Pancreatitis is a complex, progressively destructive inflammatory disorder. Alcohol was long thought to be the primary causative agent, but genetic contributions have been of interest since the discovery that rare PRSS1, CFTR and SPINK1 variants were associated with pancreatitis risk. We now report two associations at genome-wide significance identified and replicated at PRSS1-PRSS2 and X-linked CLDN2 through a two-stage genome-wide study. The PRSS1 variant likely affects disease susceptibility by altering expression of the primary trypsinogen gene. The CLDN2 risk allele is (...) associated with atypical localization of claudin-2 in pancreatic acinar cells. The homozygous CLDN2 genotype confers the greatest risk, and its alleles interact with alcohol consumption to amplify risk. François Furet (4) Any synthesis is incomplete which ends in an object or an abstract concept and not a living relationship between two individuals. René Girard (Deceit 178) Karl Marx offers two observations which I take as (...) the point of departure for a critique ofthe anti-liberal historicism in thinkers like Martin Heidegger, Friedrich Nietzsche, and Marx himself. The first is that the modern project of emancipation presupposes that "man is the supreme being for man," and the second is that modern equality gives rise to its own type of relations of /«equality, indeed of "master and slave." I will not interpret these observations as Marx himself did, in economic terms. Instead, I use them as clues to uncover the kind of human relation implicit in but also hidden by the ontology of freedom on which radical historicism rests. By ontology offreedom, I mean the view that "being" is historical and grounded in radical freedom or contingency. For this theory, history is a process ofcreation ex nihilo, a power traditionally reserved for God. I suggest that the fascination ofthe radical critique ofbourgeois society with historical "being" reflects the transformation of human relations by "equality of conditions," in the sense of Alexis de Tocqueville, the French sociologist and student ofthe democratic soul.2 As modern equality subverts the old orthodoxies, human beings tend to seek their gods in each other, as models of "freedom." And so there arises a "master-slave" psychology from the impact of equality, legitimized by romanticism and the modern cult ofEros. This, I suspect, is the effectual truth offreedom in Heidegger, Nietzsche, and Marx.3 René Girard offers a classic description of this psychology in his analysis ofthe modern novel, Deceit, Desire, and the Novel, to which I am indebted throughout this paper. As regards that book, my general thesis—applicable not only to Marx but also to Heidegger and Nietzsche—may be stated as follows.4 Girard's analysis of the literary characters Don Quixote, Julien Sorel, Emma Bovary, Raskolnikov, and others is in effect a devastating if indirect critique of modem radical thought, or the pursuit ofradical freedom. Modern novels, he argues, tend to divide into two types: either "romantic" or "romanesque." The latter reveal the romantic illusions of the modern psyche, rather than merely reflect them like the former. "Romanesque" novelists such as Cervantes, Stendhal, Flaubert, Dostoevsky, and Proust are great because they uncover the human relations behind romantic passions and the theories that legitimate them. The "romanesque" novel is a Socratic labor of selfknowledge in which the author emancipates himself from his own past romantic illusions. In effect, it exposes the fictions associated with the modern novel itself. Over 115 000 people are waiting for life-saving organ transplants, of whom a small fraction will receive transplants and many others will die while waiting. Existing efforts to expand the number of available organs, including increasing the number of registered donors and procuring organs in uncontrolled environments, are crucial but unlikely to address the shortage in the near future and will not improve donor/recipient compatibility or organ quality. If successful, organ bioengineering can solve the shortage and improve functional outcomes. Studying (...) manufactured organs in animal models has produced valuable data, but is not sufficient to understand viability in humans. Before risking manufactured organ experimentation in living humans, study of bioengineered organs in recently deceased humans would facilitate evaluation of the function of engineered tissues and the complex interactions between the host and the transplanted tissue. Although such studies do not pose risk to human subjects, they pose unique ethical challenges concerning the previous wishes of the deceased, rights of surviving family members, effective operation and fair distribution of medical services, and public transparency. Last year saw the 20th anniversary edition of JECP, and in the introduction to the philosophy section of that landmark edition, we posed the question: apart from ethics, what is the role of philosophy ‘at the bedside’? The purpose of this question was not to downplay the significance of ethics to clinical practice. Rather, we raised it as part of a broader argument to the effect that ethical questions – about what we should do in any given situation – are (...) embedded within whole understandings of the situation, inseparable from our beliefs about what is the case, what it is that we feel we can claim to know, as well as the meaning we ascribe to different aspects of the situation or to our perception of it. Philosophy concerns fundamental questions: it is a discipline requiring us to examine the underlying assumptions we bring with us to our thinking about practical problems. Traditional academic philosophers divide their discipline into distinct areas that typically include logic: questions about meaning, truth and validity; ontology: questions about the nature of reality, what exists; epistemology: concerning knowledge; and ethics: how we should live and practice, the nature of value. Any credible attempt to analyse clinical reasoning will require us to think carefully about these types of question and the relationships between them, as they influence our thinking about specific situations and problems. So, the answers to the question we posed, about the role of philosophy at the bedside, are numerous and diverse, and that diversity is illustrated in the contributions to this thematic edition. Most donated organs in the United States come from brain dead donors, while a small percentage come from patients who die in “controlled,” or expected, circumstances, typically after the family or surrogate makes a decision to withdraw life support. The number of organs available for transplant could be substantially if donations were permitted in “uncontrolled” circumstances–that is, from people who die unexpectedly, often outside the hospital. According to projections from the Institute of Medicine, establishing programs permitting “uncontrolled donation after circulatory (...) determination of death,” or uDCDD, throughout the United States has the potential to provide 22,000 more donation opportunities annually. In contrast, U.S. controlled donation after circulatory determination of death, or cDCDD, cases have increased progressively over the past decade from 87 to 848 donors, but currently account for only 10.6 percent of all deceased donors. Following the IOM recommendations, several projects exploring the feasibility of uDCDD were funded by the federal government, including a grant from the Health Resources and Services Administration that supported a pilot project in New York City in which the authors of this article participated.A key feature of our protocol, and indeed of many uDCDD protocols, is the initiation of preservation methods such as chest compressions and extracorporeal membrane oxygenation shortly after death in order to perfuse and preserve the donor's organs. Critics of uDCDD argue that the means of determining death deviates from generally ascribed principles. They assert that reinstituting circulation in order to preserve organs has the effect of “undoing” the prior determination of death. The result is that cDCDD is widely accepted and practiced routinely even though it only marginally increases the number of organs available for transplantation, and uDCDD is widely considered unacceptable despite being ethically embraced and proven to significantly increase organ donation opportunities in other countries. This article explores the evolution of this counterintuitive state of affairs and calls for a policy that, in line with the IOM report, allows for both cDCDD and uDCDD protocols. (shrink)

Over the course of the past 15 years, there has been a vast sea change in American psychoanalysis. It takes the form of a broad movement away from classical psychoanalytic theorizing grounded in Freud's drive theory toward models of mind and development grounded in object relations concepts. In clinical practice, there has been a corresponding movement away from the classical principles of neutrality, abstinence and anonymity toward an interactive vision of the analytic situation that places the analytic relationship, with its (...) powerful, reciprocal affective currents, in the foreground. These developments have been evident in virtually all schools of psychoanalysis in America, from the most traditional to the most radical. This paper argues that analyses of the ways in which Big Data has been enacted in other academic disciplines can provide us with concepts that will help understand the application of Big Data to social questions. We use examples drawn from our Science and Technology Studies analyses of -omic biology and high energy physics to demonstrate the utility of three theoretical concepts: primary and secondary inscriptions, crafted and found data, and the locus of legitimate interpretation. These help us to show (...) how the histories, organisational forms, and power dynamics of a field lead to different enactments of big data. The paper suggests that these concepts can be used to help us to understand the ways in which Big Data is being enacted in the domain of the social sciences, and to outline in general terms the ways in which this enactment might be different to that which we have observed in the ‘hard’ sciences. We contend that the locus of legitimate interpretation of Big Data biology and physics is tightly delineated, found within the disciplinary institutions and cultures of these disciplines. We suggest that when using Big Data to make knowledge claims about ‘the social’ the locus of legitimate interpretation is more diffuse, with knowledge claims that are treated as being credible made from other disciplines, or even by those outside academia entirely. Within the school of thought known as Critical Thinking, identifying or finding missing assumptions is viewed as one of the principal thinking skills. Within the new subject in schools and colleges, usually called Critical Thinking, the skill of finding missing assumptions is similarly prominent, as it is in that subject's public examinations. In this article we examine how school- and college-focused texts explain and teach ‘this very important skill’. The same texts also deal with the nature of assumptions, validity and (...) the role of stated reasons in arguments, and the way these matters are tackled will also be examined in our inquiry. In addition we explore what respected contributors to the critical thinking movement have had to say about some of these issues. (shrink)

The medical profession is not to blame for the limitations of the biomedical model with which it is often associated; the biology upon which that model is built is incomplete and bears some of the responsibility. Some of the more fundamental aspects of biological theory which are currently missing from the biomedical model need to be introduced in order to help provide a better description of the integrated biology involved. AIMS AND OBJECTIVES: By considering the biological nature of the individual (...) human being as an organism, this article introduces a brief sketch of a new biomedical model of the individual as a biological entity. In April 2017, scientists and science sympathizers held marches in the United Kingdom as part of a coordinated international March for Science movement that was held in over 600 cities worldwide. This article reports from participant-observation studies of the marches that took place in London and Cardiff. Supplemented with data from 37 interviews from marchers at the London event, the article reports on an analysis of the placards, focusing on marchers’ concerns and the language and images through which they expressed (...) those concerns. How did the protesters articulate their concerns and objectives, and how were these articulations used to build a community? The placards did not represent a clear, focused, and unifying message; they instead illustrated disparate concerns ranging from human-induced climate change, Trump and “alternative facts,” and local UK specific political issues concerning the country’s exit from the European Union. Our analysis shows that placards gave a playful and whimsical character to the march, with slogans displaying significant amounts of humor, often formulated through insider jokes, scientific puns, or self-deprecating appropriation of negative stereotypes about scientists. We analyze the march through the social movement literature and as a collective identity-building exercise for an community of scientists and sympathizers with long-term aims of establishing a louder voice for scientists, and experts, in public discourse. The essay discusses the March 5, 1944 "Discussion on Sin," an event that was held between French intellectual Georges Bataille and the Jesuit priest and patristics scholar Jean Daniélou, along with other important Christian and non-Christian intellectuals. I argue that the event is the best recorded wartime intellectual encounter between the founders of contestation (subsequently so important in deconstructive thought) and serious practitioners of Christianity. Biological scientists are increasingly encountering work-related ethical problems. For most, their training leaves them quite unprepared. Rather than merely providing additional bolt-on courses in ethics, a way of introducing critical thinking skills seamlessly into the curriculum is proposed. A method is described whereby students become engaged in self-generated discussion about the scientifically recognized, but philosophically complex, terms ‘disease’ and ‘health.’ Addressing these words, students are confrontedwith the need to develop critical thinking skills without realizing that they are entering into overt (...) philosophical argument—the like of which many often prejudge to be abstract and worthless. While humanists have pondered the subject of love to the point of obsessiveness, philosophers have steadfastly ignored it. One might wonder whether the discipline of philosophy even recognizes love. The word _philosophy _means “love of wisdom,” but the absence of love from philosophical discourse is curiously glaring. So where did the love go? In _The Erotic Phenomenon,_ Jean-Luc Marion asks this fundamental question of philosophy, while reviving inquiry into the concept of love itself. Non-suicidal self-injury has consistently been associated with self-reported difficulties in emotion reactivity and the regulation of negative emotions; however, less is known about the accuracy of these self-reports or the reactivity and regulation of positive emotions. The present study sought to investigate differences between women with and without a history of NSSI on: self-reported general tendencies of negative and positive emotion reactivity, self-reported general tendencies of negative and positive emotion regulation, and emotion regulation reported in response to a positive and (...) negative mood induction. The sample consisted of 36 women with a recent history of NSSI within the last 2 years and a comparison group with no history of NSSI. Participants completed self-report measures of negative and positive emotion reactivity and regulation. In a separate session, participants underwent both a negative and positive mood induction using a counterbalanced design and reported their experienced emotions. Results from two-way MANOVAs and ANOVAs revealed those with a history of NSSI reported significantly greater difficulties in negative emotion reactivity and regulation than the no-NSSI comparison group; however, no group differences emerged in self-reported reactivity or regulation of positive emotions. In contrast, repeated measures ANOVAs on data from the mood induction task found no group differences in reactivity or regulation for either negative or positive emotions. These findings highlight the possibility that although individuals with a history of NSSI evaluate their ability to manage negative emotions as significantly worse than individuals with no history of self-injury, this may not reflect their actual emotion regulatory processes. The effectiveness of information retrieval technology in electronic discovery (E-discovery) has become the subject of judicial rulings and practitioner controversy. The scale and nature of E-discovery tasks, however, has pushed traditional information retrieval evaluation approaches to their limits. This paper reviews the legal and operational context of E-discovery and the approaches to evaluating search technology that have evolved in the research community. It then describes a multi-year effort carried out as part of the Text Retrieval Conference to develop evaluation methods (...) for responsive review tasks in E-discovery. This work has led to new approaches to measuring effectiveness in both batch and interactive frameworks, large data sets, and some surprising results for the recall and precision of Boolean and statistical information retrieval methods. The paper concludes by offering some thoughts about future research in both the legal and technical communities toward the goal of reliable, effective use of information retrieval in E-discovery. This selection of articles by Lewis E. Hahn addresses the philosophical school of contextualism and four contemporary American philosophers: John Dewey, Henry Nelson Wieman, Stephen C. Pepper, and Brand Blanshard. Stressing the relatively recent contextualistic worldview, which he considers one of the best world hypotheses, Hahn seeks to achieve a broad perspective within which all things may be given their due place. After providing a brief outline, Hahn explains contextualism in relation to other philosophies. In his opening chapter, (...) as in later chapters, he expresses contextualism as a form of pragmatic naturalism. In spite of Hahn’s high regard for contextualism, however, he does not think it would be good if we were limited to a single worldview. “The more different views we have and the more different sources of possible light we have, the better our chances that some of these cosmic maps will shed light on our world and our place in it.”. This paper argues that the innovation of an ordering source parameter in the standard Lewis-Kratzer semantics for modals was a mistake, at least for English auxiliaries like ‘ought’, and that a simpler dyadic semantics (as proposed in my earlier work) provides a superior account of normative uses of modals. I programmatically investigate problems arising from (i) instrumental conditionals, (ii) gradability and “weak necessity”, (iii) information-sensitivity, and (iv) conflicts, and show how the simpler semantics provides intuitive solutions given three basic (...) moves: (1) an end-relational analysis of normative modality, (2) a “most” analysis of “weak necessity”, and (3) well-motivated appeals to pragmatics. This chapter focuses on a question: how does the intentionality of language 'derive' from the original intentionality of thought. Hardly any philosopher of language would deny that if something is an expression which has meaning in a population, then that is by virtue of facts about the linguistic behavior and psychological states of members of that population. The chapter starts with a reconstruction of Lewis's account of the relation in Convention because a problem that immediately arises for that account (...) provides a natural segue to the more ambitious Intention‐Based Semantics (IBS) project. IBS is a program for reducing all questions about the intentionality of speech acts and linguistic expressions to questions about the intentionality of thought. It takes as foundational in the theory of meaning a certain notion of speaker‐meaning and seeks to define it, without recourse to any semantic notions, in terms of acting with certain audience‐directed intentions.
