contends that there are two Fanons: a dominant Fanon who conceives of change as a dialectical process of becoming and a subterranean Fanon who experiments with an even more explosive underground theory of transformation. Contemporary metaphysics is dominated by the view that every object belongs to a kind permanently in the sense that it cannot cease to belong to that kind without thereby ceasing to exist. For example, some philosophers think that a person is destroyed if they cease to be a person, a statue is destroyed if it ceases to be a statue, and so on. I believe that this standard view is false. Being a person, or a statue, or etc., is like (...) being a child: just as I did not cease to exist when I ceased to be a child, so people and statues need not cease to exist when they cease to be people and statues. Borrowing a term from Daniel Korman (who uses it in a similar but perhaps not identical way), I call this view phasalism because it entails that the kind-properties which ordinary objects instantiate are phase sortal properties, i.e., kind-properties that an object can instantiate for a temporary phase of its career. This dissertation is a partial defense of phasalism. I develop a phasalist metaphysics in detail, highlighting its virtues and rebutting objections along the way. After some stage-setting (Chapter 1), I defend a phasalist criterion of identity over time for ordinary objects (Chapter 2), as well as a phasalist account of the role that sortal properties play in the identity over time of ordinary objects (Chapter 3). Then I defend a phasalist-friendly approach to the identity over time of lumps, hunks, pieces, etc. of matter (Chapter 4). The material in these chapters amounts to a phasalist solution to certain material coincidence puzzles, such as the puzzle of the statue and the piece of clay. I go on to show that familiar puzzles about undetached parts (Chapter 5) and fission (Chapter 6) can be solved within the confines of my phasalist metaphysics as well. (shrink)

The objective of this study was to assess whether, and how, the attitudes towards business ethics of former South African business students have changed between the early 1990s and 2010. The study used the Attitudes Toward Business Ethics Questionnaire and applied a comparative analysis between leading business schools in South Africa. The findings of this study found a significant change in attitudes based on a set time frame, with a trend towards stronger opinions on business ethics and espoused values. Eleven (...) factors came out as fundamental, although they were less able to explain the variation in the attitudes than the previous study. A significant change in the rankings of variables was noted and indicated a shift in attitude toward a teleological moral philosophy as well as utilitarian motives. This shows a clear trend towards compliance-based ethics, which can be explained by the proliferation of business legislation and regulation in the wake of recent corporate governance failures and the subsequent global financial crisis. The Western philosophic tradition has tended to tie the question of action to that of freedom, with the relationship structured around the free will/determinism opposition. In contrast, I show that in Being and Nothingness, Sartre offers a stringent and radical critique of these approaches. I briefly outline the conceptual parameters of Sartre’s early ontology, before showing that he rejects the free will tradition because of its underlying conception of freedom and insistence that action is reflective and will-based. According to Sartre, (...) consciousness is not a sum of parts, with one aspect (will) guiding the rest. Consciousness is a differentiated whole, divided between reflective and pre-reflective levels. Will is tied to the reflective level of consciousness and so cannot be said to be foundational given that reflectivity depends upon pre-reflectivity. Instead, it is an expressive effect of consciousness’s spontaneous, pre-reflective, projection of itself towards a particular end and value. (shrink)

Big Data has been variously defined in the literature. In the main, definitions suggest that Big Data possess a suite of key traits: volume, velocity and variety, but also exhaustivity, resolution, indexicality, relationality, extensionality and scalability. However, these definitions lack ontological clarity, with the term acting as an amorphous, catch-all label for a wide selection of data. In this paper, we consider the question ‘what makes Big Data, Big Data?’, applying Kitchin’s taxonomy of seven Big Data traits to 26 datasets (...) drawn from seven domains, each of which is considered in the literature to constitute Big Data. The results demonstrate that only a handful of datasets possess all seven traits, and some do not possess either volume and/or variety. Instead, there are multiple forms of Big Data. Our analysis reveals that the key definitional boundary markers are the traits of velocity and exhaustivity. We contend that Big Data as an analytical category needs to be unpacked, with the genus of Big Data further delineated and its various species identified. It is only through such ontological work that we will gain conceptual clarity about what constitutes Big Data, formulate how best to make sense of it, and identify how it might be best used to make sense of the world. Carl Jung has always lain at the edge of sociology's consciousness, despite the existence of a long-established Freudian tradition. Yet, over the years, a small number of sociological writers have considered Jung; one or two Jungian writers have considered sociology. The range of perspectives is quite wide: Durkheim, Weber, Marx, Levi-Strauss, feminism, mass society, postmodernism. These scattered writings, however, have had little cumulative impact and inspired little debate. The authors seem often not to have known of each other, while the (...) sociological mainstream has remained unmoved or unaware. This is the situation that this book seeks to change. Jung and Sociological Theory brings together a selection of articles and excerpts in a single volume, together with some writings from anthropology, and seeks to begin the task of critical evaluation. There has always been a strong connection historically between aesthetics and radical politics, and this is no less true for the global justice movements current preoccupation with cultural approaches to political action. The essays collected here seek to engage with past and present convergences between the theories and practices of artists and writers and the theories and practices of movements for radical social change. There is already a massive amount of literature on Marxist approaches to aesthetics, art and literature, and (...) whilst recognising the usefulness of such approaches, the essays collected here attempt to engage with culture from other radical critical positions - whether they be anarchist, autonomist, ecological or otherwise. Such perspectives have often been overlooked historically, but it is arguable that they now more centrally influence the activities of radical artists and activists. As such, the perspectives of these essays, which are often drawn from or inspired by the practices of the current global justice movement, exhibit an exhilarating political and generational break with the suppositions of earlier radical theoretical approaches to cultural critique.
