Ensuring that life-saving antimicrobials remain available as effective treatment options in the face of rapidly rising levels of antimicrobial resistance will require a massive and coordinated global effort. Setting a collective direction for progress is the first step towards aligning global efforts on AMR. This process would be greatly accelerated by adopting a unifying global target — a well-defined global target that unites all countries and sectors. The proposed pandemic instrument — with its focus on prevention, preparedness and response — (...) represents an ideal opportunity to develop and adopt a unifying global target that catalyzes global action on AMR. We propose three key characteristics of a unifying global target for AMR that — if embedded within the pandemic preparedness instrument — could rally public support, funding, and political commitment commensurate with the scale of the AMR challenge. It has been theoretically proposed that employees’ perceptions of their supervisor social power in the organization entail a potential to influence their beliefs, attitudes, and behaviors. However, no study has investigated such potential. This lack of research stems from the absence of a common understanding around the meaning of perceived supervisor social power and the absence of any validated measure. Therefore, the purpose of this article is to establish PSSP definition and to validate a five-item scale to measure this construct. (...) Three studies encompassing four independent samples of employees from three different countries and three different languages were conducted to assess the validity of PSSP. Results showed that responses to the PSSP scale presented excellent psychometric properties. Furthermore, the structure of the proposed five-item measure of PSSP was found to be invariant across four samples. Finally, PSSP nomological validity was assessed. Study 1 and Study 2 showed that PSSP was positively related to affective organizational commitment. All three studies showed that PSSP acted as a positive moderator of the relation between affective commitment to the supervisor and affective organizational commitment. Together, these studies support the psychometric soundness of the PSSP scale and presented the first evidence of its potential to influence followers. Implications of these findings for future research on supervisor social power are discussed. (shrink)

The standard medical case report often reduces patients and caregivers to complexes of medical facts and clinical decisions. Restructuring the genre itself to acknowledge the human dimensions of both patients and physicians allows questions of human values to regain their stature as integral components of the discourse.

The medical record, as a managerial, historic, and legal document, serves many purposes. Although its form may be well established and many of the cases documented in it routine in medical experience, what is written in the medical record nevertheless records decisions and actions of individuals. Viewed as an interpretive text, it can itself become the object of interpretation. This essay applies literary theory and methodology to the structure, content, and writing style(s) of an actual medical record for the purpose (...) of exploring the relationship between the forms and language of medical discourse and the daily decisions surrounding medical treatment. The medical record is shown to document not only the absence of a consistent treatment plan for the patient studied but also a breakdown in communication between different health professionals caring for that patient. The paper raises questions about the kind of education being given to house staff in this instance. The essay concludes with a consideration of how such situations might be more generally avoided. BackgroundIn the Canadian Alliance for Healthy Hearts and Minds (CAHHM) cohort, participants underwent magnetic resonance imaging (MRI) of the brain, heart, and abdomen, that generated incidental findings (IFs). The approach to managing these unexpected results remain a complex issue. Our objectives were to describe the CAHHM policy for the management of IFs, to understand the impact of disclosing IFs to healthy research participants, and to reflect on the ethical obligations of researchers in future MRI studies.MethodsBetween 2013 and 2019, 8252 participants (...) (mean age 58 ± 9 years, 54% women) were recruited with a follow-up questionnaire administered to 909 participants (40% response rate) at 1-year. The CAHHM policy followed a restricted approach, whereby routine feedback on IFs was not provided. Only IFs of severe structural abnormalities were reported.ResultsSevere structural abnormalities occurred in 8.3% (95% confidence interval 7.7–8.9%) of participants, with the highest proportions found in the brain (4.2%) and abdomen (3.1%). The majority of participants (97%) informed of an IF reported no change in quality of life, with 3% of participants reporting that the knowledge of an IF negatively impacted their quality of life. Furthermore, 50% reported increased stress in learning about an IF, and in 95%, the discovery of an IF did not adversely impact his/her life insurance policy. Most participants (90%) would enrol in the study again and perceived the MRI scan to be beneficial, regardless of whether they were informed of IFs. While the implications of a restricted approach to IF management was perceived to be mostly positive, a degree of diagnostic misconception was present amongst participants, indicating the importance of a more thorough consent process to support participant autonomy.ConclusionThe management of IFs from research MRI scans remain a challenging issue, as participants may experience stress and a reduced quality of life when IFs are disclosed. The restricted approach to IF management in CAHHM demonstrated a fair fulfillment of the overarching ethical principles of respect for autonomy, concern for wellbeing, and justice. In an earlier article [Found. Phys. 30, 1191 (2000)], a quasiclassical phase space approximation for quantum projection operators was presented, whose accuracy increases in the limit of large basis size (projection subspace dimensionality). In a second paper [J. Chem. Phys. 111, 4869 (1999)], this approximation was used to generate a nearly optimal direct-product basis for representing an arbitrary (Cartesian) quantum Hamiltonian, within a given energy range of interest. From a few reduced-dimensional integrals, the method determines the optimal 1D marginal Hamiltonians, (...) whose eigenstates comprise the direct-product basis. In the present paper, this phase space optimized direct-product basis method is generalized to incorporate non-Cartesian coordinate spaces, composed of radii and angles, that arise in molecular applications. Much attention has been paid recently to the two sixteenth-century translations and their intended readers—royal and bourgeois respectively. The three French translations of the seventeenth and eighteenth centuries are less known but all the more intriguing. In 1642 Folly addresses herself to the French pre-classicist readers, adepts of Richelieu’s new Académie Française—although her (...) translator, Hélie Poirier, was a Protestant refugee, recently settled in the Netherlands. In 1671 Folly seeks her readers in the Parisian salons, satirizing the same societal wrongs as her great contemporary Molière in Tartuffe and Les femmes savantes. The successful translation by Nicolas Gueudeville is also a chameleon: originally translated and printed in Leiden, the text gradually becomes more Parisian with each passing edition. Folly’s language is bowdlerized according to the principles of bienséance, and Vianen’s illustrations, based on Holbein, are discarded as rude and old-fashioned. In 1751 they are replaced by Charles Eisen’s elegant, long-limbed, periwigged figures, dressed to the latest fashion. Although she changes her name, her language, her appearance and attire, Folly remains much the same through the ages—everlasting and omnipresent, just as the vices she laughs at. (shrink)

On reconnait en Castoriadis l'un des principaux penseurs politiques alternatifs contemporains, toujours actuel. Mais on connaît sans doute moins le lien étroit qui existe entre ses analyses politiques et ses prises de position philosophiques. Or, elles s'éclairent réciproquement. Le refus de subordonner la raison pratique à la raison théorique permet à Castoriadis de penser l'idée de création. Sa rupture avec Marx, auquel il reproche son ontologie déterministe, l'amène à repenser l'émancipation politique et sociale à partir de la notion d'imaginaire créateur. (...) Pour bien comprendre cette notion, il faut la confronter avec les conceptions de l'imagination défendues notamment par la phénoménologie, particulièrement Sartre et Husserl. Elle débouche sur le concept de chaos, dont il ressort que l'action humaine repose sur un désordre pour une part irréductible, ce qui explique qu'on ne puisse fonder celle-ci, pas plus que la démocratie, en référence à une légitimité indiscutable. Cherchant à tisser un lien entre la pensée, l'oeuvre et la vie d'un homme engagé dans son temps, ce livre trace un parcours à travers les thèmes centraux de la pensée philosophique et politique de Castoriadis, qu'il met en rapport avec d'autres approches contemporaines : Maurice Merleau-Ponty, Claude Lefort, Richard Rorty, Vincent Descombes, ou encore Michel Foucault à propos de l'éthique du sujet. Assimilé à la French Theory, Jean Baudrillard a été aussi célèbre, ou presque, sur les campus américains que Foucault, Derrida, Deleuze, Guattari ou Lacan. Il faut dire qu'il a tout fait pour brouiller les pistes, en se refusant à tout simulacre de réalisme pour mieux tenter de prendre la (...) réalité de vitesse, jusqu'à annoncer son évanescence dans l'hyper-réalité du virtuel. Sa pensée, fulgurante, attirait étrangement, mais on pouvait croire que cette séduction fatale tenait à un goût tout pataphysicien des paradoxes. Or, force est de se demander s'il n'avait pas au bout du compte vu plus juste et mieux anticipé que tous ses contemporains ce qu'est désormais devenu notre monde. Ce volume, qui réunit sociologues, philosophes, économistes, théoriciens de l'esthétique et des médias, permet de prendre la mesure de son œuvre inclassable. Contributors to this volume extend invitations to those not familiar with critical animal studies to welcome them in with gestures of solidarity towards total liberation. This is not controversy for the sake of being provocative, but not being afraid to target root causes of oppression. Si, aujourd'hui, la démocratie fait, encore et toujours, question, peu d'oeuvres comparables à celles de Cornelius Castoriadis (1922-1997) et de Claude Lefort (1924-2010) permettent de circonscrire avec plus de profondeur et de radicalité ce questionnement. Sous les regards de la philosophie, de la sociologie, de l'histoire et de la science politique, cet ouvrage propose d'interroger le travail de réflexion entrepris par ces deux grandes figures de la philosophie politique contemporaine à la fois en commun, dans le cadre de Socialisme ou (...) Barbarie, puis de manière indépendante par la suite. Pour l'essentiel, le travail de Castoriadis et de Lefort aura principalement consisté à redonner sens à la notion de démocratie, que l'on ne doit pas entendre uniquement en tant que régime et institution politique, ou même en tant que réalité sociologique, mais bien plutôt en tant qu'expérience par laquelle les hommes s'emploient à transformer les conditions de leur existence commune. Si les convergences entre ces auteurs sont profondes, des différences se marquent aussi entre une pensée de la démocratie comme auto-institution explicite de la société et une conception de l'agir démocratique soucieuse de mettre avant tout l'accent sur le fait de la division sociale, et suggérant davantage de contester le pouvoir, en revendiquant contre lui des droits, qu'à chercher à en démocratiser l'exercice effectif Pour autant, l'un comme l'autre invitent à envisager la perspective de l'émancipation à la manière d'un processus conflictuel dont il est impossible de connaître le terme et qui n'a d'autre fondement que le désir de liberté. The contrast between third- and first-personal accounts of the experiences of autistic persons has much to teach us about epistemic injustice and epistemic agency. This paper argues that bringing about greater epistemic justice for autistic people requires developing a relational account of epistemic agency. We begin by systematically identifying the many types of epistemic injustice autistic people face, specifically with regard to general assumptions regarding autistic people’s sociability or lack thereof, and by locating the source of these epistemic injustices in (...) neuronormativity and neurotypical ignorance. We then argue that this systematic identification pushes us to construe epistemic agency as resulting from a fundamentally relational and dynamic process between an individual, others around them, and their social, cultural, or institutional environment, rather than as a fixed and inherent property of individuals. Finally, we show how our relational account of epistemic agency allows us to introduce the novel concepts of epistemic disablement and epistemic enablement. We argue that these two concepts allow us to more accurately track the mechanisms that undermine or facilitate epistemic agency, and thereby to better understand how epistemic injustice arises and to design more effective interventions to foster greater epistemic justice for autistic people. What happens when someone acts? A familiar answer goes like this. There is something that the agent wants, and there is an action that he believes conducive to its attainment. His desire for the end, and his belief in the action as a means, justify taking the action, and they jointly cause an intention to take it, which in turn causes the corresponding movements of the agent's body. I think that the standard story is flawed in several respects. The flaw (...) that will concern me in this paper is that the story fails to include an agent-or, more precisely, fails to cast the agent in his proper role. (shrink)

This paper aims to show that genders are enacted, by providing an account of how an individual can be said to enact a gender and explaining how, consequently, genders can be fluid. On the enactive-ecological view we defend, individuals first and foremost perceive the world as fields of affordances, that is, structured sets of action possibilities. Fields of natural affordances offer action possibilities because of the natural properties of organisms and environments. Handles offer graspability to humans because of physical-structural properties (...) of handles and the anatomical-physiological properties of humans. Although humans live in fields of bodily, action, and cultural affordances, our work focuses on cultural affordances, where action possibilities are offered to individuals because of the normative responses of individuals in that culture. Knocking on a door affords entrance because knocking provides cultured individuals on the other side of the door an affordance to which they themselves behave normatively. Usually, behaving normatively in response to cultural affordances brings about sequences of perception-action loops, which we will call “scripts”: for instance, closed doors afford knocking, which affords the individual inside opening the door, which affords an interpersonal meeting, which afford entrance. Although the notion of script has a strong cognitivist flavor, one of the aims of the paper to provide an ecological account of scripts, to show that what cognitivists viewed as representations are in fact environmentally structured perception-action loops. On our account of gender, gendered cultures build and maintain gendered cultural affordance landscapes, that is, landscapes in which the action possibilities individuals face are normed according to a specific body type or situation; most often biological sex. Individuals enact a given gender when they come to perceive the affordances reserved for one gender by their culture and respond in the culturally normative way, thus enacting gendered sequences of perception-action loops. With the shifting landscapes of cultural affordances brought about by several recent social, technological, and epistemic developments in some cultures, the gendered landscapes of affordances offered to individuals in these cultures have become more varied and less rigid, thus increasing the variety and flexibility of scripts individuals can enact. This entails that individuals in such cultures have an increased possibility for gender fluidity, which may in part explain the increasing number of people currently identifying outside the binary. We present and contrast two accounts of cooperative communication, both based on Active Inference, a framework that unifies biological and cognitive processes. The mental alignment account, defended in Vasil et al., takes the function of cooperative communication to be the alignment of the interlocutor's mental states, and cooperative communicative behavior to be driven by an evolutionarily selected adaptive prior belief favoring the selection of action policies that promote such an alignment. We argue that the mental alignment account should be rejected (...) because it neglects the action-oriented nature of cooperative communication, which skews its view of the dynamics of communicative interaction. We introduce our own conception of cooperative communication, inspired by a more radical ecological interpretation of the active inference framework. Cooperative communication, on our ecological conception, serves to guide and constrain the dynamics of the cooperative interaction via the construction and restructuring of shared fields of affordances, in order to reach the local goals of the joint actions in which episodes of cooperative communication are embedded. We argue that our ecological conception provides a better theoretical standpoint to account for the action-oriented nature of cooperative communication in the active inference framework. (shrink)
