BackgroundAssessment of capacity to consent to treatment is an important legal and ethical issue in daily medical practice. In this study we carefully evaluated the capacity to consent to treatment in patients admitted to an acute medical ward using an assessment by members of the medical team, the specific Silberfeld's score, the MMSE and an assessment by a senior psychiatrist.MethodsOver a 3 month period, 195 consecutive patients of an internal medicine ward in a university hospital were included and their capacity (...) to consent was evaluated within 72 hours of admission.ResultsAmong the 195 patients, 38 were incapable of consenting to treatment (unconscious patients or severe cognitive impairment) and 14 were considered as incapable of consenting by the psychiatrist (prevalence of incapacity to consent of 26.7%). Agreement between the psychiatrist's evaluation and the Silberfeld questionnaire was poor (sensitivity 35.7%, specificity 91.6%). Experienced clinicians showed a higher agreement (sensitivity 57.1%, specificity 96.5%). A decision shared by residents, chief residents and nurses was the best predictor for agreement with the psychiatric assessment (sensitivity 78.6%, specificity 94.3%).ConclusionPrevalence of incapacity to consent to treatment in patients admitted to an acute internal medicine ward is high. While the standardized Silberfeld questionnaire and the MMSE are not appropriate for the evaluation of the capacity to consent in this setting, an assessment by the multidisciplinary medical team concurs with the evaluation by a senior psychiatrist. This commentary came from within the framework of integrating the humanities in medicine and from accompanying research on disease-related issues by teams involving clinicians and researchers in medical humanities. The purpose is to reflect on the challenges faced by researchers when conducting emotionally laden research and on how they impact observations and subsequent research findings. This commentary is furthermore a call to action since it promotes the institutionalization of a supportive context for medical humanities researchers who have not been trained (...) to cope with sensitive medical topics in research. In addition, Karl Menger's "Mathematical Colloquium" with Kurt Gödel is presented as a parallel movement. The final chapter of this section describes the demise of the Vienna Circle and the forced exodus of scientists and intellectuals from Austria.The second part of the book includes a bio-bibliographical documentation of the Vienna Circle members and for the first time of the assassination of Moritz Schlick in 1936, followed by an appendix comprising an extensive list of sources and literature. Isis oder Encyklopädische Zeitung, edited by the German natural philosopher Lorenz Oken (1779‐1851), is supposed to be one of the most significant journals of natural sciences in the early 19th century. The following study, based on a quantitative analyses of all articles (1817‐1822), investigates the profile of the natural sciences in this journal. The results are interpreted according to Oken's concept of the journal as well as his editorial notes. It is shown that the encyclopedically broadly designed journal focuses on (...) the natural sciences as major part of it's content, presenting the descriptive history of natural sciences (Naturgeschichte) as main subject. The journal's research perspective follows the encyclopedical spread of issues and scientific subjects; a clear research profile of scientific disciplines can not be discerned. Although scientific subjects, issues and authors are heterogeneous, Oken preserves an inner coherence by a common perspective founded in natural philosophy: his strategy was therefore to interfere with the content of the articles by comments and footnotes. Finally it is shown that Oken's idea of encyclopedism and it's realisation in Isis comprises sciences as well as politics as two sides of the same coin: based on the philosophical understanding of the unity of nature Oken tries to educate the consciousness of his readers towards a socially and politically homogeneous nation. Eusociality is the form of animal social organization with a reproductive division of labor, most prominently known from ants and bees. Here I ask the question why this enormously successful form of social organization is missing in the largest and most diverse group of vertebrates, the teleost fishes. I first briefly review the phylogenetic distribution and likely evolutionary origins of eusociality. Then, after an equally very brief review of the diverse life history strategies of teleosts, I conclude that it is (...) not the lack of evolutionary pre-adaptations which is keeping teleosts from becoming eusocial. Rather, I argue, that the absence of eusocial fish is caused by a number of differences between aquatic (chiefly marine) and terrestrial ecosystems: (1) Greater offspring dispersal in aquatic ecosystems reduces the role of kin-selection. (2) Lesser predictability of the environment at larger timescales in marine ecosystems disfavors eusociality. (3) A briefer impact of resource pulses in aquatic ecosystems will cause less evolutionary pressure towards cooperation, and eventually eusociality. Finally, I conclude by predicting that the most likely places to find eusocial fishes will be the deep-water regions of the ocean and the African rift lakes. (shrink)

With the advent of powerful parallel computers, efforts have commenced to simulate complete mammalian brains. However, so far none of these efforts has produced outcomes close to explaining even the behavioral complexities of animals. In this article, we suggest four challenges that ground this shortcoming. First, we discuss the connection between hypothesis testing and simulations. Typically, efforts to simulate complete mammalian brains lack a clear hypothesis. Second, we treat complications related to a lack of parameter constraints for large-scale simulations. To (...) demonstrate the severity of this issue, we review work on two small-scale neural systems, the crustacean stomatogastric ganglion and the Caenorhabditis elegans nervous system. Both of these small nervous systems are very thoroughly, but not completely understood, mainly due to issues with variable and plastic parameters. Third, we discuss the hierarchical structure of neural systems as a principled obstacle to whole-brain simulations. Different organizational levels imply qualitative differences not only in structure, but in choice and appropriateness of investigative technique and perspective. The challenge of reconciling different levels also undergirds the challenge of simulating and hypothesis testing, as modeling a system is not the same thing as simulating it. Fourth, we point out that animal brains are information processing systems tailored very specifically for the ecological niches the respective animals live in. (shrink)

I propose that a dishonest signaling system can be evolutionarily stable in eusocial animal societies if the amount of dishonesty is balanced by the chance of non-reproductive workers to advance to the reproductive caste in the future. I express this trade-off in a modified form of Hamilton’s rule, where I distinguish between the real and perceived cost of an altruistic act, and between the real and perceived genetic relatedness between colony members. Furthermore, I elaborate how the vertebrate neuromodulator oxytocin could (...) serve as an internal representation of the perceived cost of an altruistic act and of perceived relatedness. Behavioral and receptor localization data support this hypothesis. The encoding of cost and relatedness by oxytocin is likely integrated with a number of other functions related to social bonding. I conclude with a discussion of honesty in signaling, an outline of testable consequences of this hypothesis, and a comparison between vertebrate and insect eusociality. (shrink)

“Supposing that truth is a women-what then?” This is the very first sentence in Nietzsche’s Beyond Good and Evil . Not very often are philosophers so disarmingly explicit in their intention to discomfort the reader. In fact, one might say that the natural state of Nietzsche’s reader is one of perplexity. Yet it is in the process of overcoming the perplexity that one realizes how rewarding to have one’s ideas challenged. In Beyond Good and Evil, Nietzsche critiques the mediocre in (...) modernity and challenges the reader to accept their state of becoming and accept improvisation and creativity of the process. Nietzsche’s book is carefully designed to disorient the reader, to systematically provoke and tease her to the point of stealing away her certainties. The Origin of the Family, Private Property and the State (1884), was a provocative and profoundly influential critique of the Victorian nuclear family. Engels argued that the traditional monogamous household was in fact a recent construct, closely bound up with capitalist societies. Under this patriarchal system, women were servants and, effectively, prostitutes. Only Communism would herald the dawn of communal living and a new sexual freedom and, in turn, the role of the state would become superfluous.

Obwohl formal als Roman aufgebaut, kommt dieses einzigartige philosophische Werk im dichterischen Gewand daher. Nietzsche selbst bezeichnete seinen Zarathustra als "das tiefste Buch, das die Menschheit besitzt.".
