We argue that contemporary psychiatry adopts a defensive strategy vis-à-vis various external sources of pressure. We will identify two of these sources – the plea for individual autonomy and the idea of Managed Care – and explain how they have promoted a strict biomedical conception of disease. The demand for objectivity, however, does not take into account the complexity of mental illness. It ignores that the psychiatrist’s profession is essentially characterized by fragility: fluctuating between scientific reduction and the irreducible complexity (...) of reality. Therefore, the psychiatrist is not in need of hard and fast rules, but of judgment. At the end, we suggest that philosophy could inject some healthy uncertainty within psychiatry in order to restore its fragile identity. Our examples are drawn from the Dutch situation but we are confident that they apply to other countries as well. (shrink)

The aim of this article is to compare the theories of Isaiah Berlin and Charles Taylor with regard to the topic of freedom. I will argue that Berlin’s famous positive-negative distinction still serves an important purpose by maintaining a crucial tension within the concept of liberty. This tension allows ethical pluralism to be taken seriously instead of being covered up by ideological retorics. Berlin held that the implementation of positive liberty – defining the boundaries of true liberty – is always (...) problematic. Taylor, however, tries to by-pass the gap between negative and positive liberty by means of his concept of authenticity. I will argue that this notion is a sound descriptive term for an individual’s entrenchment in community, but that the normative appeal from authenticity amounts to a project of ‘re-sourcing the self’ which is ultimately rooted in an optimist perspective on pluralism and multiculturalism. However, to the extent that there are indeed different communities with different values and different ways of being authentic, it is worthwhile to repeat the Berlinian Grundgedanke that human beings should cope with the inexorable and irreducible tragedy in moral life. Aberrant FAK signaling may contribute to the process of cell transformation by certain oncoproteins, including v‐Src. Progress toward elucidating the events leading to FAK activation following integrin‐mediated cell adhesion, as well as events downstream of FAK, has come through the identification of FAK phosphorylation sites and interacting proteins. Harrowing stories reported in the media describe Covid‐19 ravaging through families. This essay reports professional experiences of this phenomenon, family clustering, as encountered during the pandemic's spread across Southern California. We identify three ethical challenges following from it: Family clustering impedes shared decision‐making by reducing available surrogate decision‐makers for incapacitated patients, increases the emotional burdens of surrogate decision‐makers, and exacerbates health disparities for and the suffering of people of color at increased likelihood of experiencing family clustering. We propose that, in (...) response to these challenges, efforts in advance care planning be expanded, emotional support offered to surrogates and family members be increased, more robust state guidance be issued on ethical decision‐making for unrepresented patients, ethics consultation be increased in the setting of conflict following from family clustering dynamics, and health care professionals pay more attention to systemic and personal racial biases and inequities that affect patient care and the surrogate experience. Although ethics is an essential component of undergraduate medical education, research suggests current medical ethics curricula face considerable challenges in improving students’ ethical reasoning. This paper discusses these challenges and introduces a promising new mode of graduate and professional ethics instruction for overcoming them. We begin by describing common ethics curricula, focusing in particular on established problems with current approaches. Next, we describe a novel method of ethics education and assessment for medical students that we have devised, the Medical Ethics (...) Bowl. RECENT attempts to explain and justify Aristotle's principle of non-contradiction have focused to a great extent on the dialectical dimension of Aristotle's account. For example, T. Irwin maintains that Aristotle justifies the PNC by arguing that there is a sub-set of dialectical opinions which no one can rationally give up. J. Lear supports the importance of the dialectical dimension by summarizing Aristotle's defense of the PNC as follows: The opponent of the PNC tries to argue dialectically that one should not (...) accept it. "Aristotle's point is that there is no conceptual space in which such a rational discussion can occur." By appealing to the limits of rationally, externally, expressible discourse as the basis of the PNC, Irwin and Lear fail to take seriously the more explicitly psychological and metaphysical bases of the PNC. C. Kirwan, and most recently, R. Smith also either argue against or ignore the role of the psychological and metaphysical bases. Smith goes so far as to argue that common axioms like the PNC are simply empty statement schemata of universal validity, i.e., general verbal forms or patterns which no one can deny with intelligibility. Snell suggests that the common experience of the loss of meaning in the modern world can be attributed to the vice of acedia. Relying on Evagrius of Pontus, as well as the Angelic Doctor, St Thomas Aquinas, Snell defines acedia as a rejection of life itself and an antipathy with one’s place in the world. BackgroundIn the Canadian Alliance for Healthy Hearts and Minds (CAHHM) cohort, participants underwent magnetic resonance imaging (MRI) of the brain, heart, and abdomen, that generated incidental findings (IFs). The approach to managing these unexpected results remain a complex issue. Our objectives were to describe the CAHHM policy for the management of IFs, to understand the impact of disclosing IFs to healthy research participants, and to reflect on the ethical obligations of researchers in future MRI studies.MethodsBetween 2013 and 2019, 8252 participants (...) (mean age 58 ± 9 years, 54% women) were recruited with a follow-up questionnaire administered to 909 participants (40% response rate) at 1-year. The CAHHM policy followed a restricted approach, whereby routine feedback on IFs was not provided. Only IFs of severe structural abnormalities were reported.ResultsSevere structural abnormalities occurred in 8.3% (95% confidence interval 7.7–8.9%) of participants, with the highest proportions found in the brain (4.2%) and abdomen (3.1%). The majority of participants (97%) informed of an IF reported no change in quality of life, with 3% of participants reporting that the knowledge of an IF negatively impacted their quality of life. Furthermore, 50% reported increased stress in learning about an IF, and in 95%, the discovery of an IF did not adversely impact his/her life insurance policy. Most participants (90%) would enrol in the study again and perceived the MRI scan to be beneficial, regardless of whether they were informed of IFs. While the implications of a restricted approach to IF management was perceived to be mostly positive, a degree of diagnostic misconception was present amongst participants, indicating the importance of a more thorough consent process to support participant autonomy.ConclusionThe management of IFs from research MRI scans remain a challenging issue, as participants may experience stress and a reduced quality of life when IFs are disclosed. The restricted approach to IF management in CAHHM demonstrated a fair fulfillment of the overarching ethical principles of respect for autonomy, concern for wellbeing, and justice. In the final analysis, sustainable agriculture must derive from applied ecology, especially the principle of the regulation of the abundance and distribution of species (and, secondarily, their activities) in space and time. Interspecific competition in natural ecosystems has its counterparts in agriculture, designed to divert greater amounts of energy, nutrients, and water into crops. Whereas natural ecosystems select for a diversity of species in communities, recent agriculture has minimized diversity in favour of vulnerable monocultures. Such systems show intrinsically less stability (...) and resilience to perturbations. Some kinds of crop rotation resemble ecological succession in that one crop prepares the land for successive crop production. Such rotations enhance soil organic processes such as decomposition and material cycling, build a nutrient capital to sustain later crop growth, and reduce the intensity of pest buildup. Species in natural communities occur at discrete points along the r-K continuum of reproductive maturity. Clearing forested land for agriculture, rotational burning practices, and replacing perennial grassland communities by cereal monocultures moves the agricultural community towards the r extreme. Plant breeders select for varieties which yield at an earlier age and lower plant biomass, effectively moving a variety towards the r type. Features of more natural landscapes, such as hedgerows, may act as physical and biological adjuncts to agricultural production. They should exist as networks in agricultural lands to be most effective. Soil is of major importance in agroecosystems, and maintaining, deliberately, its vitality and resilience to agricultural perturbations is the very basis of sustainable land use. (shrink)

In der Auseinandersetzung mit der Semantik Alteuropas hat Niklas Luhmann darauf verwiesen, dass der Begriff der menschlichen Lebensform in der antiken Semantik jenen Platz besetzt hält, an dem eigentlich ein Gesellschaftsbegriff zu entwickeln wäre (I). Der Beitrag geht der Frage nach, ob der zeitgenössische Begriff der menschlichen Lebensform, der eine tragende Bedeutung in der post-wittgensteinianischen Philosophie besitzt, dadurch erhellt werden kann, dass man ihn als funktionales Äquivalent des Gesellschaftsbegriffs versteht. Anhand von Wittgensteins Philosophischen Untersuchungen wird nachgezeichnet, dass der Begriff sich (...) in der Tat als ein begriffliches Angebot für die Frage nach der »Realität des Sozialen schlechthin« auffassen lässt (II, III). Die heutigen Verwendungen des Begriffs weisen dabei allerdings noch immer jene Gefahren auf, die Luhmanns Vorbehalte gegenüber der antiken Semantik begründeten: die Gefahr, die unerreichbare Einheit des Sozialen zu reifizieren und die in einem solchen Begriff zu entfaltenden Probleme vorschnell zu verdecken (IV). Diesen Gefahren steht zugleich ein spezifisches Problematisierungspotential gegenüber, das den Begriff der Lebensform gegenüber dem Gesellschaftsbegriff auszeichnet. The theory of nonnegative matrices is an example of a theory motivated in its origins and development by purely mathematical concerns that later proved to have a remarkably broad spectrum of applications to such diverse fields as probability theory, numerical analysis, economics, dynamical programming, and demography. At the heart of the theory is what is usually known as the Perron–Frobenius Theorem. It was inspired by a theorem of Oskar Perron on positive matrices, usually called Perron’s Theorem. This paper is primarily (...) concerned with the origins of Perron’s Theorem in his masterful work on ordinary and generalized continued fractions (1907) and its role in inspiring the remarkable work of Frobenius on nonnegative matrices (1912) that produced, inter alia, the Perron–Frobenius Theorem. The paper is not at all intended exclusively for readers with expertise in the theory of nonnegative matrices. Anyone with a basic grounding in linear algebra should be able to read this article and come away with a good understanding of the Perron–Frobenius Theorem as well as its historical origins. The final section of the paper considers the first major application of the Perron–Frobenius Theorem, namely, to the theory of Markov chains. When he introduced the eponymous chains in 1908, Markov adumbrated several key notions and results of the Perron–Frobenius theory albeit within the much simpler context of stochastic matrices; but it was by means of Frobenius’ 1912 paper that the linear algebraic foundations of Markov’s theory for nonpositive stochastic matrices were first established by R. Music is a stimulus capable of triggering an array of basic and complex emotions. We investigated whether and how individuals employ music to induce specific emotional states in everyday situations for the purpose of emotion regulation. Furthermore, we wanted to examine whether specific emotion-regulation styles influence music selection in specific situations. Participants indicated how likely it would be that they would want to listen to various pieces of music (which are known to elicit specific emotions) in various emotional situations. Data (...) analyses by means of non-metric multidimensional scaling revealed a clear preference for pieces of music that were emotionally congruent with an emotional situation. In addition, we found that specific emotion-regulation styles might influence the selection of pieces of music characterised by specific emotions. Our findings demonstrate emotion-congruent music selection and highlight the important role of specific emotion-regulation styles in the selection of music in everyday situations. (shrink)
