It has frequently been suggested that science and religion are innately in conflict. One example from the history of medicine is the introduction of anaesthesia into obstetrics in 1847, which is commonly said to have stimulated massive religious opposition. Historians have almost unanimously averred that such opposition arose from the belief that obstetric anaesthesia interfered with the primeval curse— ‘In sorrow thou shalt bring forth children’ . Despite considerable opposition to obstetric anaesthesia upon medical, physiological, and general moral grounds, evidence (...) of genuine religious opposition in contemporary sources has proved to be virtually non-existent. On examination, this particular ‘conflict’ appears to be an artifact of historiography based upon a contemporary defence prepared against an attack which never materialized. (shrink)

In 1795 the Marquis de Sade published his La Philosophic dans le boudoir, in which he proposed the use of induced abortion for social reasons and as a means of population control. It is from this time that medical and social acceptance of abortion can be dated, although previously the subject had not been discussed in public in modern times. It is suggested that it was largely due to de Sade's writing that induced abortion received the impetus which resulted in (...) its subsequent spread in western society. (shrink)

Context Conflicts over treatment decisions have been linked to physicians' emotional states. Objective To measure the prevalence of emotional exhaustion and conflicts over treatment decisions among US obstetrician/gynaecologists (ob/gyns), and to examine the relationship between the two and the physician characteristics that predict each. Methods Mailed survey of a stratified random sample of 1800 US ob/gyn physicians. Criterion variables were levels of emotional exhaustion and frequency of conflict with colleagues and patients. Predictors included physicians' religious characteristics and self-perceived empathy. Results (...) Response rate among eligible physicians was 66% (1154/1760). 36% of ob/gyns reported high levels of emotional exhaustion, and majorities reported conflict with colleagues (59%) and patients (61%). Those reporting conflict were much more likely to report emotional exhaustion (58% vs 29% who never conflict, OR, 95% CI 2.8, 1.6 to 4.8 for conflict with colleagues; 55% versus 26%, OR, 95% CI 2.2, 1.4 to 3.5 for conflict with patients). Physicians with lower self-perceived empathy were more likely to report physician-patient conflicts (65% vs 58% with higher empathy, OR, 95% CI 1.4, 1.0 to 1.9), as were female ob/gyns (66% vs 57% of males, OR, 95% CI 1.5, 1.1 to 2.0). Foreign-born physicians were less likely to report such conflicts (47% vs 64% of US born, OR, 95% CI 0.5, 0.4 to 0.8). Physicians' religious characteristics were not significantly associated with reporting conflict. Conclusions Conflicts over treatment decisions are associated with physicians' empathy, gender, immigration history and level of emotional exhaustion. With respect to the latter, conflict in the clinical encounter may represent an overlooked source or sign of burnout among ob/gyns. (shrink)

It is an unresolved question in quantum mechanics whether quantum states apply to individual quantum systems, or to ensembles of quantum systems. We show by way of a thought experiment that quantum states apply only to ensembles of quantum systems. A further unresolved question is whether quantum systems possess ontic states. If a quantum state is the state of an ensemble, as we claim, the answer to this question is that quantum states are not ontic. However, a notable recent result (...) in quantum foundations shows that if there are any ontic states at all, then the quantum state must be ontic. Collectively, these two results imply that there are no ontic states. We examine the assumptions required for these results, and suggest that the retrospective effect on state preparations by entangling measurements provides good reason for relaxing the assumption of preparation independence at the ontic level. Background While prenatal surgery historically was performed exclusively for lethal conditions, today intrauterine surgery is also performed to decrease postnatal disabilities for non-lethal conditions. We sought to describe physicians' attitudes about prenatal surgery for lethal and non-lethal conditions and to elucidate characteristics associated with these attitudes. Methods Survey of 1200 paediatric surgeons, neonatologists and maternal–fetal medicine specialists. Results Of 1176 eligible physicians, 670 responded. In the setting of a lethal condition for which prenatal surgery would likely result in the child (...) surviving with a severe disability, most respondents either disagreed or strongly disagreed that they would recommend the surgery. Male physicians were twice as likely to recommend surgery for the lethal condition, as were physicians who believe that abortion is morally wrong. Older physicians were less likely to recommend surgery. For non-lethal conditions, most respondents agreed that they would recommend prenatal surgery, even if the surgery increases the risk of prematurity or fetal death. Compared with MFMs, surgeons were less likely to recommend such surgery, as were physicians not affiliated with a fetal centre, and physicians who were religious. Conclusion Physician’s attitudes about prenatal surgery relate to physicians’ beliefs about disability as well as demographic, cultural and religious characteristics. Given the variety of views, parents are likely to receive different recommendations from their doctors about the preferable treatment choice. (shrink)

Market metaphors have come to dominate discourse on medical practice. In this essay, we revisit Peter Berger and colleagues’ analysis of modernization in their book The Homeless Mind and place that analysis in conversation with Max Weber’s 1917 lecture “Science as a Vocation” to argue that the rise of market metaphors betokens the carry-over to medical practice of various features from the institutions of technological production and bureaucratic administration. We refer to this carry-over as the product presumption. The product presumption (...) foregrounds accidental features of medicine while hiding its essential features. It thereby confounds the public understanding of medicine and impedes the professional achievement of the excellences most central to medical practice. In demonstrating this pattern, we focus on a recent article, “Physicians, Not Conscripts—Conscientious Objection in Health Care,” in which Ronit Stahl and Ezekiel Emanuel decry conscientious refusals by medical practitioners. We demonstrate that Stahl and Emanuel’s argument depends on the product presumption, ignoring and undermining central features of good medicine. We conclude by encouraging conscientious resistance to the product presumption and the language it engenders. (shrink)

Evolutionary psychology and human sociobiology often reject the mere possibility of symbolic causality. Conversely, theories in which symbolic causality plays a central role tend to be both anti-nativist and anti-evolutionary. This article sketches how these apparent scientific rivals can be reconciled in the study of disgust. First, we argue that there are no good philosophical or evolutionary reasons to assume that symbolic causality is impossible. Then, we examine to what extent symbolic causality can be part of the theoretical toolbox of (...) the evolutionary social sciences. This examination leads to the conclusion that it is possible to make evolutionary sense of Mary Douglas’s theory of disgust, and that her view of symbolic causality can and should inform evolutionary theories of (sociocultural) disgust. The monograph shows that in the last 50 years theoretical linguistics remains a compendium of mutually contradicting doctrines on multiple levels: the level of general theories of language, the level of its main constituents (the lexicon, syntax, and the lexical-syntactic interface that connects them), and the lower levels of specific linguistic problems (such as lexical polysemy, grammatical meanings, etc.). The contradictions of contemporary linguistic theories are indicative of a deep crisis. Evolutionary-synthetic theory of language is aimed at overcoming this crisis. (...) It implements a multidisciplinary approach that describes the language a) in evolutionary and ontogenetic development and b) in close interaction with other human subsystems: the mental representation of the world, human activity and social interaction. Language is defined as a dual system developing in ontogeny: 'universal sensory sublanguage + ethnically specific functional sublanguage' where the sensory sublanguage is used to describe the 'visible' world (accessible to the senses), and the functional sublanguage is employed to describe the conceivable world ('comprehended by human mind'). Within the framework of the approach presented, many problems are solved: 1) the cognitive (non-verbal) language for representing language meanings; 2) diversity and unity of languages (using the structure alternative to Chomsky's Universal Grammar); 3) the main (original) function of the language; 4) the cognitive language of thought (its basis is the lexical and grammatical meanings of the sensory sublanguage); 5) dual (functionally prototypical) structure of human categories; 6) mechanisms in the early acquisition of lexical and grammatical meanings; 7) the meaning (assignment) of the syntactic structure of a sentence, etc. The book deals with a wide range of issues related to human cognition, glottogenesis and anthropogenesis, universal cognitive structures for the representation of objects, actions and situations, so it will be of interest not only to linguists, but also to psychologists, anthropologists, sociologists, philosophers. Unlike some other reproductions of classic texts (1) We have not used OCR(Optical Character Recognition), as this leads to bad quality books with introduced typos. (2) In books where there are images such as portraits, maps, sketches etc We have endeavoured to keep the quality of these images, so they represent accurately the original artefact. Although occasionally there may be certain imperfections with these old texts, we feel they deserve to be made available for future generations to enjoy.

That a coherent account of the origins and early history of the British Association for the Advancement of Science has yet to be written is not altogether surprising. Even when the facts of the matter have been retrieved from the scattered papers of Babbage, Brewster, J. D. Forbes, Murchison, John Phillips, Vernon Harcourt, Whewell, and the rest, their organization into a connected whole remains a formidable business. The present paper seeks to identify the roles played in this important chapter in (...) the chronicles of British science by David Brewster (1781–1868), the Scottish natural philosopher, and William Vernon Harcourt (1789–1871), the York clergyman. Inquiries of this kind—into the proper apportioning of the credit for a discovery, a technique, or the rise of an institution—are only saved from sterility if they make possible a better understanding of the critical events. The present review of the origins of the British Association leads to the modest but important conclusion that the organization brought into existence by Vernon Harcourt at York in September 1831 was subtly but significantly different from that which had originally been proposed by Brewster. If this is so, some of the existing accounts of the matter stand in need of revision. (shrink)
