The purpose of this contemporary history study is to analyse nursing strategy documents produced by NHS Trusts in England in the period 2009–2013, through a process of discourse analysis. In 2013 the Francis Report on the Mid-Staffordshire NHS Foundation Trust was published. The Report highlighted the full range of organisational failures in a Trust that valued financial efficiency over patient care. The analysis that followed, however, dwelt heavily on the failings of the nurses. Nursing strategy documents at that time served (...) to set the future direction for NHS Trusts, prescribing specific value frameworks for each nursing workforce. However, the values chosen frequently conflicted with each other pitting nursing values against a managerial trope. It is argued that documents provided a response to wider NHS concerns and high-profile failures in care, particularly the Francis Report, paying lip service to staff engagement whilst maintaining a corporate focus. Nursing values were placed firmly within a managerialist discourse, one that has needed to be re-evaluated in the current Covid-19 pandemic. Wider implications of the research suggest discussion of value conflict may be beneficial within nursing education and a truly local approach to strategy creation would potentially promote staff buy-in to strategy documents. Current health policy, high‐profile failures and increased media scrutiny have led to a significant focus on patient experience in Britain's National Health Service (NHS). Patient experience data is typically gathered through surveys of satisfaction. The study aimed to support a better understanding of the patient experience and patients' expression of it through consideration of the aspects of the patient experience on NHS wards which are by their nature impossible to capture through patient satisfaction surveys. Existential phenomenology was used to develop (...) an in‐depth exploratory narrative, expressed through the voices of the participants. Data collection involved in‐depth face‐to‐face interviews with 12 purposively sampled participants, with analysis by means of hermeneutics. Though the individuality of each experience was apparent and cannot be overemphasised, common factors emerging from the data included uncertainty and unexpectedness, suffering and finitude, the futility of feedback and bureaucracy and absurdity. Overall, participants demonstrated how their individual personalities and expectations affected their response both to illness or injury and to their hospital admissions, highlighting feelings of vulnerability and voicelessness as a response to hospitalisation. The findings of this study provide useful insight into the patient experience on British hospital wards, and the value of an existential–phenomenological approach is demonstrated. (shrink)

Our defence against microbes depends largely on the ability of neutrophils to migrate from the blood stream to sites of infection. Although the ability of animal cells to move may be primitive, and also fundamental for a number of phenomena in biology, the cellular mechanism by which neutrophils are able to move rapidly towards the infection remains an enigma. Even though the structures of the receptors involved have been sequenced and many of the molecules involved in neutrophil adherence and traction (...) identified, the essential mechanisms that control and regulate the neutrophil motor remain obscure. Here, an outline of the fundamental inadequacies in our current understanding is given, along with some recent developments that promise to produce some significant advances. (shrink)

He makes a strong case that when this parity claim is appropriately restricted to a single, sound other-minds belief, belief in God and belief in other minds do prove epistemically comparable. The subject on which I am to address you this evening is one which, though it is of fundamental importance both for philosophy and for practice, cannot but present the gravest difficulties for such treatment as falls within the limits of this occasion. Philosophical problems are always difficult, but those of ultimate metaphysics are in this respect egregious. For the simplifications that are open to the scientific phenomenologist who can rest content with a spatiotemporal world, or to the analyst who (...) concentrates on the objective content with which the human understanding most conspicuously concerns itself, are not open to the metaphysician. He must meet the full complexity of things: not limiting himself to their appearances as objects within human experience, nor to a reflective analysis of their objective content, but taking them as they must be in themselves in order that they may appear as they empirically and reflectively do appear for the special cognitive faculties of man. I ask, therefore, for your patience and attention as I try first of all to lead you into, and then, as I hope, out of, the metaphysical labyrinth. For what I have to describe is something of a metaphysical adventure, or, if you like, a metaphysical ghoststory, in which the ghosts assume such reality as to make the solid things “give up the ghost”—which is just as it should be in a good ghost-story! Prima facie knowledge is an ineradicable monster. Conceived as a relation of a mind to objects extrinsic to it, it is chimerical: for knowledge, as such, is of the real , i.e. of things as they are in se ; but the prima facie form of knowledge precludes this. Its object is a thing apprehended ab extra , i.e. as referred to a subject to which it is extrinsic. To seek to escape this impasse either by making the object intrinsic (...) to the subject, or the subject a function of the object, is to reject the prima facie character of knowledge as a relation of compresent terms. The incoherence of prima facie knowledge is that its object must be both extrinsic and intrinsic to mind—extrinsic as independently real, intrinsic as known—yet can be neither : not extrinsic since thus its inseitas is occulted, not intrinsic since thus truth is mere appearance. And this chimaera becomes a monster because knowledge is also ineradicable. We cannot know that knowledge is impossible; and, though we may be in doubt about its extancy, that very doubt is epistemic in form. It is our acceptance of the demands of knowledge that lies at the basis of our doubt; and this applies not only to legitimate doubt about the extancy of knowledge, but also to chimerical doubt about its possibility. Doubt is an inchoation of knowledge, which is thus ineradicable. (shrink)

This paper treats some of the issues raised by Putnam's discussion of, and claims for, quantum logic, specifically: that its proposal is a response to experimental difficulties; that it is a reasonable replacement for classical logic because its connectives retain their classical meanings, and because it can be derived as a logic of tests. We argue that the first claim is wrong (1), and that while conjunction and disjunction can be considered to retain their classical meanings, negation crucially does not. (...) The argument is conducted via a thorough analysis of how the meet, join and complementation operations are defined in the relevant logical structures, respectively Boolean- and ortholattices (3). Since Putnam wishes to reinstate a realist interpretation of quantum mechanics, we ask how quantum logic can be a logic of realism. We show that it certainly cannot be a logic of bivalence realism (i.e., of truth and falsity), although it is consistent with some form of ontological realism (4). Finally, we show that while a reasonable explication of the idealized notion of test yields interesting mathematical structure, it by no means yields the rich ortholattice structure which Putnam (following Finkelstein) seeks. This historical study aims to refine understanding of the nature of nursing work. The study focuses on the 1880 crisis at Guy's Hospital in London to examine the nature and meaning of nursing work, particularly the concept of nursing work as many ‘little things.’ In this paper, an examination of Margaret Lonsdale's writing offers an original contribution to our understanding of the ways in which nursing work differs from medical practice. In this way, we use the late-nineteenth-century controversy at Guy's (...) Hospital as a prism through which to examine the contested nature of nursing work. Lonsdale's ideas are corroborated by examination of writings by nurse leaders Florence Nightingale and Eva Luckes. Luckes, in particular, elaborated what was meant by nursing as the performance of a thousand little things, which are specific to nursing work. While physicians had been performing much of what was considered to be nursing work, nurses developed some of these and other interventions into a unique body of work characterized by meticulous attention to significant details. Some implications regarding current nursing practice are discussed. (shrink)

A long-standing theme in discussion of perception and thought has been that our primary cognitive contact with individual objects and events in the world derives from our perceptual contact with them. When I look at a duck in front of me, I am not merely presented with the fact that there is at least one duck in the area, rather I seem to be presented withthisthing (as one might put it from my perspective) in front of me, which looks to (...) me to be a duck. Furthermore, such a perception would seem to put me in a position not merely to make the existential judgment that there is some duck or other present, but rather to make a singular, demonstrative judgment, that that is a duck. My grounds for an existential judgment in this case derives from my apprehension of the demonstrative thought and not vice versa. (shrink)

Father Hallett also cites and quotes secondary sources, and he includes an appendix relating Wittgenstein to more than 150 authors, particularly those of his own generation or earlier whom he read, or knew personally, and who are mentioned in this commentary. Written in straightforward and lucid prose, this outstanding book reveals continuities in Wittgenstein's thought over long periods of time. This book defends antitheodicism, arguing that theodicies, seeking to excuse God for evil and suffering in the world, fail to ethically acknowledge the victims of suffering. The authors argue for this view using literary and philosophical resources, commencing with Immanuel Kant's 1791 "Theodicy Essay" and its reading of the Book of Job. Three important twentieth century antitheodicist positions are explored, including "Jewish" post-Holocaust ethical antitheodicism, Wittgensteinian antitheodicism exemplified by D.Z. Phillips and pragmatist antitheodicism defended by William James. The authors argue (...) that these approaches to evil and suffering are fundamentally Kantian. Literary works such as Franz Kafka'sThe Trial, Samuel Beckett's Waiting for Godot, and George Orwell's Nineteen Eighty-Four, are examined in order to crucially advance the philosophical case for antitheodicism.
