IntroductionThe objectives of this study are to assess and compare differences in the intensity, frequency, and overall severity of moral distress among a diverse group of healthcare professionals.MethodsParticipants from within Baylor Health Care System completed an online seven-point Likert scale (range, 0 to 6) moral distress survey containing nine core clinical scenarios and additional scenarios specific to each participant’s discipline. Higher scores reflected greater intensity and/or frequency of moral distress.ResultsMore than 2,700 healthcare professionals responded to the survey (response rate 18.14 (...) percent); survey respondents represented multiple healthcare disciplines across a variety of settings in a single healthcare system. Intensity of moral distress was high in all disciplines, although the causes of highest intensity varied by discipline. Mean moral distress intensity for the nine core scenarios was higher among physicians than nurses, but the mean moral distress frequency was higher among nurses. Taking into account both intensity and frequency, the difference in mean moral distress score was statistically significant among the various disciplines. Using post hoc analysis, differences were greatest between nurses and therapists.ConclusionsMoral distress has previously been described as a phenomenon predominantly among nursing professionals. This first-of-its-kind multidisciplinary study of moral distress suggests the phenomenon is significant across multiple professional healthcare disciplines. Healthcare professionals should be sensitive to situations that create moral distress for colleagues from other disciplines. Policy makers and administrators should explore options to lessen moral distress and professional burnout that frequently accompanies it. (shrink)

Machine learning algorithms may radically improve our ability to diagnose and treat disease. For moral, legal, and scientific reasons, it is essential that doctors and patients be able to understand and explain the predictions of these models. Scalable, customisable, and ethical solutions can be achieved by working together with relevant stakeholders, including patients, data scientists, and policy makers.

Moral theorists and game theorists are both interested in situations where rational agents are to constrain their future actions and co-operate with others instead of being free riders. These theorists have constructed a variety of hypothetical games which illuminate this problem of constraint. In this paper, I draw a distinction between like the Newcomb paradox and like Kavka's toxin puzzle, a prisoner's dilemma and Parfit's hitchhiker example. I then employ this distinction to argue that agents who subscribe to the orthodox (...) theory of rationality do significantly better in disposition games than those who subscribe to revisionist theories like David Gauthier's, while revisionist agents do marginally better in behaviour games. I argue that because of agents' ability to manipulate their own weakness of will, orthodox agents do better at all of these games than has previously been thought. And, by elucidating the distinction between behaviour games and disposition games, I uncover the virtues that underlie the success of each theory of rationality. (shrink)

Almost uniquely for someone whose thought has been so influential, Socrates wrote nothing himself, and our knowledge of his philosophical opinions and method is derived mainly from the engaging and infuriating figure who appears in Plato's dialogues. The philosophy of Socrates and Plato is therefore closely interconnected, and the most powerful elements of Plato's mature thought form the basis of an interpretation of knowledge, reality, and morality which is still held and debated by philosophers today. An adequate moral theory must provide for cooperation and trust between moral agents, but a tension exists between cooperating and maximizing. This tension is prominent in utilitarianism and contractarianism . Various puzzles illustrate this tension, and both utilitarians and contractarians must solve these to present a coherent moral theory. These puzzles include new and resilient versions of classic objections to utilitarianism, such as the claims that utilitarian agents cannot be trusted to keep promises and cannot take rights seriously. They also (...) include the contractarian puzzle of why rational agents would cooperate by making and keeping impartial contracts. In solving these problems, utilitarians and contractarians alike must choose between traditional and revisionary maximizing theories. ;In light of these problems: David Gauthier urges contractarians to appeal to a revisionary theory of rationality, Donald Regan has developed a new variety of utilitarianism, and many other theorists still argue for the revisionary theory of rule utilitarianism. These people underestimate the potential of traditional theories. Although traditional theories like act utilitarianism look only to the future--while revisionary theories sometimes take prior acts such as promises, deliberations, and intentions to justify action--traditional theories are still able to solve these puzzles and furnish a satisfactory account of cooperation and trust. Surprisingly, even though these puzzles were developed as objections to the traditional theories and the revisionist theories were designed to deal with these puzzles, the traditional theories provide superior solutions. ;This defense of traditional theories has broad implications in philosophy and elsewhere, but I specifically discuss two significant implications for modern moral theory and two implications for applied ethics. I reject the criticism that act utilitarianism fails on its own terms and argue that it has previously unnoticed advantages over rule utilitarianism. I also reformulate Gauthier's moral theory using the traditional theory of rationality, thereby answering a major criticism of his theory. In addition, I apply my understanding of traditional maximizing theories to the utilitarian theory of rights and then to the strategic problems of nuclear deterrence and of threats in general. As early feminist scholars note, male privilege renders women’s lives invisible.4 But male privilege renders aspects of men’s lives invisible, too. In this article we highlight how institutional review boards (IRBs) in universities and hospitals erect barriers to research that render particular aspects of men’s lives invisible. We draw on our experiences of submitting ten applications for a series of methodologically diverse studies of masculinity to eight IRBs. First, we argue that, despite their original function of protecting subordinated groups, IRBs are gendered institutions in which members base their decisions on culturally dominant, normative images of women and men. We show how this results in the stringent protection of male research participants, the safeguarding of participants’ personal masculinity, and the shielding of men’s lives from social inquiry. Second, we argue that IRBs are gendering institutions. Our experiences reveal how the privileging and protection of masculinity results in mandatory protocol modifications that idealize hegemonic masculinity. These modifications require researcher-participant interactions that socialize men to engage in gender “appropriate” behavior and reify gendered expectations. Third, our experiences reveal the ways in which IRBs protect the institution itself and privilege the universities, hospitals, and (predominantly male) medical doctors they represent. Consequently, the ways they reproduce social inequality remain impervious to academic inquiry. Our gender identity is our sense of ourselves as a woman, a man, as genderqueer, or as another gender. Our gender is the property we have of being a woman, being a man, being non-binary, or being another gender. What is the relationship between our gender identity and our gender? Recently, much work has been done on ameliorative accounts of the gender concepts that we should accept and on the metaphysics of gender properties. From this work 4 views of the (...) relationship between having gender identity G (e.g. having a female gender identity) and being a gender G (e.g. being a woman) have emerged. A first, the no connection view, says that gender identity and gender are entirely distinct; Sally Haslanger’s ameliorative account of gender, as well as Theodore Bach’s account of gender properties, are instances of this view. A second, the gender-identity-first view, says that all there is to our gender is our gender identity; this view is at least similar to views that Talia Bettcher and R.A. Briggs and B.R. George have argued for. A third, contextualist view, says that in some contexts and relative to certain standards, our gender identity determines our gender, and that in others it does not; Ásta and Robin Dembroff have proposed and defended views along these lines. A fourth view, the two gender properties view, says that there are two properties that can make it the case that one is gender G (in any context), one’s having gender identity G and one’s being socially classed as a G; Katharine Jenkins and Elizabeth Barnes have proposed views along these lines. Aesthetics in philosophy of mathematics is too narrowly construed. Beauty is not the only feature in mathematics that is arguably aesthetic. While not the highest aesthetic value, being interesting is a sine qua non for publishability. Of the many ways to be interesting, being explanatory has recently been discussed. The motivational power of what is interesting is important for both directing research and stimulating education. The scientific satisfaction of curiosity and the artistic desire for beautiful results are complementary but both (...) aesthetic. (shrink)

Explanations in behavioural neuroscience are often said to be mechanistic in the sense that they explain an organism’s behaviour by describing the activities and organisation of the organism’s parts that are “constitutively relevant” to organism behaviour. Much has been said about the constitutive relevance of working parts (in debates about the so-called “mutual manipulability criterion”), but relatively little has been said about the constitutive relevance of the organising relations between working parts. Some New Mechanists seem to endorse a simple causal-linking (...) account: organising relations are constitutively relevant to organism behaviour if and only if (and because) they are causal relations that link the working parts that are constitutively relevant to organism behaviour. In this paper, I argue that the causal-linking account is inadequate because it neglects the constitutive relevance of anatomical relations that organise the working parts of a behaving organism. I demonstrate this by considering a case study where the anatomical organisation of the barn owl (Tyto alba) is constitutively relevant to their mechanism for sound localization. The anatomical organisation of this mechanism is best understood as the back-and-forth flow of task information across 7 “levels of anatomy” (a notion that I distinguish from levels of mechanism). A further implication, I conclude, is that at least some of the interlevel structure of neuroscientific explanation is accounted for by levels of anatomy, not levels of mechanism. (shrink)

Just as in our world, some people or groups would have more power than others, and some of these might have more power than most. The Emperor would still be subject to the demands of the Spacing Guild, for example, while the Spacing Guild would still need to be on good terms with the governor of Arrakis because of (...) the Guild's absolute dependence upon spice. In a world like this, it would be difficult not to see potential threats around every corner, and with good reason. And in this state of wariness, political and social stability will be tough to attain. The eminent philosopher and political theorist Hannah Arendt once attended a similar trial with a similar plea: the 1961 trial of the mid‐level Nazi official Adolf Eichmann. She portrayed him as an exemplar of what she termed the banality of evil. After his capture in 1960, Eichmann was tried on charges including war crimes and crimes against humanity. Eichmann was an exemplary case of the thoughtlessness and lack of self‐reflection that goes into setting unthinkable atrocities into motion. Like Eichmann, Mao (...) doesn't cut the figure of a hardline fanatic, blinded by ideology. Mao's unwillingness to place the cause ahead of his own desires should indicate that he isn't wholly blinded by fanaticism, as Dresden is. In her characterizations of Eichmann, Arendt noted his odd lack of imagination, his inability, in other words, “to think from the standpoint of someone else.”. (shrink)

To meet a dilemma between the axiomatic theory of demonstrative science in Posterior analytics and the non-axiomatic practice of demonstrative science in the physical treatises, Jonathan Barnes has proposed that the theory of demonstration was not meant to guide scientific research but rather scientific pedagogy. The present paper argues that far from contributing directly to oral instruction, the axiomatic account of demonstrative science is a model for the written expression of science.
