Abstract25.6 Million people in the United States have Limited English Proficiency (LEP), defined as insufficient ability to read, write, or understand English. We will (1) Delineate the merits of approaching language as a social determinant of health, (2) highlight pertinent public health values and guidelines which are most relevant to the plight of populations with LEP and (3) Use the COVID-19 pandemic as an example of how a breakdown in public health ethics values created harm for populations and patients with (...) LEP. We define a framework to tease out public health responsibilities given some populations’ limited proficiency in a society’s predominant language. The American Public Health Association (APHA) public health ethics core values serve as a framework to interrogate current practices. We use the COVID-19 case to illustrate gaps between health policy and healthcare disparities experienced by populations with LEP. (shrink)

MacKay’s Public Health Virtue Ethics offers a distinctive approach to public health ethics, with social structures at the forefront. MacKay’s helpful overview of the recent literature considers three distinct referents for ascribing virtues in public health ethics: (i) individuals, such as public health practitioners, (ii) social structures, such as public health institutions and policies and (iii) the communities affected by public health policy. While MacKay is interested in virtuous structures, I am interested in the structure of virtue as a precursor (...) to this approach. In this commentary, I seek to unpack the structure of virtue itself, to delineate what various accounts of public health virtues offer, including MacKay’s new account. For such clarity, I turn to David Pears’ neo-Aristotelian essay on moral courage, in which he distinguishes external goals, internal goals and countergoals. Additional virtue vocabulary advances discussion of how the moral psychology of virtue traditions can be best adapted to public health professions, policy and practice. (shrink)

A dominant cultural narrative within Costa Rica describes Costa Ricans not only as different from their Central American neighbours, but it also exalts them as better: specifically, as more white, peaceful, egalitarian and democratic. This notion of Costa Rican exceptionalism played a key role in the creation of their health care system, which is based on the four core principles of equity, universality, solidarity and obligation. While the political justification and design of the current health care system does, in part, (...) realize this ideal, we argue that the narrative of Costa Rican exceptionalism prevents the full actualization of these principles by marginalizing and excluding disadvantaged groups, especially indigenous and black citizens and the substantial Nicaraguan minority. We offer three suggestions to mitigate the self-undermining effects of the dominant national narrative: 1) encouragement and development of counternarratives; 2) support of an emerging field of Costa Rican bioethics; and 3) decoupling health and national successes. Though antiretroviral therapy is the standard of care for people living with HIV, its treatment limitations, burdens, stigma and costs lead to continued interest in HIV cure research. Early-phase cure trials, particularly those that include analytic treatment interruption, involve uncertain and potentially high risk, with minimal chance of clinical benefit. Some question whether such trials should be offered, given the risk/benefit imbalance, and whether those who choose to participate are acting rationally. We address these questions through a longitudinal decision-making study (...) nested in a Thai acute HIV research cohort. In-depth interviews revealed central themes about decisions to join. Participants felt they possessed an important identity as members of the acute cohort, viewing their bodies as uniquely suited to both testing and potentially benefiting from HIV cure approaches. While acknowledging risks of ATI, most perceived they were given an opportunity to interrupt treatment, to test their own bodies and increase normalcy in a safe, highly monitored circumstance. They were motivated by potential benefits to themselves, the investigators and larger acute cohort, and others with HIV. They believed their own trial experiences and being able to give back to the community were sufficient to offset participation risks. These decisions were driven by the specific circumstances experienced by our participants. Judging risk/benefit ratios without appreciating these lived experiences can lead to false determinations of irrational decision- making. While this does not minimise vital oversight considerations about risk reduction and protection from harm, it argues for inclusion of a more participant-centered approach. (shrink)

Recently much has been made of the grounding relation, and of the idea that it is intimately tied to fundamentality. If A grounds B, then A is more fundamental than B (though not vice versa ), and A is ungrounded if and only if it is fundamental full stop—absolutely fundamental. But here is a puzzle: is grounding itself absolutely fundamental?

There is a long-standing debate about whether poetry can make a substantive contribution to philosophy with compelling arguments to show that poetry and philosophy involve distinct modes of thought and aims, albeit with similar concerns. This paper argues that reading lyric poetry can play a substantive role in philosophy by helping the philosopher understand how to forge connections with the perspectives of others. The paper takes the view that poetry is not directly philosophical but can play an important role in (...) developing the philosopher's awareness of how perspective shapes beliefs and thought processes. Following Robert Nozick, David Lewis, and Helen Beebee, it argues for the need to broaden our conception of the aims of philosophy beyond aiming for conclusive proof, at the least where philosophical inquiry is focused on questions of value. It then shows how poetry cultivates an intellectual virtue essential to philosophy aimed at uncovering equilibria and collective understanding. (shrink)

A renowned scholar investigates the "human crisis" that Albert Camus confronted in his world and in ours, producing a brilliant study of Camus's life and influence for those readers who, in Camus's words, "cannot live without dialogue and friendship. As France--and all of the world--was emerging from the depths of World War II, Camus summed up what he saw as 'the human crisis'. 'We gasp for air among people who believe they are absolutely right, whether it be in their machines (...) or their ideas. And for all who cannot live without dialogue and the friendship of other human beings, this silence is the end of the world.' In the years after he wrote these words, until his death fourteen years later, Camus labored to address this crisis, arguing for dialogue, understanding, clarity, and truth. When he sailed to New York, in March 1946--for his first and only visit to the United States--he found an ebullient nation celebrating victory. Camus warned against the common postwar complacency that took false comfort in the fact that Hitler was dead and the Third Reich had fallen. Yes, the serpentine beast was dead, but "we know perfectly well," he argued, "that the venom is not gone, that each of us carries it in our own hearts." All around him in the postwar world, Camus saw disheartening evidence of a global community revealing a heightened indifference to a number of societal ills. This paper discusses Jean-Paul Sartre's conceptions of pure reflection and impure reflection, affirming the distinction but arguing for an intermediate mode of reflection: creolized reflection. Creolized reflection is not impure as it does not regard consciousness as being-in-itself, but it transcends pure reflection in concretely negating the relationship between consciousness and imposed conceptions of being-in-itself. I argue that this mode of reflection is at play in much Africana phenomenology and conceptions of potentiated double consciousness.

Armies know all about killing. It is what they do, and ours does it more effectively than most. We are painfully coming to realize, however, that we are also especially good at killing our own ''from the inside out, '' silently, invisibly. In every major war since Korea, more of our veterans have taken their lives than have lost them in combat. The latest research, rooted in veteran testimony, reveals that the most severe and intractable PTSD -- fraught with shame, (...) despair, and suicide -- stems from ''moral injury.'' But how can there be rampant moral injury in what our military, our government, our churches, and most everyone else call just wars? At the root of our incomprehension lies just war theory -- developed, expanded, and updated across the centuries to accommodate the evolution of warfare, its weaponry, its scale, and its victims. Any serious critique of war, as well any true attempt to understand the profound, invisible wounds it inflicts, will be undermined from the outset by the unthinking and all-but-universal acceptance of just war doctrine. Killing from the Inside Out radically questions that theory, examines its legacy, and challenges us to look beyond it, beyond just war. (shrink)

In seeking an answer to the question of what it means for a theory to be fundamental, it is enlightening to ask why the current best theories of physics are not generally believed to be fundamental. This reveals a set of conditions that a theory of physics must satisfy in order to be considered fundamental. Physics aspires to describe ever deeper levels of reality, which may be without end. Ultimately, at any stage we may not be able to tell whether (...) we've reached rock bottom, or even if there is a base level – nevertheless, I draft a checklist to help us identify when to stop digging, in the case where we may have reached a candidate for a final theory. Given that the list is – according to (current) mainstream belief in high-energy physics – complete, and each criterion well-motivated, I argue that a physical theory that satisfies all the criteria can be assumed to be fundamental in the absence of evidence to the contrary (i.e., I argue that the necessary conditions are jointly sufficient for a claim of fundamentality in physics). The presented paper addresses the concept of technoscience and its possible implications for technology assessment. Drawing on the discourse about converging technologies, it formulates the assumption that a general shift within science from epistemic cultures to techno-epistemic cultures lies at the heart of the propagated convergence between nano-, bio-, info- and cogno-sciences and technologies. This shift is adequately captured—so the main thesis—by the technoscience label. The paper elaborates on the shared characteristics of the new technosciences, especially their hybrid character and (...) their non-linear integration of basic research, object construction and technological innovation. Recent calls for new modes of technology governance (e.g. for upstream technology assessment and public participation) are discussed against the background of these technoscience characteristics. It is concluded that an adequate model of emerging technosciences and a detailed understanding of their characteristics is of paramount importance for current practices of technology assessment. Several approaches to developing a theory of quantum gravity suggest that spacetime—as described by general relativity—is not fundamental. Instead, spacetime is supposed to be explained by reference to the relations between more fundamental entities, analogous to `atoms' of spacetime, which themselves are not (fully) spatiotemporal. Such a case may be understood as emergence of \textit{content}: a `hierarchical' case of emergence, where spacetime emerges at a `higher', or less-fundamental, level than its `lower-level' non-spatiotempral basis. But quantum gravity cosmology also presents us (...) with the possibility of emergence of \textit{context}: where spacetime emerges from some `prior' non-spatiotemporal state (replacing the Big Bang), due to particular conditions in the early universe. I present a general conception of emergence which is plausibly able to accommodate both pictures. This is a positive conception that does not rely on a failure of reduction or explanation in any sense (indeed, reduction is a necessary feature of quantum gravity, and is useful in understanding emergence in this case). I also consider the possibility that the distinction between content- and context- based explanations is blurred, or usefully `collapsed', in the case of spacetime emergence. (shrink)

Performance is increasingly important to the poet, which is evidenced by the growing numbers of videos and audio recordings online including YouTube, the National Poetry library, and Poetry Archive. As a result, there are greater opportunities to engage with poets reading their own work and consequently, there is a need to move away from thinking of poetry as primary something that takes shape on the page. Furthermore, by refocusing attention to poetry as an oral artform, in particular to poetry performance, (...) different ways of understanding the value of poetry come to the fore. The live poetry performance, I argue, offers an attempt to reclaim the personal individual expression of language in contemporary society and demands attention by the audience because of the role of voice and body in the performance. Through awareness of ‘uniqueness of voice,’ the performed poem becomes a site of collective meaning making with individual identity intact as a shared endeavour between performer and audience, and necessarily resists appropriation (in the sense that the words experienced cannot be divorced from the performer or the performance space to serve as a proxy for meaning elsewhere). This chapter therefore makes an argument for the need to engage with spoken word and performance poetry in the live performance space. (shrink)

This paper is about a category of social conventions that, I will argue, have significant moral implications. The category consists in our conventions about what we notice and choose not to notice about persons, features of persons, and their circumstances. We normally do not think much about what we notice about others, and what they notice about us, but I will argue that we should. Noticing people is a way of engaging with them in social contexts. We can engage in (...) social noticing more or less respectfully, more or less benevolently. Moreover, our standard conventions about noticing often have disparate effects on different groups of people. To be noticed appropriately is to have one's moral and social standing affirmed; conversely, to be denied notice or to be noticed inappropriately is very often to be denigrated or objectified. My aim in this paper is to unpack these moral dimensions of conventions of noticing and discuss their implications for how people engage with each other in certain kinds of social exchanges involving persons with disabilities. In Part I, I explain what I mean by the conventions of noticing and how they operate in social interactions. I show that although we do not always attend to them, these conventions are thoroughly embedded in our everyday social life. In Part II, I argue that these conventions have important moral dimensions and as such, should be governed by moral principles and values. I employ a Kantian framework of duties of love and respect in order to show how moral concerns should shape the way we use conventions of noticing and respond to their use by others. In Part III, I draw out the implications of this picture for social interactions among strangers when one or more of the parties involved has an immediately visible disability. (shrink)

The French studies scholar Patrick Coleman made the important observation that over the course of the eighteenth century, the social meanings of anger became increasingly democratized. The work of Jean-Jacques Rousseau is an outstanding example of this change. In Man or Citizen, Karen Pagani expands, in original and fascinating ways, the study of anger in Rousseau’s autobiographical, literary, and philosophical works. Pagani is especially interested in how and to what degree anger—and various reconciliatory responses to anger, such as forgiveness—functions (...) as a defining aspect of one’s identity, both as a private individual and as a public citizen. Rousseau himself was, as Pagani puts it, “unabashed” in his own anger and indignation—toward society on one hand (corrupter of our naturally good and authentic selves) and, on the other, toward certain individuals who had somehow wronged him (his famous philosophical disputes with Voltaire and Diderot, for example). In Rousseau’s work, Pagani finds that the extent to which an individual processes, expresses, and eventually resolves or satisfies anger is very much of moral and political concern. She argues that for Rousseau, anger is not only inevitable but also indispensable, and that the incapacity to experience it renders one amoral, while the ability to experience it is a key element of good citizenship. This paper surveys recent work on moral expertise. Much of that work defends an asymmetry thesis according to which the cognitive deference to expertise that characterizes other areas of inquiry is out of place in morality. There are two reasons why you might think asymmetry holds. The problem might lie in the existence of expertise or in deferring to it. We argue that both types of arguments for asymmetry fail. They appear to be stronger than they are because of their (...) focus on moral expertise regarding all-in judgments about rightness. We reject this emphasis on all-in judgment in favor of an account of moral expertise as typically multi-stranded and domain limited. This account of moral expertise is better able to address the problem of how to identify those who have expertise. It also offers a more nuanced picture of the contrast between accepting a moral claim on one's own and accepting it on testimony. (shrink)

Justified trust is rationally permitted trust; wise trust is excellent trust. Excellent (dis)trust is always justified (dis)trust, but the reverse is not true. You can be justified in distrusting someone and yet it be wise for you to trust. Contrary to folk saying, wisdom does not favour distrust ahead of trust. This paper explores what it takes to be wise in entering, maintaining, modifying and exiting trust relations. The view that narrative artworks can offer insights into our lives, in particular, into the nature of the emotions, has gained increasing popularity in recent years. However, talk of narrative often involves reference to a perspective or point of view, which indicates a more fundamental mechanism at work. In this article, I argue that our understanding of the emotions is incomplete without adequate attention to the perspectival structures in which they are embedded. Drawing on Bennett Helm’s theory of emotion, I (...) argue that the narrative view fails to take into account the influence of perspective on the emotions. In order to address this gap in our understanding of the emotions, I highlight a mode of engagement with literature that prioritizes the perspectival features of a work. Focusing on lyric poetry, I argue that non-narrative artworks are those best placed to highlight this fundamental aspect of our emotional experiences. (shrink)

Erzählung dient hier nicht allein als Inbegriff einer Gesamtheit von Sinnbildungen, sondern vor allem zur Kennzeichnung des wirklich Menschlichen. Das narrative Selbst gibt nicht nur auf die Frage nach dem Menschen eine Antwort, sondern ist auch grundlegend für die Beantwortung der Frage nach dem Tunsollen des Menschen. (...) Und es wird sichtbar, dass die "Form der Erzählung" die Art und Weise des Verstehens der eigenen Handlungen und die der anderen sein kann. Die Klärung der Bedeutung von Narrativität im Spannungsfeld von Ethik und deren lebenspraktischem Bezug steht in systematischer Hinsicht allerdings noch aus. Der vorliegende Band leistet einen wichtigen Beitrag zur Klärung dieses Defizits, indem er Grundlagen, Grundpositionen und Anwendungen einer narrativen Ethik vorstellt. (shrink)

This study has two agendas: to interrogate popular notions of religion by reading it, out of Derrida and Certeau, as a signifier for a situated historical scene; and to show the existential philosophy of Beauvoir as a performance of that scene. In particular, it shows how the structure of relationships she presents in her ethics clearly reproduces the rhythms of the scene of religion. One of the implications of this reproduction is that existential philosophy can only emerge in the context (...) of religion, and is necessarily an iteration of religion. The other implication is that we might reassess how we code the category ‘religion’ in our public and private discourse, with all the disruption that such a different coding might entail. (shrink)

Controversies surrounding social media platforms have provided opportunities for institutional reflexivity amongst users and regulators on how to understand and govern platforms. Amidst contestation, platform companies have continued to enact projects that draw upon existing modes of privatized governance. We investigate how social media companies have attempted to achieve closure by continuing to set the terms around platform governance. We investigate two projects implemented by Facebook (Meta)—authenticity regulation and privacy controls—in response to the Russian Interference and Cambridge Analytica controversies surrounding (...) the 2016 U.S. Presidential Election. Drawing on Goffman’s metaphor of stage management, we analyze the techniques deployed by Facebook to reinforce a division between what is visible and invisible to the user experience. These platform governance projects propose to act upon _front-stage data relations:_ information that users can see from other users—whether that is content that users can see from “bad actors”, or information that other users can see about oneself. At the same time, these projects relegate _back-stage data relations_—information flows between users constituted by recommendation and targeted advertising systems—to invisibility and inaction. As such, Facebook renders the user experience actionable for governance, while foreclosing governance of back-stage data relations central to the economic value of the platform. As social media companies continue to perform platform governance projects following controversies, our paper invites reflection on the politics of these projects. By destabilizing the boundaries drawn by platform companies, we open space for continuous reflexivity on how platforms should be understood and governed. (shrink)
