We propose a novel interpretation of Lewis on the analysis of modality that is constructed from primary sources, comprehensive and unprecedented. Our guiding precepts are to distinguish semantics from metaphysics, while respecting the inter-relations between them, and to discern whatever may be special, semantically or metaphysically, about the modal case. Following detailed presentation, we amplify and advocate our interpretation by providing a conforming genealogy of Lewis’s theory of modality and applying it to construct a detailed and newly illuminating version of (...) the Lewisian theory of modality de re. The emergence of clinical ethicists raises it again, as a question about professionalism. What role clinical ethicists should play in healthcare decision making – teacher, mediator, or consultant – is a question that has generated considerable debate but no consensus.

Hospital ethics committees have become widespread over the last 25 years, stimulated by the Quinlan decision of the New Jersey Supreme Court, the report of a President's Commission, and most recently by the Joint Commission on Accreditation of Health Care Organizations , which now man dates that each hospital seeking accreditation have a functioning process for the consideration of ethical issues in patient care. Laws and regulations in several states require that hospitals establish ethics committees, and some states stipulate that (...) certain types of cases and disputes be taken to such committees. Conventionally, multilevel analysis of covariance (ML-ANCOVA) has been the recommended approach for analyzing treatment effects in quasi-experimental multilevel designs with treatment application at the cluster-level. In this paper, we introduce the generalized ML-ANCOVA with linear effect functions that identifies average and conditional treatment effects in the presence of treatment-covariate interactions. We show how the generalized ML-ANCOVA model can be estimated with multigroup multilevel structural equation models that offer considerable advantages compared to traditional ML-ANCOVA. The proposed model takes into account measurement (...) error in the covariates, sampling error in contextual covariates, treatment-covariate interactions, and stochastic predictors. We illustrate the implementation of ML-ANCOVA with an example from educational effectiveness research where we estimate average and conditional effects of early transition to secondary schooling on reading comprehension. Moral and social arguments weigh heavily against performing medical procedures solely for purposes of sex selection. The medical profession has a responsibility to abandon its posture of ethical neutrality and take a firm stand now against sex selection.

The controversy currently surrounding national literatures in foreign languages can be summed up by the famous statement: “to be or not to be, that is the question.”* But how is one to be oneself without having been that other whom one cannot deny (in the Marxist sense) since one sings as one grows? How, whatever effort one makes to deny one's father, can one fail to be presently what one is by separating oneself from the story of one's own birth (...) or from the birth of one's own story? As the potential for the first human trials of somatic cell gene therapy nears, two ethical issues are examined: (1) problems of moral choice for members of institutional review boards who consider the first protocols, for parents, and for the clinical researchers, and the special protections that may be required for the infants and children to be involved, and (2) ethical objections to somatic cell therapy made by those concerned about a putative inevitable progression of genetic knowledge from therapy to (...) mass genetic engineering in human reproduction. The author's viewpoint is that a consensus exists on the required moral approach to somatic cell therapy, but that no moral approach yet exists for experiments beyond this level, especially in the germline cells of human beings. Gallin, Director of the Magnuson Clinical Center of the National Institutes of Health (NIH), convened a one-day "Conference on the Future of Clinical Bioethics at the National Institutes of Health Intramural Program." Conferees included NIH officials and a panel of consultants from bioethics programs around the nation.1 The subject was (...) the future of NIH's intramural bioethics program and specifically a vision of a new multi-component program, presented by Dr. Gallin, the first speaker. Dr. Harold Varmus, Director of NIH, was there to assure his support for Dr. Gallin's vision. This author, honored to be second speaker, was invited to sketch a brief history of bioethics at NIH. The present article reviews some of this history and then reports on Dr. Gallin's proposal, points of consensus among speakers and conferees, and the search for a Chief of the Clinical Bioethics Program.History of Bioethics at NIHIt is often said that NIH is a well-kept secret as the nation's leading biomedical research facility. Also, few people know that NIH has a long history in bioethics. Some key events in that history are shown in Figure 1, beginning with two that occurred prior to the birth of bioethics. NIH made an early policy response to protect human subjects of research and to find acceptable limits for scientific freedom. The Clinical Center (CC), a 550-bed research hospital, opened in 1953. A subcommittee of the Medical Board, the Clinical Research Committee (CRC), was formed for prior group review of all studies with normal volunteers. The traditional practice was for researchers themselves to decide when studies would begin, which subjects to recruit, and whether informed consent would be sought. The CRC intervention bit into this tradition. In addition, patient studies that posed significant risks or departed widely from medical practice were reviewed by referral from the Clinical Directors of the various Institutes. Was this CRC an early institutional review [End Page 355] board (IRB)? As a guest researcher, I was permitted to observe the CRCs meetings between 1966 and 1969. It functioned much like an IRB but had no community or non-scientist members. Other IRB-like review groups undoubtedly existed prior to the CRC. Robert Levine (personal communication, September 7, 1995) recalls that Beth Israel Hospital in Boston had one in 1938. The policy was the first in any nation to mandate ethics committees for clinical research. Modernity has been working since the sixteenth century in western Europe at what Mr. Gauchet has described as the “exit from religion,” adding that Christianity alone has been able to gain the historical position of “the religion of the exit from religion.” It is indeed the case that the other great religions have not felt, as Christianity has, the intellectual, political and legal necessity to revise their theological foundations radically. Islam in particular has not only been shielded from the fundamental (...) criticisms of intellectual and scientific modernity, but the managers of the sacred have formed alliances with nationalist movements engaged in the anti-colonialist struggle to legitimize wars of liberation and post-independence one-party states. It is true that Islam's mechanisms of state control first came into play in the period of prose-lytism of the new religion, chiefly between 622 and 632; but state control turned into the complete takeover of the irreducible autonomy of the religious domain (the spiritual sphere) by all the states that emerged after the 1920s (Ataturk's radical experiment) and even more so after the post-1945 liberations. The conflict which has tended recently to crystallize in particular around the name of Martin Heidegger goes back a long way. Where do philosophers stand in relation to political and social reality? What assistance can their problems and insights offer the process of coming to terms with this reality? In the context of the discussions surrounding Farias's book I set out my own position in Paris in November 1987; the full text was later published in German under the heading “Return (...) to Syracuse?” This title referred to the disappointment Plato felt in his own time when, at the invitation of the tyrant of Syracuse, that city's absolute ruler, he had twice gone to initiate the young prince in the basic principles of his thought concerning the just idea of the state and the just ordering of society. Things turned out badly and Plato had considerable difficulty getting back home again. Later still he was to suffer bitter disappointment when his intimate associate Dion, a member of the innermost circle of his academic community, was suddenly murdered by his own friends after heading a victorious operation to liberate Syracuse (one would like to know more about what lay behind that particular assassination). “Copying”: this practice, in China as elsewhere, was and still is the first exercise of every apprenticeship at the same time as an irreplaceable technique for spreading know-how, talent, and innovation; but the place and interest accorded to it throw light on the rather special positions being taken up. Thus, when a Chinese author speaks of copy, he is thinking primarily of the “copy-image,” in two dimensions. Sculpture in China plays a religious and propitiatory role; it only indirectly gives rise (...) to reflections about art. The copies of objects thus found themselves relegated to the sphere of the utilitarian, even of the frankly deceptive (the “fakes”); they held little interest for esthetes, with a single exception: that of archaic bronzes, which I shall be dealing with later since they fall within the scope of an overall reflection on history, ritual, and the foundations of the state. Now the latter, like the bases of painting, flow from an original cultural context, the elements of which that are most resistant to comparison are language and writing. (shrink)
