The problems of racism and racially motivated violence in predominantly African American communities in the United States are complex, multifactorial, and historically rooted. While these problems are also deeply morally troubling, bioethicists have not contributed substantially to addressing them. Concern for justice has been one of the core commitments of bioethics. For this and other reasons, bioethicists should contribute to addressing these problems. We consider how bioethicists can offer meaningful contributions to the public discourse, research, teaching, training, policy development, and (...) academic scholarship in response to the alarming and persistent patterns of racism and implicit biases associated with it. To make any useful contribution, bioethicists will require preparation and should expect to play a significant role through collaborative action with others. (shrink)

Intersectionality has become a significant intellectual approach for those thinking about the ways that race, gender, and other social identities converge in order to create unique forms of oppression. Although the initial work on intersectionality addressed the unique position of black women relative to both black men and white women, the concept has since been expanded to address a range of social identities. Here we consider how to apply some of the theoretical tools provided by intersectionality to the clinical (...) context. We begin with a brief discussion of intersectionality and how it might be useful in a clinical context. We then discuss two clinical scenarios that highlight how we think considering intersectionality could lead to more successful patient–clinician interactions. Finally, we extrapolate general strategies for applying intersectionality to the clinical context before considering objections and replies. (shrink)

Deborah is a thirty-three-year-old who presented to labor and delivery at thirty-seven weeks gestation with complaints of contractions. Upon arrival, she explained that her fetus, Nathan, had been diagnosed with a “lethal” condition by her primary obstetrician. At twenty-two weeks gestation, an amniocentesis confirmed trisomy 13, a chromosomal abnormality leading to miscarriage or stillbirth in nearly one-half of affected pregnancies. During the admission process, Deborah voices the worry that due to Nathan's brain and heart structure, vaginal delivery could be traumatic (...) and cause him to suffer. Deborah wishes for him to have as painless and as dignified a death as possible; cesarean section, she feels, will achieve this. Yet with her history of three prior vaginal deliveries, normally progressing labor, and poor fetal prognosis that is unlikely to improve with cesarean delivery, there is no maternal or fetal indication for a cesarean section. Should the obstetrician proceed with a cesarean delivery despite knowing that it would expose the mother to surgical risks with little or no corresponding fetal or neonatal benefit? (shrink)

This article explores the concept of internationally acceptable codes of ethics within the context of an Egyptian nurse’s PhD studies. Theoretical work, including gaining ethical approval for the project, took place in the UK, while the data collection phase of the study was done in Egypt. This highlighted areas where the Arab Muslim interpretation of some ethical principles, especially around the issue of gaining informed consent, differed from that currently accepted in British research ethics. The authors argue that it may (...) not be possible, or even desirable, to standardize codes of ethics globally in areas such as academic research. Ethical principles develop from a unique mix of culture and religion. It may be more important to develop cultural competence that includes the ability to understand and respect the way in which ethical principles are interpreted by various societies. (shrink)

Background Despite its negative impact on patients and nurses, the use of restraint in somatic health care continues in many settings. Understanding the reasons and justifications for the use of restraint among nurses is crucial in order to manage this challenge. Aim To understand nurses’ justifications for restraint use in neurosurgical care. Research design A qualitative, descriptive design was used. Data were analysed with inductive qualitative content analysis. Participants and research context Semi-structured interviews with 15 nurses working in three neurosurgical (...) departments in Sweden. Ethical considerations Approved by The Regional Ethics Committee, Stockholm, Sweden. Findings The analysis resulted in three categories. The category Patient factors influencing restraint use describes patient factors that trigger restraint, such as a diminished decision-making competence, restlessness, and need for invasive devices. The category Specific reasons for justifying restraint describes reasons for restraining patients, such as restraint being used for the sake of the patient or for the sake of others. The category General reasoning in justifying restraint describes how nurses reason when using restraint, and the decision to use restraint was often based on a consequentialist approach where the nurses’ weighed the pros and cons of different alternatives. Discussion Nurses with experience of restraint use were engaged in a constant process of justifying and balancing different options and actions. Restraint was considered legitimate if the benefit exceeded the suffering, but decisions on which restraint measures to use and when to use them depended on the values of the individual nurse. Conclusion How nurses reason when justifying restraint, why they use restraint, and who they use restraint on must be considered when creating programs and guidelines to reduce the use of restraint and to ensure that when it is used it is used carefully, appropriately, and with respect. Faith development theory has evolved as a prominent theoretical perspective during the past three decades to explain different ways of relating to religious beliefs and worldviews. Recent revisions of the theory have elaborated on these characteristic ways as religious styles namely the fundamentalist, mutual, individuative-systemic, and dialogical. The present study developed an Urdu version of its principal measure, i.e., Faith Development Interview, to analyze twelve cases of Muslims of various religious affiliations within Islam in Pakistan. Four case studies representative of (...) each faith style are presented in detail. The cases are compared to analyze Islamic faith in terms of faith development theory and to understand fundamentalism in a Muslim context. The findings support faith development theory as a comprehensive paradigm to address the varieties of faith orientations in Islam. Implications for future research with larger samples in highly religious and collectivistic cultures are discussed. (shrink)

I argue that its appearing to you that P does not provide justification for believing that P unless you have independent justification for the denial of skeptical alternatives – hypotheses incompatible with P but such that if they were true, it would still appear to you that P. Thus I challenge the popular view of ‘dogmatism,’ according to which for some contents P, you need only lack reason to suspect that skeptical alternatives are true, in order for an experience as (...) of P to justify belief that P. I pursue three lines of objection to dogmatism, having to do with probabilistic reasoning, considerations of future or hypothetically available justification, and epistemic circularity. Restraint is regularly used in somatic healthcare settings, and countries have chosen different paths to regulate restraint in somatic healthcare. One overarching problem when regulating restraint is to ensure that patients with reduced decision-making capacity receive the care they need and at the same time ensure that patients with a sufficient degree of decision-making capacity are not forced into care that they do not want. Here, arguments of justice, trust in the healthcare system, minimising harm and respecting autonomy are contrasted (...) with different national regulations. We conclude that a regulation that incorporates an assessment of patients’ decision-making capacity and considers the patient’s best interests is preferable, in contrast to regulations based on psychiatric diagnoses or regulations where there are no legal possibilities to exercise restraint at all in somatic care. I believe that Tom is the proud father of a baby boy. Why do I think his child is a boy? A natural answer might be that I remember that his name is ‘Owen’ which is usually a boy’s name. Here I’ve given information that might be part of a causal explanation of my believing that Tom’s baby is a boy. I do have such a memory and it is largely what sustains my conviction. But I haven’t given you just (...) any causally relevant information, I’ve given my grounds for my belief. I’ve given reasons that might justify me in supposing that Tom’s baby is a boy. Less naturally, the question might be taken as a request for a broader causal explanation of my holding this belief. Appropriate answers might cite all manner of facts concerning the evolution of the human race, why I chose to pursue philosophy and hence came to know Tom, the mechanisms of email transmission, the firing of various neurons, the circumstances of concept formation as a result of which I’m able to grasp the thought that Tom’s baby is a boy, and so on. It is an interesting question what distinguishes the narrower set of answers that I first suggested. I won’t pursue that here. I assume you have a good enough sense of the distinction I’m drawing. We might call the narrower set of answers justifying reasons, the kind of reasons I might cite in justifying my belief. Answers of the first sort are clearly relevant to epistemological evaluation. In assessing whether you know p or are rational in believing it to the degree you do, I will naturally want to consider what reasons you have for your belief. In deliberating myself about whether to believe p, in seeking an answer to the question of whether p, I will naturally consider what reasons or grounds I have to suppose that p. But what I want to focus on here is how explanations of the broader sort bear on such questions as whether to believe p. From a third-person perspective we can ask, ‘In assessing the epistemic status of S’s belief that p, what is the relevance of causal information that lies outside of the realm of justifying reasons?’ From the first-person standpoint we can ask ‘In seeking to answer whether p, how should such causal information affect my deliberations?’ At first it might seem that such broader causal information could have little relevance if any. The positive role of religion in reducing prejudice has remained a neglected theme in Psychology of religion, concerning itself mostly with prejudice and fundamentalism. Recently, noting the absence of a positive antithesis to prejudice and fundamentalism, faith development theory presents xenosophia as going beyond mere tolerance to a creative engagement with other religious faiths to develop new insights and broaden one’s own worldview. The current research undertakes a study of Muslim faith contents to get insights into how these beliefs shape (...) construction of self in relation to other faith communities. Conducting inductive thematic analysis of faith development interviews from 12 Muslim participants from three major religious affiliations in Pakistan in an earlier research, the research analyzes a range of xenological patterns from xenophobia to xenosophia with associated potentials for inter-faith dialogue. Focusing on residents of a country with a dense Muslim population, the study carries implications for religious socialization and religious education in a globalized world. (shrink)

Wadud analyzes citizenship in theocratic states governed by Shari’ah, or Islamic law, states which do not traditionally envisage the equality of women. Against such theocratic systems, Wadud argues that the canonical Islamic tradition of jurisprudence is open to reinterpretation in light of changing conditions in Muslim societies. According to Wadud, the Qur’an does not restrict agency on the grounds of gender. The gender disparity that has developed in Islamic tradition and theory denies women the means of completing their duties before (...) Allah. Wadud calls upon all Muslims, female and male, to reform their societies so as to implement the equality inherent in the “tawhidic” Islamic paradigm, which stresses unity and harmony. (shrink)

The following review summarizes and examines Mark Solms's article Dreaming and REM Sleep are controlled by different brain mechanisms, which argues why the understanding of REM sleep as the physiological equivalent of dreaming needs to be re-analyzed. An analysis of Solms's article demonstrates that he makes a convincing argument against the paradigmatic activation-synthesis model proposed by Hobson and McCarley and provides provocative evidence to support his claim that REM and dreaming are dissociable states. John Locke is one of the key figures in the history of early modern philosophy. He had a great influence on the development of philosophy, which is based on the empirical method, and on the political systems of countries. And even though his works have not been deprived of attention, some questions of his philosophy remain insufficiently disclosed. The author's deals with the religious turn in studies of John Loke. Attention is paid to the role of religion in the philosophy (...) of the thinker. The issue of religious influences on the issue of identity is being discussed. The essence of the term “modern” is highlighted and the criteria according to which John Locke's ideas should be considered modern are indicated. The author notes that some researchers considered John Locke one of the founders of modern philosophy, while others believed that the role of the English philosopher in the development of modern philosophy was overestimated. A characteristic feature of modern philosophy is the emphasis on research and exploration. The author names the identity of John Locke a subjective identity. The main attributes of identity are: reflection, consciousness and memory. Self-reflection and meaningful self-perception allows each person to choose their own way of worshiping God. Consciousness allows a person to evaluate his actions, forms an idea of morality, the concept of sin and so on. The article deals with different meanings of the concept of identity in the religious and secular aspects. The difference - is in memory. Personal responsibility is an integral part of identity. The author of the article notes that we should make a difference between responsibility in religious and political field. Religious responsibility requires observance not only of legal law, but also of moral rules and biblical commandments. (shrink)

Wittgenstein famously remarks that ‘the meaning of a word is its use’ (PI §43). Whether or not one views this as gesturing at a ‘theory’ of meaning, or instead as aiming primarily at dissuading us from certain misconceptions of language that are a source of puzzlement, it is clear that Wittgenstein held that for certain purposes the meaning of an expression could profitably be characterised as its use. Throughout his later writings, however, Wittgenstein’s appeal to the notion of use pulls (...) in two directions. In several places, Wittgenstein seems to connect the notion of an expression’s meaning with that of use in the sense of usage or practice. More specifically, he suggests that for an expression to possess meaning is for there to be a practice of employing it according to certain rules. ‘That’, he tells us, ‘is why there exists a correspondence between the concepts “rule” and “meaning”’ (OC §62; cf. PG 68; PO 51; RFM VI §28; VW 103). Indeed, Wittgenstein goes so far as to say, ‘The rule-governed nature of our languages permeates our life’ (RC §303). Although republican political theory has undergone something of a revival in recent years, some question its contemporary relevance on the grounds that republicanism has little to say about central questions of modern economic organization. In response, this paper offers an account of core republican values and then considers how capitalism stands in relation to these values. It identifies three areas of republican concern related to: the impact of unequal wealth distribution on personal liberty; the impact of the private control of (...) investment on popular sovereignty and pursuit of the common good; and the impact of capitalism on the level and quality of political participation. In Pakistan, as in many other societies, politico-religious movements or so-called Islamist fundamentalist movements are becoming an important site for women's activism as well as the harnessing of such activism to promote agendas that seem to undermine women's autonomy. This has become a concern for a growing feminist literature which from a variety of political and theoretical positions seeks to understand and explain the subject-position of Muslim women as politico-religious activists. This paper attempts a deconstructive reading of texts by leading (...) Pakistani feminist scholars as they attempt the difficult process of steering between fundamentalism and Orientalism in their accounts of ‘fundamentalist’ women in the political ideological space of Pakistan. Feminist scholarship on women in religious and right-wing social and political movements has moved from a reductive focus on causal or motivational factors to more sophisticated analyses explicating processes of agency and subject formation. With the aim of expanding and deepening this conceptual space, I will discuss some of my interactions with a group of women in the Jamaat-e-Islami in Pakistan, as we attempted to explore the complex meanings of ‘the modern’ that informed the self-understanding of my interviewees. My work (...) corroborates some of the contemporary scholarship on what is referred to as Political Islam in arguing that Islamist movements in Muslim societies are also the catalysts of modernization, rather than simply its interlocutors. This article argues that these processes of social and political organizing entail particular interrogations and the reconstituting of identities in ways that blur the line between ‘the religious’ and ‘the secular’. On the one hand, we need to understand Jamaat women's self-construction as religious or pious women; on the other hand, we must grasp the specificity of their claims to act as modern subjects situated in the time of political and cultural modernity. Recently in this journal, Jessica Morley and colleagues (AI & SOC 2023 38:411–423) review AI ethics and education, suggesting that a cultural shift is necessary in order to prepare students for their responsibilities in developing technology infrastructure that should shape ways of life for many generations. Current AI ethics guidelines are abstract and difficult to implement as practical moral concerns proliferate. They call for improvements in ethics course design, focusing on real-world cases and perspective-taking tools to immerse students in challenging (...) situations with relatable personal, social and environmental implications. The present paper considers the ACTWith model of moral cognition for use in such training contexts. Morley and colleagues' paper and contemporary literature are reviewed. Whistleblowing as a source for relatable case studies is discussed, and the purpose of ethics education considered. The ACTWith model is introduced, and correlations with contemporaries noted. The ACTWith model has been successfully employed in a broad range of technology ethics courses in the Netherlands and South Korea. Illustrations are offered. Correlations with contemporary efforts are drawn in the context of software engineering. Summary discussion considers strengths and limitations of the ACTWith program. Overall, the paper emphasizes training high-level moral reasoning through guided inquiry education informing lifetime professional practice, and indicates an outstanding need for teachers able to do the job. In recent decades, a new scientific approach to understand, explain, and predict many features of religion has emerged. The cognitive science of religion has amassed research on the forces that shape the tendency for humans to be religious and on what forms belief takes. It suggests that religion, like language or music, naturally emerges in humans with tractable similarities. This new approach has profound implications for how we understand religion, including why it appears so easily, and why people are willing (...) to fight-and die for it. Whether hailed for transitioning to the ballot box, or condemned for failing to hold elections, Africa’s postcolonial states exhibit profound contradictions in the arena of gender politics. Where reforms have been achieved, implementation remains minimal, as undemocratic state structures and uncivil societies alike lack the political will to change. This article addresses the emergence of feminism as an intellectual and political force for freedom that radically challenges the ongoing exploitation and oppression of women in Africa. Because anticipated and retrospective regret play important roles in practical deliberation and motivation, better understanding them can illuminate the contours of human agency. However, the possibility of self-ignorance and the fact that we change over time can make regret—especially anticipatory regret—not only a poor predictor of where the agent will be in the future but also an unreliable indicator of where the agent stands. Granting these, this paper examines the way in which prospective and, particularly, retrospective regret can nevertheless yield (...) important insight into the sorts of creatures we are, both generally and individually. The experience of retrospective regret can show a person she values something in a way she did not know or that she is (or was) a different person than she had thought, insights which can factor into forward-looking, or prospective, deliberation. Such instances of revelatory regret reveal something to the agent about herself as agent. I examine two cases of agential self-ignorance. In the first, the experience of regret reveals what the agent values, not only to others but even to the agent himself. In the second, the agent anticipates experiencing regret for an action but does not experience the regret, suggesting that the agent did not value the rejected alternative in the way she thought. Anticipatory regret is forward-looking and can play an important role in practical deliberation. But insofar as anticipatory regret flows from one’s imperfect judgment and prospection about oneself, retrospective regret can be an important corrective in helping the agent understand her own standpoint.
