BackgroundIn the Canadian Alliance for Healthy Hearts and Minds (CAHHM) cohort, participants underwent magnetic resonance imaging (MRI) of the brain, heart, and abdomen, that generated incidental findings (IFs). The approach to managing these unexpected results remain a complex issue. Our objectives were to describe the CAHHM policy for the management of IFs, to understand the impact of disclosing IFs to healthy research participants, and to reflect on the ethical obligations of researchers in future MRI studies.MethodsBetween 2013 and 2019, 8252 participants (...) (mean age 58 ± 9 years, 54% women) were recruited with a follow-up questionnaire administered to 909 participants (40% response rate) at 1-year. The CAHHM policy followed a restricted approach, whereby routine feedback on IFs was not provided. Only IFs of severe structural abnormalities were reported.ResultsSevere structural abnormalities occurred in 8.3% (95% confidence interval 7.7–8.9%) of participants, with the highest proportions found in the brain (4.2%) and abdomen (3.1%). The majority of participants (97%) informed of an IF reported no change in quality of life, with 3% of participants reporting that the knowledge of an IF negatively impacted their quality of life. Furthermore, 50% reported increased stress in learning about an IF, and in 95%, the discovery of an IF did not adversely impact his/her life insurance policy. Most participants (90%) would enrol in the study again and perceived the MRI scan to be beneficial, regardless of whether they were informed of IFs. While the implications of a restricted approach to IF management was perceived to be mostly positive, a degree of diagnostic misconception was present amongst participants, indicating the importance of a more thorough consent process to support participant autonomy.ConclusionThe management of IFs from research MRI scans remain a challenging issue, as participants may experience stress and a reduced quality of life when IFs are disclosed. The restricted approach to IF management in CAHHM demonstrated a fair fulfillment of the overarching ethical principles of respect for autonomy, concern for wellbeing, and justice. Ensuring that life-saving antimicrobials remain available as effective treatment options in the face of rapidly rising levels of antimicrobial resistance will require a massive and coordinated global effort. Setting a collective direction for progress is the first step towards aligning global efforts on AMR. This process would be greatly accelerated by adopting a unifying global target — a well-defined global target that unites all countries and sectors. The proposed pandemic instrument — with its focus on prevention, preparedness and response — (...) represents an ideal opportunity to develop and adopt a unifying global target that catalyzes global action on AMR. We propose three key characteristics of a unifying global target for AMR that — if embedded within the pandemic preparedness instrument — could rally public support, funding, and political commitment commensurate with the scale of the AMR challenge. In two chapters of Enn. II.9 [33], Plotinus discusses the Gnostic idea that the creation of the world is due to the ‘decline’ of a principle that he variously calls Soul or Sophia. The identity of Plotinus' Gnostics is notoriously difficult to establish with any degree of precision; I can only note here that the idea of Sophia's ‘decline’ features in a number of extant Gnostic texts, such as those from Nag Hammadi and the Berlin Codex, as a recent survey (...) of the evidence by Poirier has demonstrated. Assimilé à la French Theory, Jean Baudrillard a été aussi célèbre, ou presque, sur les campus américains que Foucault, Derrida, Deleuze, Guattari ou Lacan. Mais il est loin d'avoir aujourd'hui leur diffusion mondiale. Il a même presque totalement disparu des écrans radars. Officiellement sociologue, aucun sociologue ne le cite, aucun étudiant de sociologie ne le lit. Il faut dire qu'il a tout fait pour brouiller les pistes, en se refusant à tout simulacre de réalisme pour mieux tenter de prendre la (...) réalité de vitesse, jusqu'à annoncer son évanescence dans l'hyper-réalité du virtuel. Sa pensée, fulgurante, attirait étrangement, mais on pouvait croire que cette séduction fatale tenait à un goût tout pataphysicien des paradoxes. Or, force est de se demander s'il n'avait pas au bout du compte vu plus juste et mieux anticipé que tous ses contemporains ce qu'est désormais devenu notre monde. Ce volume, qui réunit sociologues, philosophes, économistes, théoriciens de l'esthétique et des médias, permet de prendre la mesure de son œuvre inclassable. On reconnait en Castoriadis l'un des principaux penseurs politiques alternatifs contemporains, toujours actuel. Mais on connaît sans doute moins le lien étroit qui existe entre ses analyses politiques et ses prises de position philosophiques. Or, elles s'éclairent réciproquement. Le refus de subordonner la raison pratique à la raison théorique permet à Castoriadis de penser l'idée de création. Sa rupture avec Marx, auquel il reproche son ontologie déterministe, l'amène à repenser l'émancipation politique et sociale à partir de la notion d'imaginaire créateur. (...) Pour bien comprendre cette notion, il faut la confronter avec les conceptions de l'imagination défendues notamment par la phénoménologie, particulièrement Sartre et Husserl. Elle débouche sur le concept de chaos, dont il ressort que l'action humaine repose sur un désordre pour une part irréductible, ce qui explique qu'on ne puisse fonder celle-ci, pas plus que la démocratie, en référence à une légitimité indiscutable. Cherchant à tisser un lien entre la pensée, l'oeuvre et la vie d'un homme engagé dans son temps, ce livre trace un parcours à travers les thèmes centraux de la pensée philosophique et politique de Castoriadis, qu'il met en rapport avec d'autres approches contemporaines : Maurice Merleau-Ponty, Claude Lefort, Richard Rorty, Vincent Descombes, ou encore Michel Foucault à propos de l'éthique du sujet. Expanding the Critical Animal Studies Imagination: Essays in Solidarity and Total Liberation pushes critical animal studies forward and outward by making new connections to movements and ideas that have been little engaged with in publication to the present. This book challenges critical animal studies adherents to expand their efforts of solidarity, mutual aid, and activism. Contributors to this volume extend invitations to those not familiar with critical animal studies to welcome them in with gestures of solidarity towards total liberation. Expanding (...) the Critical Animal Studies Imagination does not shy away from controversial topics but critically engages with them using care and tact. This is not controversy for the sake of being provocative, but not being afraid to target root causes of oppression. Je voulais écrire pour raconter ce que je voyais, pour rendre compte d'événements dont j'étais le témoin. L'essentiel était de prendre la route, d'explorer quelque chose qui n'avait été qu'entrevu ou d'en parler d'une manière originale. (...) Sartre a raison de dire qu'on rate toujours sa vie, qu'on ne devient jamais celui ou celle qu'on aspirait à devenir. Les points d'arrivée ne sont pas forcément des points d'aboutissement. On croit achever quelque chose alors qu'on a creusé une brèche dans son devenir. Les crises, existentielles ou intellectuelles, peuvent avoir du bon. Elles réveillent le sens de la révolte, elles aiguisent l'envie d'un nouveau départ, elles invitent à d'autres commencements. Ce livre est le récit d'une tentative pour inaugurer dans ma quelque chose de neuf en la racontant. Un écrivain n'est pas quelqu'un qui vit de son écriture.
